    Dev asks:
My just-turned 18-yr old was diagnosed with OCD in his knee at age 11. After a year of rest and PT, the doctor released him for full activity. He has had occasional swelling but no real issues until last week when he felt something catch in his knee. He could feel something moving around. Another X-ray and MRI showed a 3.5cm x 2.5cm crater at the end of the femur. The doctor wants to perform a bone graft. My question is, can this be put off until after soccer season so he can play his senior year or not? We are in the middle of recruiting and about to start high school soccer season. What are the risks of continuing to play soccer?
Osteochondritis dissecans is a problem that affects mainly young athletes, but occasionally the residual effects of an osteochondritis dissecans lesion can be seen in an adult. The lesion involves the bone under the articular cartilage in part of the knee (almost always the lateral aspect of the medial femoral condyle) starting to die. Early on the cartilage over this diseased bone is intact, but it can later break off with the underlying bone and float freely within the knee joint.
In young athletes who are not finished growing, there is often a good chance that the lesion will heal on its own. Treatment in this situation often involves making the athlete nonweightbearing for many weeks to try to decrease stress on the lesion.
If the lesion has broken away, nonsurgical treatment is not indicated. Surgery to try to put the lesion back in place and hold it with screws or pins can occasionally be attempted successfully. Other times it is a free-floating piece of bone and cartilage that is removed and filled with cylinders of bone and cartilage from the athlete’s knee or from a donor.
In this video, I explain the risk involved with delaying surgery and continuing to play sports. There are many variables to consider with osteochondritis dissecans lesions, so it is important to discuss your options with your orthopedic surgeon.

I recently just finished fully recovering from my OCD of the knee. I was diagnosed with it in my right knee in July of 2017 and had to have two surgeries. One was knee arthroscopy to take out the pieces of one stuck inside my knee and the other was to place a graft in my knee. I had to miss my junior year of soccer varsity but a year later, I’m playing my senior year as a staring senior back. I got cleared during the summer and wow it feels amazing to be back!

I was diagnosed with OCD in my right knee when i was around the age of 10. I didn't play sports or run for over a year and when the orthopedic surgeon said it was finally starting to heal and after 3 MRIs, i was abe to return to sport as long as there was no pain. Now, i am 14 and for the last three months i have had pain when sitting and laying down and also sometimes a stabbing pain when walking. when i fully straighten my leg it often hurts and when i squat. now, it is hurting in both knees and i am worried it never fully healded and i have got it in both knees.

I had OCD repaired in one of my knees when I was 17 with a bone graft the recovery period was very long, I actually did the injury when I was 15 playing basketball in those two years I still played tennis, basketball and touch footy and it never worried me. Last month I did the same thing but I'm my other knee so I'm off to get surgery again. This time I felt it going throughout the day when I was playing tennis it felt like I had an airlock and it just needed to crack but when it did crack it hurt a lot and I couldn't get my leg straight for a week. This time I can't play sport because when I did try it caught and I couldn't get my leg straight straight again for 3 days so I will have to wait until it is repaired.

I fell down 3 months ago on my right knee and my knee cap was popped out of place. I rubbed it back in place and it was so swollen. I called my doctor and they said it’s okay if I could walk on it and straighten my leg without locking to treat it as a sprained knee using the RICE method. It didn’t get better so I went in and they said it was a mild case of bursitis. Time passed and the swelling went down but I got a new job standing all day and the knee was aching badly so I got X-rays and they said OCD in the knee cap. Now I’m waiting f for my MRI

I've played rugby for 3 years now and this is my senior year. I'm 5'7 151lbs. I can run, cut , and all. But, knee has a dull pain once I'm done play/practicing and with fatigue. And I have some pain when I try to get to much extension when running. But during school I have virtually no pain when walking normally. It does pop quite often when I straighten it really hard. My doctor is recommending surgery. Should I try to make it through the season or go with the surgery? I previously played an entire season with this injury with limited problems.

I have this in both of my knees. I was born with dislocated hips and as I got older, my left leg hurt a lot. I got crutches and then a year later, a leg brace. A few months later, I was diagnosed with OCD in my right leg too because I was using that leg to keep the weight off my other leg. Since I was around 9-10 years old, I could not be operated on, due to my leg plate not being closed over. I'm thirteen now and I'm going for a checkup to see if my plate has closed over. If it has, I can be booked for an operation. In school, I get bullied for my condition. They call me faker because I had crutches. It's really hard tho, they don't understand.

When I was 10 years old I started having serious pain in both of my knees. I went to L.I. Jewish Hosp and after x-rays and a thorough going over by several Doctors I was told I have Bi-lateral Osteochondritis Dissecans. At 18 years of age I was told that the bone had already broken off into the knee joint and they wanted to do a knee operation where they drill the missing bone area to encourage blood and bone growth and they scrapped it down and put me in a cast for 12 weeks non-weight bearing to allow the subchondral cartilage to grow and fill in what was going to otherwise become a flat spot on the head of my femur. I had problems with pain my whole life and at 12 years old I had to stop participating in sports as I would pay too big of a price in pain after any stressful activities in my knees. At 30 years old I had 2 High Tibial Osteotomies and that helped me for 5 years. At 35 years old I had my first set of TKR's and they lasted 10 years. That was 10 good years of raising my children and running on the soccer field with them and even joining my local Fire Department. They broke loose after 10 years from Osteolysis and I had a second set put in that didn't work so well. I have since had 2 more revisions on my left knee and another on my right knee. I have been fighting an infection for years of MRSA. It has gotten progressively worse and currently I can not walk at all on my left knee and I use a wheel chair and do a lot of sitting. I have had excellent Doctors but sometimes it is just going to present problems no matter how good of a job the Doctor does installing Total Knee Replacements or Revisions. A Painful Life.

I haven't done any extreme things recently like hiking and competitive football playing, but I am at least as active as an average male my age, this is almost 28. I can tell you I live as if I never had OCD. No pain or complaints whatsoever. Work outs and yoga from time time probably helped me recover. There is hope people, OCD is no permanent disability according to my experience.

Hi! My daughter is a competitive gymnast and was diagnosed with OCD about seven weeks ago. Hers was more of an incidental finding as we took her in for knee pain in the center of her knee and she was diagnosed with patellar tendinitis. Her OCD has never caused her any pain or problems and we have seen four different doctors in the past six weeks and she was only told to stop high impact skills for 6 weeks and was given the ok to go back to high impact today. I’m so afraid but she wants to get back and the Dr assured me since she doesn’t have pain or issues that she should be ok. We Are going to monitor it and she will get more x-rays in three months. Would you typically say this is OK or do you think it’s just a matter of days or weeks before something bad happens? I really need a peace of mind! If you would ever be open to reviewing her MRI remotely I would be happy to pay you to do so! I just want to make sure we are doing the right thing. She is 10 years old and growth plates are open. Cartilage looks ok from what I’ve been told. Lesion is stable. Thank you!!

I had the exact same thing! I'm 15 and I've been very active all my life with travel baseball and soccer and football! I was in football practice one day and my knee started catching. I went to my local orthopedic surgeon and he said I dislocated it and put me in a brace. I waited for a few weeks and it never went away, so I went to Felix Savoie at Tulane (because I had a labrum and Rotator cuff tear and he did the surgery) and he spotted the fragment of cartilage and took it out a few days later. He told me that I had this and we decided that a graft was more safe and had better results then the micro fracture. He put me on a donor list for the cadaver and 2 months after I had the surgery! My defect was at the end of my Femur and was 24mm long! He and Doctor Heard did it and it was miserable. I had to go non weight bearing for 9 weeks and then 4 months of PT and another 5 months of healing process. Now I start on varsity football as a sophomore and have very few limitations!

Started at 12, no treatment needed. A lot of sports until 35 years old. During a run strange sensation like no strength in the knee. After IRM
    Ocd stage 3.
After 1 years breaking sports, ankle are painful too, and lower back is injured too.
My advice is don't run too much if you're overweight, and make regular IRM before it break.

I’m 14 and had to get this surgery done, right knee, open reduction internal fixation. My growth plates are open and I’m 159cm tall. I would like to share my case. I’m a figure skater, I do all the jumps and spins, just like tonya Harding and anyone else you can think of. I started getting this knee pain in November, in the middle of my practice session. All I did was tell my coach that it hurt when I do twisting motions, and when I land. She just had me sit down and roll out some quad and calf muscles that were mildly tight and I got back on. It has been the same pain, on the inside of my knee which I think is medial(?). Anyway I naturally went to my physio. He gave me a meniscus problem. I was told I was allowed to skate, but no running, or jumping and landing on the knee for a week. So I did that and all was good after a week so I went back to jumps. The pain did not come back for a whole month until December. My physio decided to do the same thing, one week no jumping. After a week of no jumping I was okay and went back to jumps but the minute i went back to jumps the pain came rushing back. The pain never increased or changed the whole time. It was bearable for me and I was just doing my physio exercises that were for strengthening the quads to stop the load. It never went away but I thought it was better than nothing. Fast forward over 8 months of getting pain and sitting out of jumps for a week then going back, I dedcided that in early June I was sick of it and had a terrible feeling something way more wrong than an annoyed meniscus. I even had a whole 3 month rest due to lockdown, and pain came back straight away even with all these physio exercises. I went to a specialist, he asked me of an MRI. When he saw the results he was horrified. He said that’s my piece of bone and cartilage and it’s right about to snap off. He even said that I was probably born with it and the reason why the physio exercises don’t work is due to the fact that the injury started already on very very strong quad and gluteus muscles. He said he won’t be able to do an arthroscopy, he had to do a large 5 inch cut down my leg and completely move my kneecap out of the way and then start the ORIF, to which is now complete. He had planned to use 4 pins but instead 9 were used. I’m not able to go skating until may.

(the lesion is around 1 cm to 1 cm as my doctor said from my MRI. help im scared i don’t know if my knee will ever get better :((( also my right knee is starting to hurt ever since i got off crutches since i’ve been using them for all of may, so i think it’s from too much pressure but i’m not sure, i think i’m gonna make an appointment with my doctor to make sure it isn’t anything serious :(

I first started having symptoms early 2016 when I was 19. My knee got locked up once and that was my first symptom. Since then my knee has never locked up again. However, while being active, I felt a strong click in my knee that was mildly painful. The following few days my knee was swollen and very hard to bend (the click followed by swelling and tightness happened on more than one occasion). I went to the doctor but he just prescribed anti-inflammatory drugs (didn't even try to diagnose the underlying issue). I used to be able to do high intensity activities during 2016 and 2017 without too bad of a reaction in my knee. However, more recently whenever I do high intensity exercises, I would get the same symptoms as when my knee clicked the first time. There would be swelling and my knee would be hard to bend (like tendons around my muscle are tight).
It's now at the end of 2018 and I still haven't had any medical treatment, my knee is steadily and slowly getting worse I believe. It is always slightly swollen, it hurts when I squat all the way down (my knee can fold all the way but adding weight to it makes feel very tense and uncomfortable). There is a piece of cartilage or bone that pops in and out when I straighten and bend my knee, you can actually feel and see it from outside. My knee is also quite noisy when bending (I know all knees make those crackling sounds when bending but the sounds in my knee are like 5 times louder). I'm hoping to get proper medical treatment over the holidays. I hope that I can return to an active lifestyle afterward.

I have the SAME exact issues as you going on again now, as I did 9 years ago. I cant even bend my knee and it feels extremely tight, like a rubberband about to snap!! My guess is You will probably need the same surgery as me. Also do ALL of your physical therapy because the stronger your quads are the less weight on your knee. You need an MRI and of course another Xray. They did not find my break on the first Xray, it took a couple, and then MRI. They literally sent me to physical therapy on a fractured Patella because they did not see the break the first time ( in the E.R. ). And it stayed very swollen until the saw the break. Take care of it now so it doesnt get worse .Good luck!!

i have had a lesion on my femur for about 5-6 years, but it was just diagnosed a few weeks ago after experiencing locking, catching, buckling, and extreme weakness in my knee for the last few years. the symptoms started at basically the exact same time as i hyperextended my knee very badly, so they were initially written off as a sprain, but it never really got better. since i am almost 16 and done growing, and it has not improved over this time, surgery was the only real option. i had the surgery last week. it is very annoying to not be able to walk.

I did not have surgery although advised by doctors, and it was very painful at the beginning, even I could not walk 1 mile without stopping, but later I changed type of shoes and always use sport shoes for overpronation and flat feet which I guess reduce weight on the damaged cartilage and I can walk miles. I am playing football but feeling pain for a day after the match, but recovering after that. When I change the shoes, I restarted to feel the pain.

My daughter whom is very active with competitive dancing was having pain in her left at the age of 12. The pediatric orthopedic doctor ordered an MRI and she was diagnosed with OCD of the left knee. The pediatric orthopedic doctor at the time didn’t really make it seem serious and was not very informative. She mainly said to rest and take Motrin as needed. Just recently my daughter whom is now 13 started experiencing a lot of pain and her left knee swelled up twice it’s normal size. I rushed her to children’s healthcare and the took an x-ray and let us know that’s the swelling was due to OCD of the left knee. They told me she should not participate in any type of sports until cleared by an orthopedic doctor. I took her to a new pediatric orthopedic doctor whom ordered an MRI and he compared both years of MRI’s and said that it never healed and got worse. She has an unstable OCD lesion her left knee and a large knee effusion. She is required to have surgery to fix the loose bone and cartilage in her left knee. The pediatric orthopedic doctor also said he will have to do a bone graft and plans to make a second incision below the knee to do so. This is very hurtful because she attends a performing arts school and dance is her major. She has been crying like crazy because she can’t participate. We are so scared about the surgery (it’s scheduled for Dec. 26th). I wanted to try to wait and do it while they are out on Christmas break to give her a little time to rest and get used to crutches. Can someone please tell me how long it normally takes to heal? Will she be in a lot of pain after the surgery? How does the scar look after the surgery? She is so tiny and young I hate for her to have a huge scar. Please send me any tips that will be helpful. I will update this post after the surgery.

My 12 year old daughter has an OCD lesion. She also has osgood schlatter so the doctors just told her she can keep playing and that it wouldn't make it worse. Later she was told she had plica syndrome. After 2 years of pain we are now considering prolotherapy or surgery.

I was doing a home cardio workout similar to P90X but was more geared towards women with the "BeachBody Program" I bought on line. I was on day 5 out of 90 when during the workout I apparently fractured my Right Patella. I was diagnosed with OCD and was told by the surgeon That I should never run again and unfortunately my knee was stage 4 OCD. I at the time was in training for my first half marathon and was up to 3 miles every other day so far. I never ran again. Fast forward 9 1/2 years later and my only exercise now is yoga and walking. I walked a lot longer than usual 2 weeks ago and my knee has been very swollen and Im in a ton of pain every day , but now for some reason its not just my knee in pain but my whole leg as well, I ice it every day and walk with my old crutches. I am unsure as what to do now because to see a specialist again even WITH my Health Insurance is gonna cost me and as of now I am barely working. Any ideas from anyone or even the Doctor above :) :) :) would be sooooooo much appreciated!!! I just want to be able to put pressure on it again to even walk and clean my house etc. etc. etc. Could my Patella be fractured agin?? I dont know. Should I go to the ER and pay that bill over time? Has this happened to anyone? I am a very active person and am not at all overweight. I have a real problem even sitting still, LOL..... I have ADD bigtime!! Some help please!! :) Thanks for reading!! God Bless!!! Dana.

I have OCD of the medial talar dome of my ankle. I am 44. I also have a piece of bone that has come away. I have constant pain swelling as well as catching and locking. I also have heightened attacks where the pain will just increase out of nowhere and it feels as tho my bones are collapsing the pain is so intense around the ankle joint and on top of the foot . I usually cannot walk for a while when this happens or if I can walk it’s very poorly then the next day I have a massive bruise around the ankle and on top of foot . I see the surgeon next week and find out what he wants to happen, however I was hoping for an arthroscopic approach not the graft . Which procedure has better outcomes ? What are recovery times for each method ?

I am 12 years old. I have had a problems with my left knee for 5 years now. Just a month ago I got diagnosed with OCD. This morning I went in for a MRI scan. The doctor told me that I should get surgery to fix it before later down the road. My parents have said it's up to me. I'm conflicted though...

Kaitlyn Slone I would get the surgery. I just got mine last week and starting therapy. My doctor said it was better to get it done or I could have arthritis at the age of 30. I think it is better to get it done sooner rather than later. Good luck

Thank you very much for the informative video! I have an ankle OCD, where my doctor recommended a surgery involving removing the loose cartilage and filling it in with another piece of cartilage or stem cell therapy (if I remember correctly, he suggested the latter). It has been two years since his recommendation and I did not do the surgery yet as I am studying abroad.
I would like to know how long I can hold off the surgery before it may lead to further complications. Additionally, I am curious to know if I can travel by airplane and resume regular activity (walking, but no intense activity) around a month or two post the surgery. Thanks!

I know this is pretty old but I’m curious. I am 23 and recovering from my second surgery due to OCD. The first surgery was an attempt to screw the fragment back in place but it didn’t heal, so this time they used donor material. So far the recovery has been 100x easier (judging from almost 6 weeks out) and there is hope that the graft is healing. I’ve been skateboarding my entire life, and am really hoping that putting my all into physical therapy, maintaining strength into the future, and eating right (balanced diet + supplementing vitamins c & d, and calcium) allows me to return. I’m okay with never jumping down stairs again but I’d like to at least cruise around and skate small obstacles, which means a lot of jumping over and over again. What are your thoughts on this?

@@burchskarate7522 Hi, I'm about 5 months out and i'm almost back to 100%! I have a lot of tissue damage still, so I have some weird pains, grinding, and swelling but I have full mobility and was recently approved for impact training, didn't take me long to feel comfortable skating again. Good luck to your daughter, this bone stuff can be kind of a gamble but based on what I've read and seen from others this surgery tends to be pretty successful.
