Basically ouma accidentally triggered a trap making him technically become Chihiro's murderer, Chihiro to avoid that decided to take a knife and thus causing it to be a trial where the guilty party would be dead.
Kokichi cannot accept that Chihiro took his own life when they were so close to winning and leaving alive.
And the last scene is that Kokichi and Angie face two final punishments, the problem is that Kokichi couldn't use his friends' cards so he used his own punishment or just died with Angie.
