The Sanitarium and the Asylum are in different universes, the Asylum in more apart of batman and it's universe. The Arkham Sanitarium has its own universe away from the asylum.

It might be a reference to Arkham in the Lovecraft fictional universe. Which was the inspiration for Batman place of the same name. He kinda looks like a deep one.
