Eh, I actually thought it sounds a lot like Peter's actually. Which makes sense, considering he's Peter's son.
This is proof that Peter, Lois, and Meg can understand Stewie but choose to ignore him cause they know that he is evil and is capable of doing terrible things so for their own safety they ignore what he says but lets him go off and do what he wants. He is able to interact with literally everyone else so clearly he is understood by everyone.
The therapist isn't real, though. It's implied that he's a manifestation of what remains of Stewie's conscience and his introspection at himself, and the therapist dying represented how Stewie threw away his insecurities about how kids see him and decides to live for himself.

The scene where Stewie’s psychiatrist dies of a heart attack drags on for way too long to the point where it becomes very disturbing and unsettling.

What's making this scene morr disturbing than the actual murders of Stewie is that those extreme things are so absurd that we know it's a joke. But this isn't an over-the-top show of violence. It's just Stewie deliberately denying to offer help to a dying man. This one's realistic and the fact that Stewie did this is making me feel horrible.
It does show that Stewie has changed though, considering how he couldn't sleep at night after not helping him.

I’d say this whole episode proves that Stewie is either insanely homicidal or that Brian killing Rupert had more of an effect on Stewie than we thought! Then again it’s pretty obvious that it’s a bit of both!

This was a very deep episode, it was a mix of funny and with real emotions and a very relatable image from Stewie ( not the killing part of course) I think this might be one of the most complex episodes that I ever saw Family Guy doing, very interesting.

It’s even worse than Olivia’s death in the burning cardboard house. But she deserved it. Dr. Cecil Pritchfield was actually trying to help Stewie. That’s what makes his death emotional. I cried during that scene. Sir Ian McKellen seems like such a sweet old man.
