    As I see the treatment procedure, I have seen some certain mistakes and some good treatment plan implemented. In my opinion first the removal of the 2nd molars is a choice which puts most of the orthodontic practitioners in a delima. Either to remove the 2nd molars or 1st premolars. But always considerations must be put into mind with regards to the patients age and the erupting 3rd molars. Since the patient, as I see it is in a growing stage therefore it is to be expected that the bone is still in its growing or expansion stage therefore the removal of the 1st premolars will cause more harm than good and eventually would cause an iatrogenic diastema cause by the extraction of the 1st premolars. So that leaves the next option of removing the 2nd molars. Since the child and the radiograph might had revealed an erupting 3rd molar then removing the 2nd molar would be the next best thing to align the anteriors by retracting the other dental elements posteriorly. And taking into consideration the growing stage of the patient, therefore any further bone expansion will not result to any diastema later on.
    Now with the extraction of the anterior incisor, in my opinion and true to the fact, it is a big no-no in orthodontics. Firstly, due to the removal of the said anterior tooth, there will be a tendency to cause a collapse in anterior bone segment since the patient is still in a growing state eventually it will cause an anterior overjet. Secondly, with the extraction of the anterior tooth, the midline is compromise. Thirdly, it is unesthetic considering that only 3 anteriors are left compared to the 4 anteriors on the upper.
    For these reasons, it is always a good thing to have in your practice the knowledge of all areas of dental practice such as esthetic dentistry and prosthodontics.

      +Juan Makabayan The extraction of one of the lower incisors is a no-no? Since when? Because, if you check there are a few cases that are solved that way. It is not the classic way, but one cannot say it is forbidden. Of course, the patient must be informed about the midline off. It is exactly for this reason that we have the Bolton index, which means deviations exist prior to ortho treatment, which means the six upper anteriors do not correlate with the six lower ones. It is not a "no-no", it is a "maybe" in a few cases....have knowledge of all areas of dental practice, such as esthetic dentistry and prosthodontics...... good one! Are these all the areas of dental practice? Or are they the only ones with impact on orthodontics? Are you sure? :)

    Lot of minor issues with this case
    1. Poor finish of the upper front teeth , they are not aligned properly , this is a bracket error(orthodontist) at the start.
    2. Ditto lowers just did not get the brackets in the right spot at the start and also the Upper right canine rotated still.
    3. Because you removed a lower incisor you created a boltons discrepancy , that normally would have given you a large overjet (difference between upper front teeth and lower teeth) , but you extracted upper molars which helped (Sort of) you have ended up giving the patient actually a really poor bite, all the back teeth meet incorrectly.
    4. Sure the teeth are reasonably aligned but not up to normal standards.
    5. Those who have said TMJ issues blah blah, you dont know what your talking about, show one scrap of actual scientific evidence that proves that if you extract teeth??? What about all the patients that have TMJD issues bite issues etc etc who have 32 teeth?? Against logic there is no armor like ignorance.
    6. Your comment about retainers and barely never using them is a poor comment teeth move throughout life regardless of extracions non extracion etc etc . Stop pretending your orthodontic does not relapse, you give real Orthodontists a bad name pal

    I used to do that kind of treatment before and i stopped because it retrude the premaxilla that can cause TMJ disorder, and it can also decreased the arch length. So i stopped doing bicuspid extraction ,besides u don't have stability in the end.

    of course the classic method is ext of 4 bi-s, but if one does not ext them, I think the extraction of one of the lower incisors is a smart choice, because it doesn't change the lower intercanine width, which in turn prevents the relapse. The extraction of the 2nd molars... I cannot agree with that! But no matter what you do, doc, teeth will always move, which means.. rotations, relapse etc MAY OCCUR

      Good thinking..but what there another alternative....The doctor used alot of technique with are great, but there another alternative....Orthodontic so many possibility,but got to choose the best and most efficiency

    Why would you extract a central incisor? The end result is either a class I molar with an excess overjet and overbite, or a class III molar and normal OJ/OB. Four first bicuspids is clearly a better choice.

    Not bad, They look like my teeth before he started. But I REALLY dislike the look of having 3 middle teeth between the K9s. To me it doesn't look as clean as 4 teeth.

    should of extracted all of the first pre-molars, put on a fixed brace with springs attatched between hes upper lateral inscisors and 2nd premolar to create space then attatched the archwire to the canine to bring them into line along with all the other teeth. class 2 elastics should of then been used on one side to correct hes midline then class 2 elastics on both sides to correct hes overbite and overjet!!

    Everybody saying pulling teeth is wrong well so far I had four baby teeth pulled and four adult teeth pulled and my smile is improving currently with braces.

    4 bicuspid extractons wuld take 18 months min, not 10. Why bother with a lower incisor extraction anyway? The distalizing of the upper molars will be where the TMJ/masseter problems come from. Nice outcome though, we're all critics on here.....

    I have a very similar case as mine but i had 4 teeth extracted at age 13 and i think thats a bad idea but my ortho said there are no more choice but to remove because of protrusion and a big possibility of having impacted wisdom teeth because of crowding

    upper and lower midlines are off. Hopefully the wisdom tooth will come in and not be impacted because than the patient will loose 8 molars. 4 first bicuspids would have been so much better and would have taken the same amount of time.

    Always use retainers after treatment! Teeth have and will relaps back on themselves it happened to me, it happened to my best friend, it happened to my sister, it happens to anyone who didn't get are retainer after treatment believe me, it helps the teeth to settle after all that movement. What is the point of going through all that pain to get the same result you started off with? Beside when you fet older your teeth edge closer together not good news for some one with overcrowded teeth.

      orthodontists remove teeth to make their job easier. well in some cases removing teeth is the only choice but in most cases the palette could have been expanded.

      If you know about orthotropics, this kid's face is already damaged for the rest of his life.
      In traditional orthodontic extractions are straight up prohibited.

      Kiana 134 No, that depends of the patients face. To align, crowded teeth tend to protude when theres no space. In some cases can be ok. Also, the results are generally faster and more stable after treatment when correct extractions are done.

    Damn I'm not profesional at all but that look really bad.. Why does the incisors removed? This is unestethic with 3 first tooth and the molar doesn't match, as I say I'm not pro but I don't like the end.

      Hello doc, saan po ba pwede ireklamo ang dentist? Sinira lang lalo ng dentist ko yung teeth ko nung nagpabraces ako sa kanya. 3years na hindi pa rin nya naayos, nagkacrossbite lang ako and nadislocate facial features ko. Then last year july 2019 i decided na ipatanggal na lang kasi di naman nya naaayos at sinisingil lang ako ng sinisingil kahit fully paid na ako. Mas malala pa nga case nyan sa vid kesa sakin. Hays, sayang binayad ko sa dentist.

    This is what my Ortho suggested we do for my 11 year old son, thanks god I Refused! Shame on all the doctors that are still pulling healthy teeth!!! This is the easiest and worst treatment done! It is really harmful on the long run. It also makes the face and cheeks flat, and caused TMJ problems and airway narrowing later in life! Go to an airway focus or Orthotropic doctor and see what they’ are doing.

    Proclined and protrude upper incisors with no overjet seems the patient doesn't have class I molar relationship and the arches aren't coordinated.
    On the other hand, Bonding wasn't that good. especially the incisors. prefer tp extract 4 1st premolars with conventional braces and if the patient was old enough I would destalize with mini screws by extraction of 2 molars

      I have the same issue, I have/had a crowded teeth before. What my dentist did was to put them all together without extracting any tooth. What bothers me is the way I smile, it seems that my upper teeth pushed forward, same goes to my lower teeth. I've already talked about this to my dentist and what they want me to do is to remove my two upper wisdom tooth but on my own research wisdom tooth has nothing to do with my crowded teeth and giving more room spaces for my teeth. I want to ask if it's really true that removing my two upper wisdom tooth will make room for my teeth or should I prefer splicing of my teeth? I'm thinking of splicing my two upper lateral incisor since it's kinda too big for the size of common lateral incisor... what do you think? PLEASE HELP..... P.S I'm still wearing a traditional braces for almost 2 years... THANK YOU

      Honestly removing teeth is pretty bad, if your teeth look kinda OK you should have no problem, people wll not judge you if your teeth are 5 degrees forward,nor should you, on the other hand if they are leaning to the front a lot, your orthodontist dint do their job correctly.
