What is the difference between Schizoid Personality Disorder and Schizotypal Personality Disorder?

  • This video describes the difference between Schizoid Personality Disorder and Schizotypal Personality Disorder. Both of these disorders are in Cluster A in the Diagnostic and Statistical Manual (DSM). Schizoid Personality Disorder is characterized by detachment, being a “loner,” no interest in forming relationships, a deficit in social skills, taking jobs that isolate them from others, no close friends, decreased interest in pleasurable activities, indifference to praise or criticism, cold or flat affect. Clinically significant distress is not required for a diagnosis of Schizoid Personality as stated in the Diagnostic and Statistical Manual (DSM). Schizotypal Personality Disorder is characterized by ideas of reference, odd beliefs, unusual perceptions, odd thinking or speech (e.g. elaborate, metaphorical, vague), paranoid ideation, inappropriate and constricted affect, peculiar behavioral appearance, lacking close friends, and social anxiety. Both disorders are characterized by lack of close friends and unusual affect.

    I'm a shzoid and have the life. I can happily go without socializing for months etc, even talking to people for weeks is fine to me. However, the strange thing is that I don't have that exterior dead pan persona typical of schzoids and I'm not pretending or puttng it on. Often after I socialize (on those rare occassions), I often feel regret at having wasted time with people. I've cut off most of my family because 80% are bad people and I didn't want the hasstle and I am happier for it (seriously), I kinda wish they weren't bad and things had gone different but I don't miss them, that's for sure !

      I'm no therapist but it sounds like you might have an attachment disorder or aversion caused by the family as opposed to schizoid, since one of the defining characteristics of that disorder is shallow emotional depth and affect.

    I have been diagnosed with SPD myself and got curious one day and took an online test based off the DSM criteria just to see the results(obviously I took the results with a grain of salt)
    I scored high in Schizoid and Schizotypal but also scored high enough to meet ASPD. Not sure if those 3 can be comorbid at the same time. I don't do antisocial criminal acts(other than cannabis lol) and I'm not aggressive. I try to lay low in life and keep to myself.
    I'm married but I don't really have friends. Don't have a need for them either. No hobbies other than smoking weed, playing a little Xbox, and working out on the weekends.
    My empathy seems out of wack. Sympathy is out of the question. I have cognitive empathy but I think that's it. I'm just not concerned with or about people. Even if my wife gets hurt or is in mental pain I can't help or really feel anything. Trying to help in those situations makes me feel like I'm navigating a maze...
    I do experience paranoid social anxiety. If I go into public a lot of times I will think that people are staring at me or something else irrational. I hate eye contact because to meet it means aggression.

    Diagnosed with both. Never understood the schizotypal diagnosis tbh other than that I'm a non-conformist and quite paranoid (specifically in interpersonal relations). I'm allergic to most types of magical thinking, I'm a very rational person and love science. So I guess they just labelled me like this because conformism seems to be a criterion for a "normal" person and the paranoid aspect. I don't dress extravagantly either, I usually dress to not stick out in a crowd.

    Sounds to me like the "odd speech" criteria of over elaborate, metaphorical, or vague would apply to the British Romantic poets. As well as some other other poets. Is this an indication of possible Schizotypal Personality Disorder being a asset for poets?

    Dr. Grande I like how much patience you have, your listening skills, your openness to new ways of looking at things, etc. Your general description about overall mood differences between normal people and people who have mental illness signs and symptoms that cultural differences cannot explain is very clear for the average person to understand. Dr. Grande I have another question for you or one of your assistants to please answer for me after a professor in the Occupational Therapy program at the U. of A. told us that she as a therapist too saw an unfortunate practise being done to paitents who first arrive in hospital by some physicians where the initial treatment they offer a patient at first exacerbates the original presenting patient health problems there and instead of helping anyone then often causes signs and symptom of mental illness on top of everything else the patient is trying to heal from. For example say someone has been tasered on a repeated basis at home while living on an acerage just outside of Dayton Nevada because they could not pay their property taxes while they were working as a stage hand doing welding on the set and at the same time they were going through divorce litiagation that included a custody dispute and then after losing their job when not feeling well after being tasered about once a week for not paying property taxes after not being born in the U.S. then having to do grocery shopping a mile away without any gas and insurance for their car? Then if she got to hospital seeking help somehow after only being given one of the folllowing drugs from the following list and being told to get some rest and see if that will be enough to find relief instead which of the following when being cleared out of the body are most apt to cause adversive symptoms enough for workers later during a welfare check asked by her estranged husband to do would cause the worker to suspect a tentative schizophrenic diagnosis when calling an ambulance for them? - (-1) sedatives and hypnotics, (2) stimulants, (3) opiates, and (4) hallucinogens and psychedelics. And, how long of a rest do you think a woman welder who had been working very hard and living in a holiday trailer in often very hot weather too after going through all that for awhile when nearing age 40 would need to not look overly withdrawn most of the time and so not look pretty enough to look odd to any doctor aorund town?

    I have all these symptoms to both disorders, some not to the same extent though like for example "Belief that certain casual incidents or events have hidden messages meant only for you" or "Little or no interest in having sex with another person".

      There are several differentiating factors, but the most prominent would be the sustained hallucinations and delusions that characterize schizophrenia, but are absent in Cluster A.

    I've had magical thinking, paranoia, anxiety, panic, loner type, but I would say I'm fine at the moment. I don't really go out with people much, but I text someone(s). Somewhat interactive.
    Maybe I am somewhat "normal" for now.

    Paranoia and "magical thinking" does not reflect upon just these cults of religion but normal everyday items such as seeing still water move, seing a bug and so fourth and once you see it you convince yourself so much that what you see if reality while another part of you is so skeptical it creates this type of paranoia. It doesn't make it hard to believe whether it's real or not but you start to question more stuff on an average basis and you start to Overthink it a lot more hence the paranoia. The magical thinking is more or so connected to the schizophrenia side along with the paranoia.

    I wonder why the schizotypal disorder is not a personality disorder in the ICD system, but schiziod is? It's actually a little controversy on that. I find the DSM system easier to comprehend. And why are schizotypal and schiziod disorders parted, they are so extremely alike, even comorbid? As other disorder are tho.

    Hi Dr. Grande. Prior to 2013, I was under the impression I suffered from Schizoid PD. However, an insightful psychologist once asked me, "Well, do you WANT a girlfriend?" and when I said yes, he dx'ed me schizotypal. I'm 44 years old. I would benefit, I think from some discussion of STPD with a co-morbid substance use disorder. Heroin took about 20 years of my life away from me, (methadone now for 18-19 months)....I now have no friends, no job/career, no money, I share a bed with my mother, don't have a clue about sex or romance and don't understand how relationships form...I am in therapy. The Zoloft and Abilify are not assisting either. I am afraid that I will either take my own life within the year, or relapse on heroin merely to relieve the major depression and experience SOMETHING pleasurable, anything. Do you have any suggestions?

      Crimson King ....hmm...go visit an islamic mosque...see if u can find god conciousness ....thays where purpose and calm is....the world is designed ...the designer revealed a set of instructions to cope with the negativity in this short perishing existence ....its all uphill work ..until u find the peak....then you will see.

      depeds on the person. For me its been from being able to talk to a higher being to being able to control events with just my "thoughts"

    I really feel like I have at least one personality disorder... A lot of the symptoms I hear from these very much describe my experiences... It seems to have some paranoia aspects of schizotypal without the magical thinking... Like, I do have some unusual beliefs when it comes to philosophical topics... I'm also just a very skeptical person though, so I don't believe in special powers or spiritualistic things... A lot of it seems very accurate though, like I've had multiple times where I've felt like friends were talking behind my back about me... (To be fair, I feel like it's really quite possible that they were...)
    Idk, though... I also can get familiar with people after long enough... I think... I have like one friend I do stuff with since I got in a disagreement with some other friends who dislike me now which I used to play video games with... (Who also seem like afterwards they were talking a lot about me angrily, but that's to be expected I guess) and so yeah...
    The craziest thing is probably the crush I had on a girl for 10 years because I was too nervous to talk to her until graduation was coming and I forced myself to go through with it only to be rejected and then feel sad for like the next 2 years... I'm just extremely neurotic (aside from anger which I can control externally although internally I get really fired up) and have high openness, low consciouncessness, low agreeableness and extremely low extraversion which all seems pretty in line with what you would expect from schizotypal going by the idea that it's should be more about dimensions rather than categorical when it comes to diagnosing...
    Just been trying to learn why I act so different... I almost always am wearing a sweater (it's like Steve Jobs' turtleneck...) so that fits with dressing odd as well as my long hair (as a guy) and also my interest in potentially one day delving into being more of a romantic/victorian/medieval sort of goth... My sense of style is quite unconventional... Anyway, I'd rant more like I often do, but I must go...

    I think i have schizotypal and schizoid from using lots lots of mushrooms with the mushroom boys.. AN D now a heavy drinking i am very agitated after days of binging so have the typal traits manifest for about a week of not gettimg trashed.

      Yes, but only rarely and for a short amount of time. We're talking seconds or few minutes. If it happens often or for a long time, it might be schizophrenia.

      some relationships can be wonderfully supportive, and sometimes friendship is joyful and fun. that said, that kind of connection, while valuable and necessary, isn't possible with every person you meet. some people just don't click, and some people are just jerks!

    I have been diagnosed with ADHD, 42 but in 2018 (yes this is on record) got fired from a company I worked for (over 16 years, took them from paper to paperless) however due to the company and my country's draconian employment equity policy I was replaced but see since labour laws here are different they had to set traps hoping I would make a mistake that would result in immediate dismissal. This was the agenda, and like I said from audio to paper this is on record... short of it, lost everything, what I had to endure leading up to said dismissal meant prolonged suspension, anxiety for months never knowing what awaits, nonetheless, lost everything, my wife left me. Today I have small business (really the only choice for white men)... so in my case although I have always been an introvert, I am void of emotion, care little for relationships all things related to SPD and as such well, I am misanthropic and well just my story

      Angelicatoo I'd suggest thats a pretty shallow interpretation of Christianity. If you're interested in nuance id recommend Dr. Jordan Peterson's Biblical lectures here on CZcams. They're quite intelligent and very insightful

      That's how "disorders" come about. They are unusual. I think christianity is fundamentally magical because it is not grounded in reality, so no it is not a shallow interpretation. Christianity isn't seen as crazy or magical because that has negative implications when in reality it can be defined that way because of how far out it is realistically. Not saying it isn't true. I'm just saying it can be defined as magical, and who's to say magic doesn't exist? :)

      Once you see someone with it you will get it. My sister told me one time that seeing a commercial for a home machine was a direct sign that she should work out. Another time she said her car just took her a certain way.

    Crimson king your therapist does not seem to know anything you need to see someone else if they diagnosed you based on whether you want a girlfriends it means they don't know anything about personality disorders you probably have just depression
