      In Assessment if you are using NEWS chart, the monitoring frequency is still the same policy in the community for Assessment.

    Hi, what if in the Disability portion the patient is confused and he scored 3, what should I put in the monitoring? Should I put minimum 1hourly since it's single parameter or should I put weekly since the setting is in the Community? Thanks for the response.

      For confusion in NEWS CHART,a patient can only be scored if it's a new onset of confusion,so if it's a new onset of confusion and the only place the patient has a score of 3 is in disability the frequency of monitoring should be 1hourly.However if the scenario is for a known dementia patient that's not a new onset so the patient will score 0 for disability.

    Please in a situation where by the question state that the patient is having dementia, he lost the wife few years ago and his still talking about the wife and also acting confuse.....can I pick confusion

      Hi, yes you can go for confusion and state the 6CIT score if applicable for planning.

      Hi, yes it is acceptable what is important is to cover all the A-E.

    In disability on the News chart, are all patients in Osce Alert or can Alzheimer patients be confused??

      Hi, at the moment, your patient is considered Alert, if it is not a new onset of confusion.

      Hi,
      For a news score of 0 monitoring frequency is 12 hourly. Refer to the back of your news chart monitoring frequency based on scores are written.

    If a patient repetedly say i was a nurse before after 1min she will say again i was a nurse did i told you before again the same..and when i reached acvpu i asked date and time she told me incorrect date but correct time can i write it as confused

      Hi, Re-orient the patient, try to have a critical look, ask more questions to establish that the patient is confused. But be mindful of time.

      Hi, at the moment NEWS chart is used for community setting. The frequency of monitoring will be dictated based on your NEWS score for Assessment.

      Hi Emily, yes if you are using NEWS chart the standard of the monitoring frequency will be applicable. A residential home is still in the community, Nurses and health workers are available to do that.

    I thought wearing gloves and touching patients document at the same time is a red flag? Is Apron and glove really necessary for assessment based on the marking criteria?

      At the moment, the use of gloves and apron is not required for assessment..

    I am questioning the use of PPE in Assessment. is it acceptable and hyegienic considering you will be in constant contact with the patient and documeting at the same time?

      Hi, during the procedure, it is ok to write with your gloved hand, it is considered a clean procedure. But you can remove and discard your PPE , do hand hygiene at the point that you will interpret your NEWS chart and ask questions on Activities of daily living.

      Use of PPE is not compulsory for Assessment based on the marking criteria, ensure you do hand hygiene as required. But if you choose to use PPE, you will not be penalised for that.

      Hi, in OSCE based on the NEWS chart, the only score that will not be escalated is a score of 0.

      Hi, based on the marking criteria, PPE is not stated. You have a choice to use it or not. If you use it it is acceptable, if you are not using PPE please do hand hygiene as required.
