I had to watch this like 5 times before I realized that at the end when it says "now that I'm without your kisses, I'll be needing stitches" it literally is what's happening in the clip, Dean won't accept Cass' "kisses", in this case, healing and since he won't, he'll be "needing stitches" to heal naturally.
