Struma ovarii with PTC is not a highly aggressive malignancy with a high mortality rate. Most differentiated thyroid cancers are highly treatable, and those in the ovary are similarly managed, depending on stage. In fact, I don't believe I had read any reports in the literature on death caused by a struma ovarii with PTC. It would be very worthy of a report should it occur. But if your case was relatively small and confined to the ovary, you would be very unlikely to be the first such case. That said, you mention "high grade" which is not a term usually used with papillary thyroid carcinoma. Perhaps you would be wise to have a second pathologist examine the slides. If the tumor has become more advanced (they tend to develop new mutations which alter the morphology and also the prognosis, at least in the neck-- I have not heard of such in the ovary) then my previous comments might not apply.

It came back with malignant struma ovarii (proliferative struma ovarii with focus of follicular variant of papillary thyroid carcinoma). Went to meet the oncologist today. He said it was very rare and had to reach out to colleagues. More test scheduled.
