As a physician, i did this once for a patient, but was approved by the lab. A side knowledge, you can actually donate your own blood and store it in a blood bank for your future use if need be.

Why does she need to decide? You’re telling me the hospital doesn’t have enough blood in storage for both kids? That seems kind of insane and unrealistic.
It’s actually really common for hospitals to have limited amounts of blood product on hand. Some types are rarer than others and on any given day 29,000 units are used in the US alone. What was unrealistic to me was that they didn’t seem to even consider splitting the 2 units between the boys while they got fresh donations
@@lunahyacinth6335 That's what I thought. Blood shortages are unfortunately a massive problem and I predicted that was what would happen based on the title and first 20 seconds of the video. But I was also really confused when they said they had two units and could only give them to one boy. Maybe they were worried that splitting it would mean neither got enough and both would die? But they were also talking about how they were in the process of screening donations which makes it seem like they would have been able to get 2 more units on time.
If you watch the episode, they had a mass casualty event, so a lot of their avalable blood was used up and they were on short supply.
It is also really common for hospitals to not have enough blood. It don't last forever and what is gotten from blood drives has to be divided across a lot of clinics all over. Some areas, there's just not enough donors.
It's a blood type problem. If their blood type is unknown, the only blood type that can be given to them without risk is O negative, the "universal donor". If both boys' blood type is O negative, same problem, no other blood type is compatible with theirs.
Only 7% of the population has group O negative. The availability of this blood type is therefore a real problem.

If the nephew had more changes to survive then he could wait until the blood cames.
That's a fair family decision (because this is an impossible situation) but just so you know, that's not how medicine works. Do you remember the movie I, Robot? Where Will Smith was saved because he was more likely to be okay than the little girl? It's like that. When two people are in critical condition and could die, they put more resources towards the person they gave the best chance of saving. Because chances are if you give it to the person with worse odds, they could both die.

There is every reason to believe that military testing for a personal diagnosis wouldn't meet the standards of hospital testing for blood transfusions, and no reason at all to imagine that it would exceed them.
She's not trying to make excuses. I think she was trying to protect everyone. I mean military goes to a lot of unknown places and who knows what they can pick up that gets into their blood that the military doctors don't even think to test for.
She’s not making excuses, they could sue the hospital if something came up in the kids blood after the transfusion all because they didn’t make sure to run through THEIR tests at the hospital
@@niyaakbar1098 but if you think about it for a second, military medical testing has to be at THE highest because they have to constantly make sure that none of us have caught something. because having a serious disease or infection is grounds for a medical discharge from service.

My mother had a complicated birth with me. My grandmother was in the room during the whole process,the doctor asked my grandmother who she wanted to save- me or my mother. She couldn’t make that choice, told the doctor she could not make that choice that it is not up to her. She said “both, won’t accept anything else, you cannot ask that of me. Save them both” as she proceeded to grab my mothers hand, kneel on the ground and pray. The doctors did what they could, when I was finally removed from my mother’s wound I did not make a single noise, move a single muscle, or even have a pulse. I was declared dead. My grandmother bursted into tears while holding my mothers hand, whom had no idea what was happening due to the drugs, and asked god to please show her the truth. If he is real, prove it now. Within 60 seconds I shrieked out crying. The hospital staff were shocked and continued with what needed to be done. When my mother came to, her first words were “where’s my son? Mom, did my son die?” My grandmother, still holding her hand, said “no, he is alive” my mother with a cracked voice, tears streaming down her face said “mom, please don’t lie to me. I’m here, he is not. tell me, is he dead?” And my grandmother squeezing her hand tighter “no mija, he is alive. I promise you. He is okay. I love you, and would not lie about such things”

To anyone wondering, on an ordinary day this isn’t going to happen but on this day in the show there was a mass casualty event. And I’m pretty sure one woman has been gravely I fired before they came in and used up almost all of the blood they had left.
On an ordinary day, they wouldn't give her the decision because only one boy is her child. Unless they have legal custody of the nephew or power of attorney in place of her sister, they wouldn't be able to even discuss her nephew's care with her.
I know its a show, but Ethans great. The actor, and character.
I'd like to believe a rea life doc would likely do a similar action.

I, at least at first, would have done the same thing the mother did. 'You decide. I can't have a hand in this.' And if my partner tries to blame me... 'What would you have decided? Huh? You'd condemn my sister's kid to death? How unbelievably selfish! Oh now you wanna save my sister's kid? What about our child? Not such an easy choice huh? Who tf do you think you are screaming at me when you can't make that kind of call right now?"

You have 2 units and 2 patients, give each one as a stop gap until the lab can verify the “cleanliness” of freshly donated O- . It’s not a perfect solution but it is enough to buy time
I think the problem is that they have to transfuse their blood during an operation. In this case, it's all or nothing.

Why was it her call to make this decision? Its not her blood and the boys are not both her sons, so IMHO doctors should have to decide that in the first place, or maybe some kind of ethics commitee!

The woman being the adult and the biological aunt of the young boy whos parents/guardians weren't around legally defaults her to be the legal guardian and authorizes her to make such decisions. I haven't seen the episode but it looks like she was looking after him/had custody of him at the time of the accident that put them in the hospital so until the parents/guardians of the young boy can be contacted and/or arrive at the hospital that guardianship would legally carry over provided the hospital staff feel she is in a sound enough state of mind to make such decisions. It's the same here in Australia.
@@TheMegaAsh I get that she is probably a legal guardian, but this decision was not really about wich treatment the boys should or shouldn´t get. Its not like they dont want to give one or both boys blood for some reason, the problem just was that they couldnt give it to both atm. So I think the question was about how to manage limited resources (in this case blood) and a doctor should decide who gets it on medical parameters and not the parents, who could only decide on "sympathy" and not rational.
Even if her sister(2nd boys mom) were there too it should be not their call to discuss or decide what to do...
@@KingSteven26 In the context of a TV show I agree but in real life the hospital would have consulted their lawyers and discussed who has the better chance of survival without the treatment & who would likely die without it and that decision would legally have to be made by a legal guardian who is medically sound of mind after they have been advised by the hospital. I know it's a TV show but in reality the boy they said has the higher chance of surviving with the treatment would have been the one that would have died without it. Again, I know it's a TV show but you can't save everyone and there are times you must leave people alone with nothing more than pain killers and let them die......regardless of how distressing or in-human it may seem. This is a fact that very few people in the modern world realize because we are so close to medical help and we suffer from so few illnesses that can't be fixed threw modern medicine but modern medicine can only go so far. Until we have a machine or medication that can instantly fix bones, damaged organs or regrow body parts (like in some Sci-Fi TV shows) this will remain a fact of life that we have to acknowledge and live with. In real life it's about who you can help and who you can't and doctors know this all to well and must make a decision. If you have two patents both needing the same treatment that is in limited supply and you are sure one will recover with the treatment but the other won't recover even with the treatment than you treat the one who you can save and don't waist a resource. The same goes for looking at who can last longer without the treatment vs who will die sooner without the treatment. In a western world we are never so far away from restock on anything so if they are one day away from restocking supplies and one will survive that one day without the treatment whilst the other will die before they can restock than the one who will die is the priority.

It’s an absolutely devastating situation, but if she had picked her son and he had still died, then it would have been for nothing because both boys would be dead.

Blood can only be stored for about 6 weeks. In the U.S., the majority of our population actually has O+ blood which can only receive either O+ or O- blood. Which also contributes to "shortages", because even if a hospital has 40 units of blood, if an O- person comes in, but none of the blood on hand is O-, they essentially have no blood for that patient.

This is definitely emergent, but i feel like with this pacing, they probably have time to type and cross blood for both so they're not using O- blood. Mom should also know her kids type too
Idk about this show but I'm pretty sure most hospitals have a policy of giving O- unless the blood type is already in their chart or they have had time to test for their blood type. It's a liability issue in case she's wrong.

If both boys would die without the blood. And there's only enough for one, wouldn't you give it to the one who has the best chance of survival??
Counter wise you could then also argue that the one with the higher chance is in less need of the blood and it'd give them an equal chance.
Speaking from a personal opinion though. If I was a professional with patients in similar conditions and I could only help one I'd pick the higher survival chance too.
If that was my mother or another relative though I would settle for my mother most likely.

Sorry, very late answer. But basically, O blood is in short supply and blood banks can run out when there's a mass casualty event, for instance - but usually, they work very hard to try to get the word out and keep supply up. Unfortunately blood donation is declining, it's not as popular with younger generations apparently.

It is actually… at least in Germany only around 6% of people have 0- blood, compared to 35% 0+ that is quite rare. 0- however is universal donor blood because most people can get it without having a adverse reaction, but I think everyone should know their blood type… if it didn’t have bad implications here in Germany I would probably have A+ tattooed on my arm
There are chronic shortages of blood supplies. This episode is a bit extreme, but it shows what can happen when supplies get too low. I'd donate, but I can't because of meds that I'm on.

@@OctoberOctopusM well idk how it is in other countries but most people that donate here do it in the medically mandated minimal wait time anyways so even if you'd have the contact info of your donors, chances are they're not allowed to donate anyways. Also Dr. Choi according to another episode is 0- if I remember correctly but apparently his last donation must have been closer than 4 weeks or else he probably would have donated.
But the part of making drama where non is needed is probably that cross matching blood types takes like 5 minutes and requires like 4 drops of blood so the fact that they just didn't do that is annoying af.. although they might have and just not have shown it here

Honestly I couldn't live with the guilt if I chose my son and they ended up both dying because I chose the one with the least chance of survival. Sometimes you have the make the truly right choice and not just the choice you want to make
@@gerdtt79Both decisions are emotionally valid, but Imho it only supports why she shouldn't even be allowed to make the decision. A lot of people have been saying they wouldn't even consider choosing the nephew (not saying this OP did but people have). Which is so hard. Especially if she's really close to her sister and our nephew. Personally, I think this should've been treated the same as any basic triage situation and they shouldn't get tried to make her choose/given her the authority to choose.
