    Our brain often overreacts and activates fight-or-flight when there is no real emergency and is instead reacting to normal worry and stress. What makes this worse is when we misinterpret the bodily symptoms as dangerous thereby increasing and prolonging out time in fight-or-flight. This can also result in neuro connections associating certain bodily sensations with anxiety, setting us up for panic disorder, a condition where our own internal bodily sensations trigger panic. It is therefore important to understand the actual body mechanics behind each type of bodily sensation during high anxiety and panic episodes. This video and PDF will review different symptoms of high anxiety and panic like heart palpitations, and rapid heart beats along with dizziness, nausea and difficulties breathing and explain the body mechanics of why you feel these symptoms and to help you normalize versus catastrophize these bodily sensations.

      @paigepradko thanks for your videos. It really helped me. I have pure o ocd from many years and trying to take medicine like paxidep for 3-4 daysbut it always increases my anxiety. Should i take it?

      @@edustudy5095 , the medication you mentioned is an SSRI antidepressant and is recommended for OCD. I am not a physician and cannot give medical advice. But, I recently heard a lecture from a psychiatrist that specializes in OCD. She said that having an anxiety reaction is a good sign that the medication will be effective in that person. She also recommended that people begin on smaller doses and increasing the dose every 2-4 weeks as the side effects decrease.

    I always get nausea and heart palpitations. I can’t sit still either. I have to keep moving. I’ve been severely agoraphobic for 8 years and OCD for a while too. My bf has OCD too and anxiety as well. This was very informative. Can’t wait to read the download!

    Thank you for your videos! For me, the anxiety happens during lectures with 150+ other students. I am almost certain it’s because I haven’t been in a physical classroom setting since March 2020, so readjusting has been extremely difficult for me. Seeing people and talking to them makes me anxious. I feel extreme panic even by just sitting in class, I start hyperventilating and sweating. I am trying to expose myself to these situations more and more so I can rewire my brain to not feel anxious, like it did before March 2020. What’s weird is that I never had these feelings of anxiety before, until the pandemic and lockdown/isolation began. Exposing yourself and learning to be comfortable with your anxiety is definitely easier said than done!

      Hi Mary. I am so sorry you are experiencing those symptoms. You are not alone. As a therapist that treats people with anxiety disorders including agoraphobia, I have never had so many calls and emails asking for help. We have heard and seen so fear messages about Covid and being close to others the past 2 years. We have also been more secluded and isolated than before the pandemic. Our brain has associated those 2 conditions. Therefore, when we leave our safe place, our home and are in different environments and are surrounded by others, our brain believes we are in danger. It is just a matter of time and exposures until your brain learns once again that you are safe. I have a playlist on how to do exposures on my playlist page if you need any extra help.

      Hi Faith. I am sorry that you are suffering from dpdr. That is one area that I continue to research and am dedicated to finding more effective ways to help people. I believe we are still at beginning stages of really understanding how to help the brain heal from dpdr. I have been particularly interested In polyvagal theory and how we can use the nervous system to heal dpdr. I hope to make another video about the latest research to heal dpdr this year. You haven't a clue how much it helps me when you make them and for all the people who commented in bout palpitations and heart flutters I feel some sense of relieve knowing I'm not alone . If I never had those horrible symptoms like flutters I'd probably recover from anxiety they set me back every time

    This is really good. It's eased me because I've had palpitations etc, worries that I might have a heartattack or worse. But it's human nature. The primordial can never disappear, we've named it fear and it's the beast in all of us that needs a leash. It's irrational most of the time, but your body fights potentially threatening situations. It just needs education.

    I like the word, very real but not dangerous... It makes me motivated and relax.. your right doc, if only I can understand the body mechanics it lower the level of anxiety and probably it well disappeared..thanks for your guidance doc, your excellent technique.. give me hope, so I can go on with my life.. and probably 1 day this anxiety & panic disorders well disappeared..

      Yes, I liked that phrase too. People feel real symptoms and sensations, but they are not dangerous when they are caused by anxiety.     Hi Paige, your videos on anxiety and sleep disturbance helped me a lot recently. I’m a new mother and have been having a hard time dealing with anxiety (particularly around sleep and health) and pregnancy/birth trauma.       Hi Charlotte. I am still working on the Agoraphobia course, and a course for OCD and Pure O. The OCD course will come out in March/April. Then, I will be focusing on Agoraphobia and Panic Disorder. An incredible amount of work goes into the development of a course and I want it to be tested before I offer it to the public. But, it is a top priority in my life to offer a guided, evidenced based program that walks people through step-by-step what to do during exposures. I plan on having more personalized treatment with this program as I know that people are often isolated and really need the guidance and support and encouragement.

    Bloating happens too. It's a terrible feeling because that can exacerbate the shortness of breath feeling and heart palpitations. Speaking from experience.

A lot of people including me suffering cause of this monster would really appreciate that. In particular it would be really helpful to understand how to apply ERP to this. Thank you

      Thank you for the suggestion. I have had clients with this theme, and want you to know that you are not alone. I think it is much more common than we realize because people are perhaps uncomfortable bringing it up. I will add this to my list. I can give you a few suggestions. Ask yourself what your core fear is underneath the jealousy. Is it that you fear you may lose your partner or that they might not be satisfied with you or that they might not be morally sound or right for you? There are endless fear scenarios and everyone is different. Once you understand your core fear, you can create ways to expose yourself to that core fear. You could do that by writing a script about your worse case scenario possibly happening. Then…the response prevention part of ERP is always the most important. What do you do when your fear is triggered? Do you ruminate? Ask for reassurance? Question your partner? Look at past pictures or texts or social media? You will have to resist all of those urges and compulsions….not just during the exposures…but forever. It is amazing how our brain can rewire when we practice response prevention all of the time. My next suggestion is to add the I. A.M. incidental ERP technique. This will keep you in ERP several times a day…every time the thoughts pop up.     Hi and thank you too you both for a GREAT video. Say you have a panick attack and you feel the symptoms you describe and your sure its your heart. After the all clear from all the test how do lose the fear of your heart after this triggers us. Thanks again.

      Hi Rick.
      What you are describing is a condition I see often and treat often. It begins with cardiac bodily sensations like chest pressure, palpitations and skipped beats brought on my high anxiety, stress or panic and the cardiac symptoms or discomfort becomes the source of the fear. Then, safety behaviors and avoidance behaviors and worry and reassurance seeking are added to the original fear. This can develop into cardio phobia/health anxiety. This can unfortunately really rob people of their joy in life if it is not treated and it is widely misunderstood in therapy. Talk therapy is not helpful here. What is helpful is fully understanding the reinforcing cycle and using similar methods used to treat OCD with heavy emphasis on response prevention.
      I treat people with and without cardiac medical diagnoses and have developed a course where I teach people step by step what to do to recover. The demand for help became so great during the pandemic that I felt the need to create the course to help more people.     Hi Paige,
    I had an ECG and a blood test both of which were normal. I asked my GP if my heart would be able to withstand exposure therapy and she said it would even though I am very underweight. I then asked her if I would be alright if my heart rate reached 150 beats per minute and stayed there through the whole exposure. She advised me if this happened to go to accident and emergency (A&E). My understanding of exposure therapy is to show the brain that panic attacks are not dangerous if I go to A& E won't that reinforce the belief that panic attacks are dangerous and further entrench my agoraphobia.       Hi Charlotte. Nice to see you again here. I have been wondering how you were. It is common for your heart rate to go up during exposures. However, I am not your physician and do not know the reasons why your physician would be concerned. Perhaps they are concerned about your weight and how that impacts your health. Would your physician also be against you raising your heart rate during exercise? It sounds like your physician has advised you on certain restrictions and this does make it more difficult to do exposures. And yes, if you went to the ER, your brain would not be learning that you are safe.

    Recently my latest symptom is feeling feverish and slight headaches. When I relax for about 30 minutes it goes away.

      Headaches can be a symptom of anxiety for some, but fever is not a symptom of anxiety. 😢

      @@PaigePradkoTherapy I have heard from other people too that they felt slightly feverish during times of anxiety but went away when they relaxed. Could that too be the issue, because it happens sometimes when am very anxious with heart palpitations and I have than 3 full blood works, 2 EKG test, 2 MRI test, 1 d dimmer, 1 head CT and 1 chest x ray and the doctors says am fine which lead to my recent health anxiety and panic disorder diagnosis. They only found out that I am iron deficient which am treating too.

      Hello to you Haroun from France. Yes…we can get sweaty hands …and all over when we are anxious. Try your best to welcome the sweatiness to stay, because this kind of message calms the brain by using the paradoxical approach of welcoming something that we do not want to have. The more we allow these uncomfortable symptoms the faster they will go away.

    Hello dr Paige first of all id like to thankyou so much as all your videos have been extremely helpful and i have a question im currently having ocd and generalized anxiety disorder. i have been following your techniques with the CALM method and also ERP and was wondering if using both methods together would be ok? Because i am afraid that it might cancel out each other primarily on the L (logic) part which isnt used in ERP.
    I read one of your replies on a comment on a different video where you said to not use cognitive restructuring before an ERP session since it calms the mind and reduces the effectiveness of ERP since the brain isnt surprised. My question is is it ok to use cognitive restructuring after an ERP session since i have both ocd and generalized anxiety disorder?

      Hi. That’s a great question and tells me that someone is actually listening to my videos. Lol. You can use cognitive restructuring for generalized anxiety, but you are correct, it does not have effectiveness for OCD. My advice to you would be to use methods for your anxiety that also help your ocd. What I am referring to would be methods where you fully accept and welcome anxiety. Anxiety is paradoxical. If we try not to have it, we make it worse by activating our fear center. If we welcome it, it calms down. Try your best to say things like, “I am willing whether anxiety shows up or not.” If you really do want to use cognitive restructuring, possibly wait until your ERP is complete.       If I were treating someone with both OCD and generalized anxiety, I would concentrate on ERP and acceptance based therapies like ACT and mindfulness based therapies. I would not do much cognitive restructuring, although sometimes logic makes its way into conversations. If I were working with a person without OCD that had generalized anxiety, I might use some cognitive restructuring with acceptance techniques as well. I tend to go with what works for that person…but stay away from using it with OCD. Boy…I am long winded tonight. 😂

    Thank you for the video! I suffer from OCD, and sometimes I get anxiety for no reason, especially when I'm doing exposures. It feels like something is wrong, but nothing really is, and it takes forever to go away(days, months). Is there anything I can do about this?

But does this thought ever go away, for me its been on my mind 24/7 for a few months now, I've tried accepting and carrying on with my day, but it just doesn't seem to work. Its also been interfering with work.

      Is it possible that you are engaging with the thought with mental compulsions like checking if it is there, wishing it away, ruminating about it, etc. Those kinds of mental activities can feed the cycle.     Hello paige i have another question regarding ocd. Whenever i have a compulsion to check something what i do is i picture a friend of mine who does not have ocd and ask my self what would he do if he was in my position and i would say he would not check and go on continuing his day and i should also follow in his path and by using this i find it easier to resist doing my compulsions so I want to ask if this is a form or reassurance seeking or not?

      I like your method. Sometimes it helps us by asking ourselves, “What would someone do in this situation who didn’t have ocd?” The only issue could be if you do that every single time, it could become a ritual or a compulsion itself. If used on a brief occasion, than it would help you to not do the compulsion.

    Hello Paige, I’m Paul how are you keeping? I was hospitalised for 6 months back in 1992. I had awful thoughts that everything I touched was dirty & I thought I would go blind if I stood beside dogs dirt on the pavement. I also got thoughts I wanted my mother to die & that I wanted horrible things to happen to missing children. Used to pour over all these stories in the papers. It was really terrible. Thought I was never told I had OCD at the time though. Only that I had bad anxiety. Do you think I should go back to the doctors for an up to date diagnosis?? I’m very certain I have OCD. Though I don’t feel as bad now as what I did.

      Hi Paul. Yes, it absolutely sounds like you had OCD. If you are not having any flair ups, you are not in need of treatment or a diagnosis. But, if you do need help, you will want to make sure you have the correct diagnosis and treatment. I am so sorry you experienced that and likely did not get effective treatment at the time.

    What about high blood pressure during anxiety period is it dangerous or it will resolve on its own when we get the anxiety under control

      That’s a great question and I am sure this depends on the person and if it is solely driven by anxiety. Anxiety symptoms themselves are not harmful or dangerous.

    I have gad, but moust symptoms are overlap with depression too. Is that means that may also may have depression? I think it is normal to be depressed when suffering with anxiety. No one can be feel glad or happy for having anxiety.

      Our moods vary day by day, and even minute by minute. You are correct in that anxiety can sometimes cause a person to feel depressed.

    I definitely have health anxiety, as well as OCD. Where and how can I find a therapist who specializes in this that takes insurance. I had a therapist but she didn't understand OCD so it was very confusing for me so I kind of just stopped going.

    Iam a medical student I had anxiety as early as I remember it got worse during the college days and I began to experience panic attacks.
    during my psychiatry round I was told that ocd is incurable horrible disease and after that I began to worry that I might have it... After that it all went bad, I began to experience intrusive thoughts of sexual nature and would panic that this must mean that I actually have ocd l couldn't sleep or eat and became very concerned with my mental health l would watch videos about ocd and how to treat it all days.. Then I would worry about having this particular theme or that and then have it and so on.. So what should I do should I continue to educate myself about ocd and how to overcome it or should I treat it as health anxiety and don't look it up and accept having ocd without doing anything about it.
    But when I have intrusive thoughts l feel this urge to search the Internet for way to get over ocd and would search for days and days about it.
    So how can I do erp for this type of worry.
    I hope you can guide me.

    Hello Ma’am. I am doing erp for my ocd rumination. I had thoughts like what if I don’t recover, when will this feeling go away. General sadness feeling. I am doing active erp for all the thoughts. I am suffering from male pattern baldness. Which is also a worry for me and even before ocd hit I was thinking of hair transplant in near future. Not very soon though. Through erp I have mostly worked on my major ocd thoughts until 2 days ago my hair loss ocd hit kinda bad. Now I can’t stop looking at each persons hair that I meet or see passing by. Just checking their hair loss or no hair loss. How to do erp for something that I see 1000 times or 1000 people every day. Now I can’t even look at their faces but my eyes straight away go to their hair. Help please. Thank you :)

      Hi Aditya. I am so sorry that you are suffering. OCD can really be initiated from any thought or image, literally anything. But, what I want you to think about is what your core fear may be. Are you afraid that no one will be attracted to you and you will be alone? What is the fear and what are you having doubt or uncertainty about? Then, you can do ERP on something that triggers both your obsessions and your core fear. Your compulsions seem to be looking at people’s hairline and rumination. Do your best to stop the compulsions.     Im sorry about my english ) Hello..I want your help..I suffer from obsessive-compulsive disorder in checking for two years..but in the recent period I have been suffering from a lot of pressure, not regarding what raises my suspicions..but from the doubt itself..I was in a situation that forced me to make sure As usual..I suffer from severe pressure and I just want to get rid of it. I don’t care like before if it is right or wrong..If I finish quickly, I feel comfortable, then my conscience builds that I did not resist my doubts, but what really hurts me is that I will do it again, not because I am wrong.. This is an obsession that I must resist or use it to get rid of the first obsession.!!?
    Help me please

      Yes…your sensations and symptoms like to jump around. It seems like as soon as your brain accepts that something isn’t dangerous, it jumps to something else to get your attention again. I am sure that is very frustrating.

      It is likely related to OCD health anxiety. Because you notice the shoulder tension and have some concern about it, the brain believes it must be dangerous. And because of that, your brain keeps reminding you. And this creates a hyper focus on that area of the body.
      If You were to focus on the big toe of your left foot, you would begin to feel your toe. Before I said that, you didn’t notice your toe. Now if you noticed your toe had an ache and you began to check on your toe with concern, it would soon become a major focus causing anxiety.
      Your shoulders have many muscles and ligaments and facia …there is a lot going on around your shoulders. It is quite normal to feel tension there when stressed. However, most people’s attention does not stay fixated on the shoulder unless there was indeed a real physical problem that would show up on an mri. If there is no real physical problem, then it is likely psychological and caused by obsessively checking and hyper focusing on your shoulder. Treatment for health anxiety is what I would recommend.

      @@PaigePradkoTherapy yeah it's the pain the subconscious must be focusing on it.ive had full MRI spinal nothing found exrays on shoulders diagnosed fybro spectrum disorder.and migraine of the body by neurologist.its these unspecific diagnosis what gave me more anxiety.perfectionism and anxiety hi hand in hand.like u said by a TMS doctor agrees with yourself a OCD pain disorder.by the way have people with OCD got very good long term memories because seams to me they have as long as meditation hasn't damaged that.people talk about anxiety muscle tension mine never lets up I had a haitus hernia before muscle skeletal started that was horrendous none stop acid.sure anxiety made that severe to.

      Yes, the symptoms that bother you and attract your hyper focus seem to shift around your body. Do your best to not respond and shift your focus to what you are doing instead of what you notice in your body.

      Many physicians are not concerned about bodily responses to anxiety because they are not physically dangerous. I do wish physicians were trained in recognizing symptoms of anxiety disorders, panic and health anxiety.
