Actually doctors could do something about it, depending on "how bad" it is, either reduce it (close to normal) or almost return it to normal. But most of the time (if not all), it's the fighters themselves that doesn't want the procedure to be done. It's a fighter's pride thing, they are proud to have the "fighter's ear" (as some would call it).
Submission moves can cause it too, basically it's damage to the ears, it scars over and over again.

It's because Shiho had a hard time when she gave birth to Sarang. She got really sick and her body broke down really bad. That's why Sunghoon doesn't want her to have another baby because it's too dangerous esp since she is also now over 40 yo. Plus Sarang herself said she doesn't want siblings at all. As long as she grows up well, is loved by her parents and have many friends to compensate for being an only child, that should be more than enough.

Cutting that much weight would be hard, but it actually puts him at a heavy advantage not a disadvantage.
Too much weight lost in a short period of time is still a disadvantage. Fighters are usually comfortable in their weight class, the body moves more "normally" when they are at their weight. Which is why not many fighters, boxers, etc would change weight class and even some who do to a certain extend, it's usually +1 or -1. Going 2 weight class difference have a much larger effect on the body. Not to mention he only had about 1.5 months to lose those weight.
There are some advantage but in this case it's only minor. Why do you think Aoki was so insistent on sticking to his weight class instead of the typical compromise of meeting in the middle. It's "fairer" too with each of them moving 1 weight class, besides it was Aoki who was calling Sunghoon out. Aoki wouldn't just hand over such an advantage, since he was aiming to win. People in those industry would be way clearer on the perceived advantage/disadvantage.
