Thank you. I haven't needed a lab test as I always know when I struggle to fall and stay asleep. As a vegetarian, I have been giving myself injections for years. Sometimes I forget but are reminded as I don't sleep well. Vit B12 always fixes that.

If serum B12 is high (with B12 defic. symptoms and feel better taking B12 in small amounts) what might mean and what labs useful? Did wonder if people with high B12 serum levels were finding some was inactive and how to get more active. Did MMA urine but not sure what optimal labs. Methylmalonic Acid, U 5.2 umol/L 1.6-29.7 , MMA - Normalized 1.4 umol/mmol cr 0.5-3.4 , Creatinine(Crt), U 0.42 g/L 0.30-3.00 . Usually MCV 94-97 (ok by lab range but...). Thank you for your explaining B12 and other labs to determine if /which problem.

Hi Dr Jin I’m on carnivor diet and my vitamin B12 is 1269 and I’m not supplementing with vitamin B12, should I be concerned about elevated B12 level??
My MCV is slightly higher than the lab range. Is that enough to confirm B12 deficiency?

Hi, I have almost identical lab numbers as you. I have not had MMA tested yet. What other symptoms do you have? I have had fatigue although it has gotten much better with vitamin d supplementation and some nerve issues. Do you supplement with B12? I do and that is why blood levels were high I would imagine. High B12 doesn't normally cause symptoms but it is possible.
My b12 test came back 204 pg/ml... I eat meat and eggs every day.... Why would I be low?

No thoughts on HoloTC? I had high normal b12 bloodserum and normal low HoloTC (active B12). My doctor abroad suggested MMA test but my dr here says there is no need because Im not deficent. But I took high doses B12 until 2 weeks before the HoloTC test and dr abroad thinks I still could be deficient that it is still from high dose supplement. I improved after taking this high dose (20000% RDA) symptom wise.
Half-life of B12 is approximately 6 days.
So to get a reliable result you must suspend B12 at least 1 month before the tests (but longer would be better) otherwise you will have a "false high"
Remember that B12 also requires riboflavin (B2) to be used correctly and works well in synergy with folic acid.
It would be a good idea to take a B Complex that contains the entire B group as a starting point so as not to have deficiencies in some B group vitamins and combine it with a specific high-dose B12 supplement (better sublingual)
If you have symptoms and B12 makes you feel better, take it without worrying about tests.
However MMA + Homocysteine are reliable markers.
If you take tests, make sure you have not taken Biotin in the previous days because it can alter the analyses.
@@stefanozebralight518 I suspended all supplements 2 weeks before the bloodtest was done to avoid the worst false results, because I couldnt do earlier as I planned to go abroad kind of last minute with those 2 weeks. Im going to take an activated B complex as soon as I am able to get it. My country is just an isolated island where things often are not available or out of stock for a long time, and ordering abroad brings huge import fees with it. I prefer to go abroad and shop there
MMA test came back ok but homosistine test came back pretty high. Could that indicate B12 still . Or must both be positive . Thanks

What next if you test these, MMA, Homocysteine, Serum, and all appears normal. but one is still having symptoms of deficiency.
Maybe you might not have a B12 deficiency and need to check other markers. One can also check intrinsic factor antibodies.
I am from the UK, I was told for years I had vitamin b12 deficiency. I had injections every 12 weeks for this for years, I had a blood test about 5 years later for something unrelated and then was told I hadn't got b12 deficiency anymore and they refused to give me any more injections even though I still have all the symptoms. I have stomach issues (motility disorder, ibs and other ongoing investigations to do with my stomach) is that related to b12? I have read somewhere before the intestines don't absorb b12 from food if you have stomach issues, but surely it would show in a blood test now?
I'd like to know too as I have several stomach issues and was told my b12 was getting too high and I needed to stop. Still have stomach issues and fatigued, etc....

Edit: If you experience fatigue from anxiety and lack of sleep, also check vitamin D. My suggestion is to drink a B-complex and vitamin D in the morning (seperate them by an hour), and magnesium before bed, for several months, at least.
And sleep as much as you want. Even during the day, before 3-4 pm, so it doesn't disturb your night sleep. Sleep repairs everything, especially the deep phase of sleep.
@@randombitsofreality Thank you so much for your kind reply. I was taking B1...500mg, because I read that it helps break down carbs and had other benefits for those with digestive issues. I was fighting h. Pylori/gastritis, gallbladder issues, slow motility etc. The pylori is gone, as well as the gastritis for now. Oh...I also had SIBO..I truly believe they're all interconnected. Still have slow motility and gallbladder issues which I'm dealing with using various appropriate suppliments. I know I'm not breaking down some important nutrients because my nails, skin and joints are weak, dry, and painful...anyway I'll go back to the B1. I do take D, 10,000 daily with k2, also magnesium. Thank you so much..
Forgot...I also take unfortified nutritional yeast daily as well.
Sorry...I forgot to say why I stopped B1...it didn't seem to do anything...took it for a month or so. Maybe wasn't high enough dosage...
My Dr. Told me i had too much B12 to stop taking b vitamins
If you are taking B12, serum B12 will be high. Doesn't mean you should stop taking it.
I have the same issue of having high B12 level in the blood test. It is more than 2000! The level drops down to 780(quite rapidly) after I stopped taking any B12 supplement for two months. Does it mean that our body needs B12 everyday?

What is the optimal range for these markers?
Lab reference range is 0-378 nmol/L. Anything above 300 would peak my interest, and I would run homocysteine and MCV. Best, Dr. Jin

There is no gold standard b12 perfect definitive b12 test
MMA, MCV, Homocysteine just like the b12 serum blood test, also cannot be tested when on b12, takes 4mths to leave your system
For God sake, I'm having gastritis problems since ever and no doctor could tell me that could be because of my vegetarian diet... 😢
Thats guaranteed b12, iron, vitamin a, zinc deficiency just for starters
I would do a youtube search for your symptoms.
