It is unclear what exactly it is about, but there are several versions:
•It is literally about a man nicknamed Cotton Eyed Joe who stole the main character’s gf and left. Cotton-eyed Joe could be a black man with very light blue eyes, an alcoholic or a man with a cataract.
•”Cotton-eyed” could be due to alcoholism, since it was broadly used at the time to refer to drunk people. The main character is an alcoholic and thus unable to get any pussy.
•Syphilis.
The STDs theory is considered to be the least likely one though and the one about stealing girlfriends the most likely one. One of the earliest published versions literally refers to Joe using a variation of the N word, so it is highly unlikely that he is a metaphor for a disease.

Every day I realize how many people don't know the origins of this song. The version you probably know is by the Rednex, a Swedish band. If you know Eurodance, it makes perfect sense, but back before the internet, we just assumed it was an American song. Which it is, originally. The exact origin, and even meaning, is unknown. Folklore holds that it was sung prior to the Civil War by slaves on the plantations, or else by folk singers travelling around the South. The earliest publication was in 1882. As for the meaning of the phrase "cotton-eyed Joe", it could be any number of things. Glaucoma, cataracts, or other type of eye problem leads to a milky-white, "cotton-like" appearance. Drinking too much moonshine could cause a person to go blind. Syphilis could do it, though it might not be an STD and could be because of poor hygiene in coal mines, where bacteria can be carried in goggles. And of course, there's the idea that Joe is simply missing an eye and stuffs cotton in his socket.
