Prometric Skills Demo for Foot Care

  • According to Prometric guidelines for the nurse aide state exam, this is a step-by-step demonstration video for foot care. This skill is one of the 21 testable skills on the Texas State CNA Exam. Remember to include indirect care with this skill. You will start this skill with the client sitting in a chair. You must support the leg at all times while providing care and NEVER let the foot touch the floor.

    Thank you Dr. Lee! I watch all your videos and i pass my clinical test ( Midand TX) I had foot care and the part when you clearly says no lotion between the toes... i was talking to my self outloud during testing and i literally repeat word for word" " Dr Lee says no lotion between the toes" the evaluator busted out laughing 😆 😂 🤣 I couldn't keep a straight face anymore i laugh and she says " Dr. Lee has this effect on her students🤣( She must know you or came across alot of student that watch you on CZcams.😂🤣)

    I am so glad I found your videos! I am currently working as a PCA at the same hospital where I received training, but I am just getting to take my state exam. I was nervous for the exam because I have only worked on a labor & delivery unit so I haven’t had much practice with ADL’s such as foot care and ROM skills.     Your videos are amazing! Thank you so much for all you do. My skills exam is next week and I was wondering if you knew if Prometric CNA Skills testing in Texas will still include feeding, denture, and mouth care even during the pandemic. I know that previously Pearson said they would not be testing such skills due to COVID-19 concerns. :)

      Hello Amber,
      During the COVID 19 epidemic Prometric will not ask students to take off their masks due to infection control precautions. Therefore, feeding, denture care, and brushing of the teeth will not be tested.

    Dr Lee I am so grateful to have found your channel. You make the skills easy to understand. I have two questions. 1. With the increasing Covid-19 Delta Variant numbers here in Texas is Prometric testing on mouth care with teeth, mouth care with dentures, and feeding at this time? 2. I know we are tested one skill from every category, but is it also from each level of difficulty i.e. most, moderate, and least?

      Hello Amie,
      Prometric is not testing on the oral skill at this time in Texas which are denture care, brushing the teeth, and feeding due to COVID-19.
      Prometric does not use the difficulty level for grouping skills for testing. Each candidate will be given the following:
      1. Handwashing
      2. Indirect Care
      3. An ADL Skill
      3. A measurement Skill
      4. A mobility Skill

      Hello Bridgett, It is a habit for me. When I rinse the basins, I consider the basins clean. I still have gloves on, which are dirty. The state wants the gloves to stay on.
      So, I use a paper towel to keep my dirty gloved hands from touching the clean basin.

    Hi, thanks for the informational video. With pearson Vue there was no need to fold the cloth in four. There is no open wound , Is it really necessary to fold the cloth?

    I have a question. If you do not wear the gloves do you just wash your hands before and after or is there a time inbetween the foot care skill that you wash your hands?

      Hello LaShane. When providing foot care you will wash your hands three times.
      1. Right after introducing yourself to the client and gathering your supplies.
      2. After putting away from dirty supplies and before going back to the client for the closing.
      3. At the end, Just before saying you are through with the skill.

    Hey ms Lee , what other technique can I use other than holding the patient’s foot in the air ? I feel it might be too heavy to hold until the end of the skill .

      Hello Julia, You can place a towel over your knee and ask the patient if he/she is comfortable with placing the leg on your knew on top of the towel. This will provide support for the leg while providing foot care. Be sure to get the patient's permission.

      Hello Albert, You are correct. Prometric does not care how we hold the washcloth when washing the foot. You are only graded on the steps listed in the Skills checklist with indirect care skills.

      Hello Lillian, I normally place my washcloths in the water before I start to use them. This is a personal choice.

    On my checklist it mentions something about using orange stock to clean under nails and shape nails with emery board? Is that not required anymore?

      Hello Marlene, Yes, an orange wooden stick is required to clean under the finger nails and NOT THE FOOT CARE. We don't put sharp things on the the feet because a person may be diabetic.

      Hello Brittney, If a lot of lotion is present, you MUST pat dry the excess lotion. This is a requirement.

    Hey Dr.Lee , is there a required amount of time for soaking the foot. I looked at the checklist from prometric but, there isn't a spesfied time. I just need to soak the foot accodring to the checklist. However, my nursing skills text book recomends 30 mintues of soaking per foot.

      Hello, This is a great question. The state guidelines DOES NOT require a specific time for soaking the foot during the state exam. The candidate only need to place the foot in the water and them lift the foot out to start the washing process.
      Remember, during the state exam, the candidate is timed to complete the 5 skills. Therefore, don't keep the foot in the water too long, or you may run out of time. I teach my students to place the foot in the water for about 30 seconds and then start the washing process.

      Hello Essence, I introduce myself to the client, tell the client what I would like to do, pull the curtain for privacy, and then wash my hand BEFORE getting my clean supplies.

      1. Prometric requires the student wash ONE foot.
      2. Prometric requires the student in step 3 to:
      Place water filled basin on protective barrier on floor to prepare for foot care? Prometric does not specify what the protective barrier should be. Therefore it can be a towel, chux, sheet, or blanket, etc... The bottom line is that the basin can not be placed on the bare floor.

      Hello, as of today (2-16-22) we are still not doing the oral skills. The students will have to get the supplies needed for each skill. All supplies will be in the testing room.

    Hi Dr. Lee, I have a question about foot care and I think I recall it being me mentioned in class. If we’re agile enough, which I am, I think you said we could sit on the floor to provide foot care as long as patient bare foot is on the towel. Am i correct?

      Hello, Yes you are correct. Prometric does not care about the position of the candidate. They ONLY care about the patient. The patient MUST be in a chair and the foot CAN NOT touch the floor. So, you are my student. HA HA

      Hello Oyin, Foot care is performed on a live person. You will remove the socks and shoes before washing and replace them after footcare.

    Dr Lee ,a quick question when sitting for your state exam do you perform the skill on one foot or both?

      Hello, The candidate can be in whatever position is comfortable for the candidate.
      The patient MUST be in a chair.

      Hello, Prometric does not require the student or patient to have on gloves when testing the water.

    Hi Dr. Lee I will be taking my test on Saturday, for the clinical test do I need to take a partner with me to do the clinical test? What if my partner is taking the test at the same time ?

      Hello Madisson, The evaluator will assign you a partner for the exam. You can't bring anyone to the exam.

      Prometric NEVER requires a student to kneel in front of a resident for foot care. That would be hard for health care workers with arthritis to do. How strange for someone to teach that. WOW

    I take my skills test Wednesday. My professor told us if we mess up at all we fail. I think she said that to scare us into doing our best but it has made me much more anxious. What is the true passing requirements? I have down all basic skills and hand washing and IC through out skills but I’m nervous to forget the order or some steps in doing the actual skill. What determines if we pass or fail?

      Hello Kendall, Prometric does require the candidate to pass all five skills to be successful with the state exam. Now, with that being said you don't have to do everything perfect. Prometric has some steps that are considered critical steps. Prometric does not tell us what those steps are. So, you can miss some steps and still pass the exam. Pay attention to all steps involving safety, patient's right, privacy, and infection control. Those are critical when testing.

      Hello Omawumi, Prometric guidelines state to replace the person's sock and shoe. Remember this will be on your partner, so you will just put back on what they wore to the testing site.

      Hello Peter, Prometric does NOT require gloves for this skill. If you want to wear gloves, you can put the gloves on after washing your hands. There is not credit for doing so.
