The reason he looks very ugly was because when he was a baby, his Mom Fratelli dropped him on the floor which made his face look ugly which also scared him from 1:02-1:14 where I guess it made him remember about what happened to him. (I'm guessing that was the reason he decided to not join his family members as a bad guy because of his anger on how his mom made him look ugly by dropping him).
