Then chant:
Namo Tassa Bhagavato Arahato Samma Sam Buddhassa (3 times)
Om-namo-put-tha-ya, Na-ma-pat-tha, Na-mae-ti, Lick-lue, Lick-lue, Na-ma-si-ta-wa, Pu-lersi-ta-fai, Sit-ti-tae-cho, Ma-ha-pun-yo, Ma-ha-ya-so, Ma-ha-la-pho, It-thi-rit, Pra-si-ti-mae.

According to some other traditions/lineages, they say that Lersi Por Gae Ta Fai/ Phra Lersi Ta Fai is actually an emanation of Phra Siwa (Mahadev Lord Shiva) himself as he's also the lord of all the Rishi/Ruesi/Lersi magickal/sorcery traditions due to the 3rd Eye association between the two.
