SUMMARY
The urinary and genital systems develop from mesodermal tissue.
Three urinary systems develop in temporal sequence from the cranial segments to the caudal:
- The pronephros, which is formed in the cervical region and is vestigial.
- The mesonephros, which is formed in the thoracic and lumbar regions, is large and is characterized by the presence of excretory units (nephrons) and by having its own collecting duct, the mesonephric or Wolff duct. In humans it may function for a short time, but most of this system disappears. Ducts and tubules derived from the mesonephros form a conduit for the emission of sperm from the testes to the urethra. In women these ducts regress.
- The metanephros, or definitive kidney, develops from two sources. It forms its own excretory tubules or nephrons like the mesonephros, but its collecting system originates from the ureteral bud, an evagination of the mesonephric duct. This bud gives rise to the ureter, the renal pelvis and the calyces, as well as the entire collecting system. The connection between the collecting duct and the tubular excretion systems is essential for normal development. WT1, which is expressed in the mesenchyme, allows this tissue to be competent to respond to induction caused by the ureteral bud. The interactions between the yolk and the mesenchyme develop through the synthesis of GDNF and HGF in the latter, and their tyrosine kinase receptors RET and MET, respectively, which are produced in the ureteral epithelium. PAX2 and WNT4, which are synthesized in the metanephric mesenchyme, induce epithelialization and differentiation of excretory tubules. Early division of the ureteral bud can produce bifid or supernumerary kidneys with ectopic ureters.
Because the kidneys develop in the pelvic region and then "rise" to their location in the lumbar region, kidneys may be seen in abnormal positions, such as the pelvic and horseshoe kidney.
The bladder forms during the fourth to seventh weeks as the cloaca, the common drainage site for the primitive kidneys and the intestinal system, is subdivided, by effect of the urorectal septum, into the urogenital sinus (anterior region) and anal canal. (posterior region). The anterior portion of this breast then differentiates to form the bladder. The remaining portion of the breast differentiates into the prostatic and membranous segments of the urethra in men, and the urethra in women.
