  • An MCL injury of the knee is one of the most common injuries athletes suffer. While you might not need surgery for an MCL tear, it can take a long time to heal and return to sports. In this Ask Dr. Geier video, I discuss the initial treatment and rehab of this common knee injury, including when you can walk with an MCL injury.

    Earl asks:
Can you walk on grade 3 MCL injury?
    WHAT IS AN MCL INJURY?
The medial collateral ligament (MCL) is the thick ligament on the side of the knee closest to the midline of your body. You can suffer an MCL injury when someone hits you on the outside of your knee. Or you can cut to change directions and injure it.
TREATMENT FOR A SPRAINED OR TORN MCL
Fortunately, most MCL injuries do not require surgery. You can often recover in 6 weeks or less depending on how severe the injury is. You might wear a brace and do physical therapy to regain knee range of motion and strength.
WHEN CAN YOU WALK WITH AN MCL INJURY?
In this video, I discuss when you can walk, exercise and return to play sports after an MCL injury and how to do it safely.

My knee swelled slightly overnight so I used 'Deep Freeze' to cool the knee. Dear Doc Geier,
I was up and down a tall ladder in a high end home, and I had to be in sox but I chose Crocs and footies. Up and down again and again till I missed a rung terribly twisted
at the knee joint and came away with a sore MCL area. More a sore spot about the size of a dime. I am almost 65 but in very good shape, a non-competitive bodybuilder.
Then, for weeks I have not stopped, continuing to do the work I had to get done which involved more ladders, staging, crawling kneeling and so forth. I went to a hinged brace right away but after a week of hard work it started to wear a spot in my calf. My knee, inside MCL area went to an 11 for pain. Here we are a month later, the spot on my calf has healed, I am back in the brace but I am still in pain, get up from bed cannot walk but slowly returns, using a crutch at work to take the strike out of my stride and have only taken a bit of time off, which seemed to help a little. I have ordered an immobilizing brace. I message each evening with castor oil and that pain is still radiating from that inside spot in line with knee center.
This morning my foot on that leg, and not the other is swollen or retaining water? I hate going to the hospital and have no insurance.

I was cycling and fell on my bike, sprained both knees but one knee stopped hurting after about 2 weeks. The other knee though, I got diagnosed with a grade 2 MCL, and a small fracture in the lateral femoral condyle. Between waiting to see how I felt after a week, doctors appointments, MRI appointments, and being referred to an orthopedic surgeon, was about 4 weeks after I jury before I saw an orthopedic. He has me wearing a hinged brace now for 6-8 weeks, I was only wearing a cheap brace from Walmart before, but their strongest available. Was doing some light cycling every few days before, but orthopedic surgeon now says to stop all exercise for 3 weeks until a follow up visit

I got knocked down on the deck by our sweet two-year-old Golden retriever and ended up with a grade two MCL injury and a bone bruise. It has been almost three weeks and I have not yet been able to put full weight on the injured knee due to pain and a wobbly feeling knee. This feels very scary to me as I have an underlying Muscular Dystrophy diagnosis which puts me at risk for irreversible atrophy of my muscles. The injured knee was my stronger, more reliable leg, so overall this is a hard pill to swallow. Yesterday I got in the therapy pool I go to and was able to walk in the water with some pain and stiffness. The shallower water was very difficult. I will be starting to work on walking on a treadmill with a hoist to keep me from falling or putting too much weight. I have been working on straightening with exercises. I am praying that walking and doing other strengthening exercises in the pool, will get me back to walking again.

The doctor at my local hospital said I had an MCL tear. My therapist thinks the same and gave me strengthening exercises to do until our next appointment. He is also going to put me in a back-brace as he reckons my Degenerative Disk Disease (app. 12 years now) is causing my knees extra work, therefore I do not recover fast enough from the MCL injury. It has now been 6-weeks and although I walk short distances without help, I still suffer from severe pain by night time. Prior to my injury I was walking 12 - 15km per day, at least 3 times a week, now I can barely walk to the gate. I got my MCL torn on a motorcycle accident. I had a huge bruise on the side of my knee. After several weeks I noticed my lower leg was very unstable and like I could not control my foot. I went to the Dr and after all testing and everything it was determined my MCL was torn. Because of testing my surgery was delayed like over a month and the knee seemed to be getting worse. I got the MCL surgery done today and I am walking on crutches and I have an ice pack machine to the knee. The orthopedic said it was going to be a 3 month recovery. Right now because of stitches I have to have it wrapped for 3 days to avoid infection. By this video I can say I got the worst kind of MCL injury. In addition to the MCL my ACL is also sprained. I got pictures of it in surgery but I don't know how bad it is yet until my follow up appointment. I know it's not torn but in the pictures it looks like it has hairs coming out of it.

Nice! I have a grade I tear after what should have been nothing more than a really slow, irrelevant snowboard fall. I felt the pain immediately, and severe point tenderness remains three weeks later at a tiny point at the most proximal area of the femoral medial epicondyle. There is no laxity or other dysfunction other than due to pain, which remains a big problem: It's my Left leg, so I can't lift my leg into the car after I first get in (too painful--must use arm to 'put' it in). Can't really exercise, so I thought I'd do some swimming: and made the mistake of taking a single breaststroke kick--which is dependent upon two intact MCLs! HURT like hell! I had to signal to the lifeguard it was just my knee when he heard me scream and was halfway into the water after me.

I partially tore my mcl almost 4 weeks ago. I could walk on it but mainly on my toes but with a brace I could use my foot. I have a limp, can’t bend my knee in certain angles, and can’t straighten my knee. It’s been a pain and I miss having full activity.

Hello Dr. Geier. I am 56 and I injured my knee on haloween by walking backwards and sliding side/backward and extending my lower leg. I and am waiting for my MRI this coming monday. I think I torn my MCL badly. I am walking in a hinged brace and I have no pain, but if I put inward preasure on my lower leg I can feel knee joint movement with a sharp pain if I do it unkowingly that goes away quickly. Also if I take my weight off my leg, dangle it and lightly sway it side to side my knee moves and feels like it opens on the inside or would pop out of place if I did it hard enough. It has been 3 weeks today. There is less movement thab=n there was 2 weeks ago but I still wouldnt walk around for any amount of time without my brace on. With covid I cannot get in to see any specialist and my MRI I am paying at a private clinic because the wait time to go through the normal channels is approx 4 months. Thought? Many Thanks.

My right leg completely run over by a vehicle. The accident caused grade three MCL tear, torn ACL, damage to the patella, Meniscus tear. My Orthopedic doctor said I will heal on my own. 1 year later, I am still in a hinged knee brace because I have a fear of more damage. My doctor said my MCL healed very tight. It has a very small give. He also said, Me still being in a brace is more mental than physical. I have no pain, but I am tired of the brace.

My doc said I have torn my MCL (grade 3) through the joint line which he said is unusual compared to tears off the attachment points that they can normally go in straight away and operate.
I got told I have to wear a brace and on crutches for 12 weeks before they operate on my ACL (grade 3), I also have torn my lateral meniscus. It sucks. I'm not weight bearing on this leg till I get surgery I don't feel confident otherwise.

I'm basketball player and i got knee injury issues. It's been a year since i got the injury. It felt like a half dislocated and i decided go to hospital and got x-ray for is there any bone injury? There was no bone cracked. And i didn't know about MRI and i healed with traditional medical and it was very successful. Few days ago in April 24th i jumped from a very high and again i just pumped my knee it goes to inside i think it didn't dislocated but it get tension or maybe torn MCL so a lot of medical thing i used pain killer some kind of little bit effective also i drink tablet for against the sore and now I'm using band to avoid my knee goes to side to side. Now I'm walking, no running no fast walking only walk please doc give me some information help me

Hi doctor, thanks for the info . I have a ligament tear since 2 months, the first month I didn’t know that i had a ligament tear just at that moment when I strained my knee I heard my thigh bone make a sound moving sideway from my down leg bone
And it got back to the opposite side at that moment, after that I didn’t complain from anything unless when i bend my knee it makes a sound and hurts unless it got back to as it was , so the first month i walked on it without caution, then I went to a doctor he told me I should use a knee brace, so i did ,then it begins to swell a little, i went to another doctor he injected my knee with hyaluronic injection, after this injection for two weeks till now I can’t step on my knee still swollen and i fell insecure about walking even with my knee brace on , he told me to start knee training after two weeks of absolutely no stepping on it . What should i do and why it’s still swollen after the injection?

I suffered from MCL injury on June 11th. Had 10 physio treatments, and I do have a brace (suggested by physiotherapist.) Brace makes knee and inside of knee worse. Xray a couple days ago says: mild medial compartment predominant OA. Questionable soft tissue swelling near MCL origin. I'm still in pain. Had gotten better, but they discharged me. How much longer am I going to suffer? I'm 59. Nobody is treating me anymore. 😂

Im a 59 year old female with osteoarthritis and bone on bone in both knees.... I am able to get around and do things for the most part but cannot walk for a long lenghts of time. I recently had an MCL injury on my left knee I have no idea how it happened I was walking that day later that night and was sitting on my reclining chair when I got up I could barely walk. I went to the doctor was put on steroids to help with the swelling and I could not put any weight on that knee at all it’s been about two weeks and I’m just now being able to walk somewhat with the help of a crutch but I can tell it’s slowly getting better. . I cannot do my stationary bike as it hurts to bend it too far so I’m doing some exercises I have learned in the past from my physical therapist. I have to say this has been a very painful experience. It’s also hard to know when you can start doing more exercises without redamaging the ligament.

When it regained traction it instantly threw me left then right and left again and during that, my left leg touchdown and twisted hard i cant recall in what direction tho. I have a complete ACL tear on that same leg, so i remember vividly how it feels when a ligament tears. This popping or stretch sensation, followed by that sharp pain...I kept working for an hour and went home. It kept swelling up and the pain got worse so i went to the hospital. They x-rayed my knee and nothings broken, but they suspect the same thing as i thought, either a grade 1 MCL tear, or a meniscus tear. Pretty sure it's the MCL tho.

Hi I just watched your video on an MCL tear which I have since getting caught in a high tidal wave at the Jersey shore. Came down on on my knee after being tossed around like a rag doll. I immediatly had pain and hard time getting out of the water because I could not walk. Once I did get back to my beach chair it started turning blue and pain going down the side of my leg. My husband took me to the nearest Medi Center where they determinded I did not have a fracture but they did see something in there. They put me on crutches and a knee brace with the advice to see my otho dr at home. Dr home said I have a grade 2 tear. Which I have to wear a hinged knee brace for 8 weeks and then physical therapy . My question is the type of brace i am in moves around to the side. I strap it tight as can be but then it is too tight. At times i get a popping noise and terrible pain. I called Doc. He said use the crutches and stay off it. For 8 weeks ? !!! What do you think

I am looking at having an MCL surgery done. I have had two other knee surgeries. One was an ACL surgery with the graft from the patellar tendon. The other was just an exploratory surgery and removal of torn cartilage. My main question is, I have always healed from traditional surgery much faster than through arthroscopic. Would someone like yourself be comfortable doing a traditional MCL surgery with the traditional longer incision? I have a grade three and it is been there for quite some time. I mean over two to three years. My knee has motion from side to side, and it will also lock up and pop. I am assuming this is all normal behavior for a torn MCL. I have had not much pain, but I have had my whole knee migrating in towards my other knee and the inside of my right knee, the affectived knee, is quite swollen and we'll never go down. My question is, how long would recovery take with that significant of an injury and my history of knee surgery?

also ive been stretching a lot my knee and it went from 100 pain to 5% and i do lunges in all

    • @cynthiaclaffey1522
      @cynthiaclaffey1522 Před 2 lety +1

Well, I think while it’s healing & still pain stay off of it. Let it heal first, mine took 3 months. But I did try to straighten it while healing as much as possible but it was so painful couldn’t do much. I’d start the physical therapy as soon as the doctor says it’s ok, they help you know how to straighten it gradually.

    • @cynthiaclaffey1522
      @cynthiaclaffey1522 Před 2 lety +1

I wouldn’t walk on it until it heals, but if you can’t straighten it then you don’t want to force it straight (like one surgeon told me he’d give me some shot in the k we to help it straighten out so I could walk peg leg, that wouldn’t been the worst thing to do especially since I hadn’t had an MRI & he didn’t k ie I had a bucket handle tear!)
You don’t need a hinged brace when it’s healing since you shouldn’t be walking on it. It’s worth staying off it to let it heal, so the recovery is shorter.

@@cynthiaclaffey1522 Sounds great! Thank you so much! When you say don’t walk on it, did you use crutches or you just limited walking as in walked only when mandatory in the house ?
I agree about knee brace, most don’t even use it & I don’t know how effective it is. Also, most don’t even go to the doctor or the doctor doesn’t do imaging for it (they diagnose on physical exam). I guess it just takes time / rest but muscle wasting is annoying lol.

    1. if i got click/pain/limp but no swelling might it be less severe and easier to heel?
    2. with low/distal MCL clicking, what is the ligament catching on to cause the click?
got 10+ year old MCL at times with pain/click/limp, specially when straight and a knee that gives-out descending stairs, other times with no pain/click/limp/give-out. i can walk one minute with pain, clicks and half stride limp; the next with no pain, clicks or limp.
i know i'm reinjuring MCL with my routine mobility: by ill-pivoting to get in/out of car, odd change of direction with leg cross-over, and have quasi pigeon-toe stride. i now get pain when leg straight so needing a pillow under knee to sleep.
some are neurological/elderly/overweight ex parkinsons which can make dressing and pulling on/off spandex sleeves difficult so we may need the wrap-around (no sleeve) type knee brace that we can strap-on over pants. let's hope this can provide 'enough' protection for the ligament.

    Hi Doctor David,
I am a 69 year old runner. I have been running for about 50 years. I coach Track and Cross Country in High School. I tell my runners to taper back if they have any pain while running. However I do not always take my own advice. July 1st 2018, I decided to up my regime by running 5 miles a day for 2 months. This would give me 300 miles by Sept 1. However on July 6th I felt discomfort in my inner knee. I ran on the 7th and felt pain, so I walked back home. Tried the 8th {slow learner} and the pain was worse. Today it is Dec. 12, and although the pain is less it is still there. I have been walking on it, but NO running. I have not yet seen an orthopedic but am getting frustrated. My only doctor is Doctor Google, and he has done a good job with other ailments, as there is a ton of info out there. There was no swelling the day it occurred just pain in the area of the menicus/mcl. I can walk a mile or 2 without much problem, however sitting for long periods stiffens it up. I will let you know how things go.
    Mike

I recently had another knee dislocation my patella popped over to the left side while pivoting I sat down on a chair thinking it’d go back in on its own when it didn’t I tried to move it over with my fingers that didn’t work so I ended up having to hit it back in. I hit it 4 or 5 times. I took Ibuprofen and iced it. It continued to swell. The next morning I went in for X-rays and need to go back on Friday for a MRI they think I may have torn my MCL when it dislocated and May need to do surgery it makes me really nervous because they already scheduled for me to meet with a orthopedic surgeon the following Tuesday when I get my MRI results to discuss further treatment. I have a feeling there going to push me to do surgery since it’s a reaccuring problem and something I’ve already been through pt for.
