    Had this surgery exactly 5 months ago today. Tough recovery but the best thing I ever did. Pain free now and got my life back, gardening, long walks and whatever I choose to do.

      Did you have any disc deterioration . I have 3 discs gone and the vertebrae have slipped way out of line. Been this way for many years and much pain.

      hello can you tell what is the surgery you had that you are able to do everything you mention.?
      because I had a fusion on my black remove the disk and have something on between the two vertebral then screws were put on there to hold everything together.
      the main pain is gone but still not pain free.
      I can't walk stand or seat for two long.
      and I can't bend down or twist like before my surgery.
      it will be interesting to know what was done in your back. please.

    I’m having this surgery done in a few weeks by a neuro surgeon. I’m neither scared nor nervous. My father will be with me during my surgery. He has my hand . He will comfort me while my surgery is going on. I have faith in him as he directs my surgery.

    That's exactly how they placed my screws with the x-ray in real time. They actually used to computer in conjunction with the monitor screen to set the screws.

    I had exactly the same diagnosis, and decompression surgery in the UK in 2012. Unfortunately, because I had had to wait 22 months for it with the stenosis badly pinching the leg nerve all that time, the sciatic nerve in the leg is dead now. I haver had great mobility since, though the severe pain was relieved. I have no screws, and the surgery was not this latest way.

    I had a similar surgery. and I stayed 3 days in the hospital and 14 days in the rehab.

    I too, have the same condition. And certainly this surgery is in my future. It was terrific to see Dr Kim’s technique for correcting spondylolisthesis. I can just add it to my recent hip replacement. Modern orthopedic surgery is amazing!

      I recently and continue to deal with the debilitating pain of Sciatica. Most days I can early stand to prepare my meals. If I sit I sit on ice packs, I have given up driving and depend on friends to help me with grocery shopping. I have had a bone density test (good), an, MRI (not so good). I have Lumbar Spinal Stenosis, Lumbar Spondylosthesis

      Penny I also Degerative disease of spine. I was told by my doctor he would not recommend surgery. I could try pain shots. Several years ago I went through pain management for arthritis around S1, no help with the injections. While I am 79 yrs old I do not feel old. I have been active until this recent diagnosisThank goodness for my friends who have helped me keep a positive attitude and kept me laughing. I thank you for your thorough explanations of spinal issues and wonder if you have any suggestions for my diagnosis.

    Why did you not do an ALIF on this patient? I have had two surgeons advise that on me, and it seems too risky for a 72-year-old like me!

    I had a scan 3 weeks ago and it doesnt make for good reading from what I can make out . Severe degeneration from T11 - S1 with stenosis at L3-4 and 5 bulging disk at T10- 11 ,endplates (modic type II),crowding of the cauda equina , oedematous changes (Modic type I) just how good or bad is this as I have yet to see the Dr . I have a LOT of back pain and numbness in both legs . BTW I am in the UK and been in construction all my working life.

    This was an excellent video. I'm slowly getting used to the idea of having screws put into my back. I'm more concerned with how long the pain will last as the body gets used to the alien material. Thank you Dr. Kim. I only hope my doctor uses minimally invasive techniques.

    I have stand alone BAK cages , at l5s1 , a few years ago l4 completely blew out a massive herniated disk, it needs to be fixed but nobody will do it for some reason

    my doc reccomended L2 toS1 fusion. i have a hern. disc at L5. i asked him just to do lamectomy 0n L4 L5. not looking for no pain, just dont want my right leg to get any weaker. now i have severe pain when i bend forward! i hope the surgery works!

    I come to find out that I made a huge mistake choosing one of these Medicare advantage plans instead of just sticking with regular Medicare. They keep denying things and paying fractions of the cost of like an MRI for instance that was around $1,300 and they only paid $288 of it whereas normal Medicare would have simply paid it without question. I believe I am trapped in it for a year at least. If I had just gone with regular Medicare I'd probably be out in San Diego right now waiting on surgery. Instead I'm stuck here in Columbus Ohio with surgeons that don't want to talk to me more than 5 minutes and insurance that is looking for every way they can to deny coverage. No one explains any of this to me and I only found out through watching yet another CZcams video!

      Medicare may not be exactly the white knight you imagine, although I find it more attractive than the managed plans. Medicare pays only 80 percent of what it approves, so to avoid paying something out of pocket, you need a good supplementary insurance policy to pick up the 20 percent. I currently pay about 290 bucks a month for my supplement, and it has a 240-dollar annual deductible. Medicare itself in 2023 will have a monthly premium of 164.90 dollars and an annual deductible of 226 dollars. But most of us couldn't afford surgeries, tests, and doctor visits without some help of this sort.

    I’m a 76 year old retired physical therapist. I have all of the above and throw a little scoliosis. No ruptured disks however. This video gives me hope. I live near Knoxville Tn. Any guidance would be appreciated.

      Are your legs weak, tingly or numb? I’ve had 5 spine surgeries including fusion’s, adjacent surgery on l3 this March. My L5-S1, L4 fusion was great for about 5 years but it caused my L3 to collapse… long story but if you’re not experiencing the above and can manage pain I’d hold off.

      @@adamgodwin3130 I have grade 2-3 Spondylolisthesis At L5. The disc is very thin and the three discs above it are bulging. Also cervical stenosis and arthritis throughout the length of the spine, but most of the pain is caused from L5.

      @@adamgodwin3130 And I appreciate that you cared enough to ask. Yes I can get around on my own, its just I run into a lot of pain after being up and around doing even simple things around the house. For example fixing dinner takes a long while, because I have to stop and take "breaks" until things settle down enough to continue. There are many times I shudder at waking up in my bed, although it feels amazing at first. When I wake and stand, it is like everything is stacking back together, and at times the pain is so severe, I can't help but holler. I'm over weight, so that doesn't help, and I'm sure they will want me to lose more. I have lost around 50-60 lbs so far, which isn't easy with such a sedentary lifestyle lol. I'm hoping after another 50 lbs, they will consider the procedure. As a bonus, I'm hoping to reverse the Type 2 diabetes at that point also.

      I had a surgery, different than this one. I had the surgery 8 months ago. It is process to get your body back to normal. I have been to PT several times and now messages. After being bent over for two years I realized my muscles needed to get back into shape. My body needed to realign. No pain from surgery. But pain from muscles. Also waking funny. Again, muscles out of alignment

    I’m had 7 Back surgeries at L3-L4, L4-5. I have rods and screws in my back and never had incisions like that. I wish I never had the first one. Recently I was in a car accident and now I have 11 herniated disc’s from my neck to L2-L3. Every Doctor I see says I have to much going on and won’t even treat me. Now I’m about to have a pain pump inserted to treat pain.

    Is it rational to expect 7mm of slippage (spondylisthesis) to be realigned during a mis TLIF procedure?
    Or would full realignment not be likely?

      I would think so. I saw another youtube about a weight lifter that had this done and he gained 2" in height afterwards. Keep watching the videos.

    Wow Doctor you guys are next to God. I am from India. I had TLIF surgery on October 12, 2021 in Artemis Hospital, Gurgaon. I was very active before that, gyming, cycling running, walking. Please can you let me know, when can I start cycling again. I have a roadbike. Thanking you.

      @@dwarkaprasad1217 Dwarka ji. It is better to get yourself opened by a Neuro Surgeon. As neuron system is more complex and vulnerable. I myself faced the same situation. Consulted both Dr. Himanshu, Orthopaedic and Dr. Rajan, neuro surgeon. and after lot of thinking zeroed down to the latter. As spine is very complex organ of our body, requires lot of expertise.

    Sir my reports says...
    1.Disc dessication seen involving l4 l5 and l5 S1 level.
    2. L4 l5 showz deffuse disc bulge With annual tear intending the thecal sac not comressing the nerve root.residual canal diameter measure 14.5 mm
    3. L5 S1 shows central disc protrusion intending the thecal sac not comressing the nerve root.residual canal diameter measure 14.5 mm
    ..
    My symptoms are
    No pain.
    Heavy stiffness
    Sitting issue
    Cant stand for more time at the same position.
    Sometimes feelings of numbness at legs area also
    And moreover my issue is not fixed .,means it's moves from left to right or from right to left.
    Psl dr do reply ...thax

    I have just the same L4-5 grade 1-2 spondylolisthesis (about 12 mm) more then 20 years... I am ex gymnast. And I am still fine and active, but there is natural fusion happen between L4-L5 during this years, no disk anymore between :( I think there is no movement there. I would like to talk to so professional neurosurgeon like you, may be I any other choice?

    My surgery is February 22, 2023. They will put several screws in. I'm somewhat concerned about the pain and how long it will take before I can bend a little. I was told I should not move side to side. And going to the bathroom requires a side to side movement. I was told that I would be shown how to do something else.

      Hope all went well i have gad 4 spine surgeries one without screws in 2015 went well but something happened year later with pain but when did new nri found i had kidney tumor cancer so had that fixed but i did something and the back pain went away for 6 years but in December 2020 things got bad had l4/l5 fusion but then part of screw popped out didn't believe me but its on my dresser in bedroom pain came back cause they didn't fix my knee tears. Had same back fusion surgery end of aug 2021. They didn't make sure the l5 nerve root was still decompressed so my left foot feels like a brick and have claw toes now. Had knee replacement December 3021 with hardware thats on recall then feb 2022 my back hurt so bad i couldn't take it did nri had huge herniated disc and vertebrae underneath fusion collapsed surgeon was going to do third surgery but anesthesia department wouldn't do so got another surgeon glad i did he said it was very challenging took twice as long since disc was so stuck to the nerve root so now have screws from l4 to s1 and a cage and pelvic screws cause bones soft from long term steroid use. My bad it much better but my foot isn't plus have a hamstring and labrum tear in that side. Think have nerve compression in performis muscle. But need surgeon to fix those was off work forever. But went back dec 5th off last year. Lucky had good short and long term disability from work but social security approved me in 2 and half months without a lawyer. Which i still have benefits since im on a trial work program. I ended up getting sepsis in my knee after replacement and blood clots in both legs and lungs had to have cath procedure to get clots out of lungs. Lucky ok to have my last back surgery sept 1st of 2022. My dad had fusion in 70's he did swimming after three times a week so if you can do that with covid i couldn't he could golf bowl hike down Grand canyon for three days. Im able to do more now but couldn't for two years. Couldn't take care of my dogs kennel bill was astronomical

      @@chrism880 So sorry to hear you are having so much trouble. Do you think you need to get a different doctor? I just had my surgery 5 days ago. It was so excruciating. I have my husband to take care of me which he is doing well. I think you need to find some additional help.

      @@lynnphillipi3536 glad your husband is helping you mine did when he came home from work. I had rehab after 1st back surgery to learn how to do everything and after this last one cause had new insurance and they didn't know i already had it but needed the extra week it was in a hospital not a nursing home so care and food were better. I did have a different surgeon this last back surgery he did a wonderful job think if i kept other one i would be disabled for life. But i need other things fixed that a spine surgeon cant do. Im working on it my family dr says i had more surgeries then all his patients combined ha ha probably have. Think its been 85 now. Most orthopedic related and i dont even do sports but im worse then a whole teams injuries ha ha. I tell my boss i should be making 35 million a year like a professional athlete he said he will get right on it. Wishing you a speedy recovery and to a good normal life.

      Only the first few days were excruciating. I'm in week 11 and about ready to give up the back brace, though I have not been wearing it for a week. If I bend over even a little, one of the side muscles hurt the rest of the day so I try not to bend over. I have to sit down to relieve it. I take Tylenol which helps, but only periodically. I get signed off May 18. I am happy with my surgery. I went to Dr. Andrew Manista at Olympia Orthopedic in Washington State. I highly recommend him. Both of his PAs are great too.

    Not criticizing because I’m not a doctor, just a lowly computer technician, but I’m very surprised to see how quickly and… incautiously you move when playing with somebody’s spine… When working in a PC I always move very slowly and carefully so I don’t slip or bump something.

    Super informative, thanks. Is the assisting physician working on the opposing side of the spine a fellow orthopedic spine surgeon, a resident, PA? To what extent will the my physician actually be performing the surgery? To what extent will it likely be a resident? Please...thanks for any information.

    This type of surgery is dangerous, patients can be get permanent disabled and this problem can be raise again after some time.

    NOPE! No surgery for me! One slip during the procedure and you could be paralyzed. Chiropractor cured my sciatic issue and have been pain free for 20 years.

    I had this surgery done Five years ago and it failed. Same pain I went in with I came out with. L4-L5. It was done by an orthopedic surgeon. The pain got even worse …blah blah blah. Making a long story short, could barely stand up. Went back in a second time. He said that the screws came loosened. That was a year and a half, actually almost two years ago. I can barely walk, the pain’s been unbelievable . I wish to God I never had this done. I’m so broken. Can’t walk well at all, can sit and lay fine but walk or stand is nope! It has now caused other things to happen because of my unableness to really move. I’m pathetic. Just pathetic. I see these surgeries and wonder why and how mine went so wrong. I actually have taken a walk through hell. I’m housebound in the end. 😢

    It would be interesting for you to talk about Dr.christopher Dunch or Dr. Death who killed 2 people and by slicing their vertebral artery this Dr maimed his patients I'm curious if you've ever heard from the case from Dallas Texas he was a neurosurgeon now he is in prison!

    Doctors in our town would take one look at those x-rays and say, there's nothing wrong, we'll just send you to pain management PT. At least that's what they have told me and my x-rays look 3 times as bad as those. Thanks for showing your techniques!!

    I wish you could sprinkle your magic fairy dust on me !!! 45 years, six lower L4L5S1 surgeries.... failed ones, Stimulator # 3 and 35 injections........and I'm still a train wreck :-( have been eating pain meds like M&M's for all 45 yrs......and all they are worried about that I'm addicted to the Oxy's !!! Dahhhhhh of course I am !!! They keep treating the symptoms not fixing the root cause........
