  • Dr. Midhun Mohan, faculty of Emergency Medicine at Calicut Medical College takes us through this simulation exercise to help us optimize the oxygenation strategies available at our disposal while managing a COVID-19 case. Beginning with the nasal cannula, which requires minimal expertise, this simulation takes us through NIV and intubation and the precautions to be adhered to while doing these lifesaving measures. These experts were the frontline warriors in the fight against Nipah Virus in 2018

    Great video. I think even senior doctors need to be sat down and shown this video. Been working on the front line but unfortunately tested positive and I’ve been spending my isolation period solving marrow Q banks. Really appreciate how up to date Marrow is.

    Once u have put the pt. on nasal cannula or mask, reassess after 15-20 mins as it takes that much time for O2 level to show on pulse oxymeter. Same goes for if u change the O2 level delivered..

    For maintaince post intubation,is muscle relaxant compulsory or only given if there is ventilator -patient dissynchrony???

      Covid patients are seen to have very high respiratory drive and there is more chance of patient ventilator asynchrony requiring high doses of sedatives and maybe paralytics. Not compulsory. Most covid patients requiring intubation may benefit from proning. Paralysis is required for that as well.

    Does using high nasal flow cannula really help critical covid patient? My grandfather had been in the hospital for 9 days, and on his 9th day his doctor advised to put on HNFC, the next day he became unconscious and eventually died.

    1 doubt sir: is hudson mask placed over the surgical mask? Y? Does it not allow germs to enter the patient's mouth?

      It is placed over mask to prevent aerosolozation ( protection for healthcare providers) and for ur next question , keeping all the viral particles at one place is better than spreading all over (logically speaking)

      Initially target for a saturation of 94%. But try to reduce the FiO2 as the respiratory distress decreases. Over a matter of a few days many patients are seen to be doing alright with a saturation of even 88%.

      Management depends on the severity of breathlessness. Oxygenation and ventilation strategies are only supportive measures till the pathology is resolved. Patients can have shortness of breath because of the pneumonia even when the saturation has not dropped. Such patients may be maintaining their saturation by increased work of breathing.
