What I love so much about this episode is that it doesn't hit you over the head with the fact that Ben was just a figment of Dr. Cox' imagination in these scenes. So many movies or TV shows would have flashbacks of all the scenes where he fades away to really hammer it home, but Scrubs trustes the audience to piece it all together on their own. One of the best shows that ever aired.
This bit was especially good/deceptive with the look Elliot gives him, which obviously meant "are you somehow making fun of me behind my back??" and just as obviously actually meant "what is Dr. Cox laughing at behind my back??"

Each person grieves differently for each person they know who passes away. In Dr. Cox's case, he didn't grieve for his abusive parents or for anyone he knows and genuinely hates, except maybe for the type of people they could've been. When Ben passed away, you see a difference. This was his best friend and brother in all but blood. He felt that if he had been there when Ben went into cardiac arrest, he could've saved him. That's why he took all of JD's patients away from him. Dr. Cox wanted someone to blame for Ben's passing. By treating JD as though he was the main reason, Dr. Cox ran himself ragged keeping those patients alive in his mind. JD and later his subconscious in the form of Ben told Dr. Cox that JD was already swamped and couldn't be everywhere all at once. It's hard work treating patients already under your care and JD did the best he could do. When Ben had his cardiac arrest, JD was notified immediately and he reacted accordingly. Despite the fun Ben had messing around with JD, Ben knew that he would do his best to take care of him should something happen. Dr. Cox needed this lesson that even if he were there, Ben would still have died. Patients die even when you are there as a doctor. Even when you have the best of intentions, which is another lesson Dr. Cox needed when he didn't what to learn what Jill died of before having three patients receive transplants from her. That was a much-needed lesson.

I think it was set up so that we would confused about who actually died while Cox was away. I originally thought that the old guy in the wheelchair had died, until the end when Cox asked Ben why he wasn't taking pictures, and remembered what Ben has said he would take picture until he was dead. Very sad episode.
it's a defense mechanism. there's history that explains a bit more about how they ended up like this, but they have a hard time letting people get emotionally close to them so they put up a crazy fight all the time to keep people distant. also explains why Jordan and Perry get along so well. they understand intuitive what theyre doing, even if they cant explicitly acknowledge it to one another without some kind of outside force.
also as a side note this was something present as a throughline in House, and one of the elements they borrowed to build Cox. curiously, given House is a reimagining of Sherlock Holmes, this behavioral habit is one we see there, too, save in House and Holmes case it's a result of addiction as a coping method for disappointment. Cox doesnt seem to have the same substance abuse problem as House or Holmes did at first glance, but his struggles with alcoholism, and his behavior, suggest that perhaps the substance he's addicted is more emotional or intellectual in composition.
@awinxa Pretty much. I think she just has intimacy issues, so she acts mean out of fear. It's funny though, because she can't easily be nice to Perry or her colleagues yet she was kind to the drug addict that tricked her.
I have _two_ Brothers-in-Law...It is my eternal disappointment that neither of them do, or ever will like _me_ to the same degree that Perry likes Ben... I wonder if when I'm dead, one of them will hallucinate spending the day with _me,_ before realizing that I'm gone...

Cox initially blamed JD for Ben's death believing that JD could have and should have done more when in reality Cox was really angry at himself; for not being there when Ben went into the cardiac arrest. The whole thing is Cox feeling guilty at himself until the very end at Ben's funeral when Ben tells him you have to forgive yourself for what happen also for the birthday party? They cancelled it due to Ben's death.

See this is why Brendon Fraiser works best when he isn't a leading man, because the guy is a charisma black hole, and only works best when he does supporting roles.
This scene is a lot more sad than funny when you realize that Ben is not actually there.
By the time you figure that out, its a bit too serious to find anything funny about it.
