  • Go inside molecular testing and see how ID NOW works.
Abbott has received emergency use authorization (EUA) from the U.S. Food and Drug Administration (FDA) for the fastest available molecular point-of-care test for the detection of novel coronavirus (COVID-19), delivering positive results in as little as five minutes and negative results in 13 minutes.

Thank you abbott, I have invested my Roth IRA in your company not only because I know White to be an upstanding patriot, but because you've been a household name in the OR for the past 20 some years of my medical training. Well done. You deserve every ounce of recognition for mobilizing this diagnostic marvel. Please make enough so Amazon, UPS, USPS etc can have one handy and routinely test all of their employees for everyone's safety. If one of these units requires maintenance in an urgent pandemic setting, what kind of on site certified technicians are available to repair and troubleshoot machines that might not be performing to spec? Is Abbott hiring mobile technicians to repair these units on site in the event of failure? Is there a control sample of COVID19 used to calibrate the machines periodically to make sure they are accurate--seeing as a false negative could yield disastrous results? Is there an Abbott ID NOW "Geek Squad"?

This is a molecular test. We need a Serology test as well ASAP. That way we can detect all the people who had the virus previously. Perhaps they had no symptoms and therefore were never tested. A molecular test is great but only tells us who has an active infection.

I have several questions on how to make it work (ID Now already available)
    1) How long are swabs good for before they can no longer be analyzed with the ID Now?
    2) Can the swabs be placed back inside the foil pouch for transport over a short distance?
    3) Can the swabs be used in distant workplaces i.e. offshore and flown back to the laboratory the same day for testing with the ID Now?
    4) If we want to test some individuals working offshore, will it work to collect their samples using the Copan eNAT swabs in UTM and then fly them back onshore for processing?
    5) Will the transport medium help preserve the samples?
    6) Does it make the most sense to simply place the ID Now in the same room with clients (workers from extractive industry) and only use it to process samples of people who are able to come in in-person? If yes, the implication may be that they decide to locate this analyzer offshore, if that is where it will be used the most.

FDA FAQ page is the only place that deems this CLIA waived however, FDA does not CLIA categorize tests authorized under EUA. Instead the settings in which an EUA authorized test may be used are described in the Letter of Authorization.
The letter does not clarify it as being a waived test it reads authorized labs that are moderate/ high complexity and for patient settings outside a clinical laboratory. These patient settings do not default to CLIA waived.

So the FAQ page has no legitimacy in my opinion that it is categorized CLIA waived, the laboratory community needs a formal statement and decision from CMS. Not just an email from them to read the FAQ FDA page.
Also, we need to look at why this test is so much faster. The biggest difference is how the RNA is pulled out of the protein coat. It’s kind of like cracking an egg, you need to break open the protein, but without damaging the RNA inside. The Abbott point-of-care test pulls the RNA differently from the gold standard Roche rt-PCR, in the Abbott it occurs all inside the instrument. It’s way faster, but it is not entirely clear whether it is better at separating out the RNA without damaging it. This causes LOD issues. There is very little publicly available data on what is called “limit of detection” LOD- that’s how many copies of the virus need to be in the sample before it will give a positive result. The lower the limit of detection, the earlier in the infection process the virus can be detected. This is why this new test will be less sensitive. The Abbott instrument per package insert has an LOD of 125 genome equivalents per ml. A gold standard RT-PCR has an LOD of 3.1 GE/ml. This is why the POC instrument will have many false negatives, it’s the ability to detect as low as 3.1 viral particles vs a required 125 viral particles for the Abbott.

It uses PCR, polymerase chain reaction. It’s a biochemical method used to amplify unique genes or genetic markers that are unique to the virus, by copying it over and over by biochemical means. If the genetic marker is there and the primer binds to start the reaction, you will see a signal. If it’s not there, no reaction. Not everyone takes biochem 400 level college courses to understand how and why it works, but it works. PCR is a process used to amplify genetic material for rapid flu test, parental dna test, genetic disease, etc.
