May be, your pay is more than 15000, so that's why your employer not remitted your EPS,, so you are supposed to get your pf account in the EPF withdrawn ( form19 )..
In some recent cases epfo office is not accepting 10c claim form online more than 1 time so if in your case it is not taking online claim form 10c, you need to file it offline..

In case of trust you have to contact the HR of your company. You can't apply it from government epfo website.

If the claim got rejected you need to re file the claim, you'll not get any amount in your bank account until your claim got settled.
