You will understand that if you listen to "LAMBIYAAN SI JUDAIYAAN" by Arijit Singh. Actually he isn't out of rhythm coz he wasn't singing a song there, that was poetic lines which gives emotions to that perticular scene in the Movie, only those who knows the language can get it,
He has not gone out off tune my dear friend. It happens in Indian music especially, Hindustani music to add some lines out of the main tune in the song. Some poetic lines added. But that only adds to it's beauty. Infact only expert artists, trained singers especially classical ones can do it. There are innumerable such examples in Indian songs where such lines are added to give depth to the song. Its part of poetry or Ghazals as it is called in Hindustani music. Some expert musicians can throw more light to it. This is how I can put it as far as my understanding of Indian music is concerned.

5:18 begins the main classical part of this song which is the toughest part of this song (this is basically about lyrics and improvisations)
the improvisations he did are really beautiful like at 5:21 "raaaaasta"(usually pronounced as rastaa)
and the notes are going up and up there are so many improvisations in this part i can't even write all
That high note at 5:57 is extremely high it's C5 in pure chest voice and its really difficult
