Just one question. If at all ever required, how difficult is it to remove the embolizing agent (coil or glue) to reopen the path of the blocked vein?

I don't visit Kolkata.
It's nearly impossible to re open the path. The whole vein can be removed.

First of all, thank you for your videos. Does embolization block my only main vein above the groin or do you only select and process varicose veins with branches near the testicles?
It blocks the veins starting from groin - at level of inguinal ligament. And then we block the vein upwards till L3 vertebra level so that collaterals do not form and cause recurrence.

Hello doctor you addressed almost everything about varicocele in simplest way possible however I still have one question is embolisation as effective as micro surgery when it comes to bilateral varicocele?
Also how often can embolisation be repeated in the event of recurrence?
1) in personal experience- yes. We have never faced technical or practical clinical issues with bilateral varicocele. Planning to start a prospective project soon to publish high powered data. 2) Can be repeated as often.

Depends on how chronic or irreversible the atrophy is.
Hello doctor...thanks for detailed information.I had surgery for curing vericocele...i had high swelling in testies for about two weeks...is that normal? and after how much time I could get good parameters in my semen analysis.
That's normal most cases. But occasionally can have other causes. Always great to visit your operating surgeon in case of such issues. We usually see dramatic semen changes after 12 weeks of surgery or embolization.

Does embolisation block only internal spermatic vein? What about external spermatic and gubernacular vein? I have seen recurrences happening due to external veins. Is surgery the best option in that case?
When a primary surgery is done for varicocele, does a urologist or uro surgeon ligate gubernacular veins?
@@DrGauravGangwani I don't know what a regular urologist would do but in the original micro surgery technique - developed by Marc Goldstein - ligating the gubernacular vein is essential for least recurrence rates.
So, u mean - extrusion of Testis outside and lighting gunernacular veins in every primary varicocele case? Besides inguinal or subinguinal ligation?

Antegrade approach can be used for diagnosis of reflux in these vessels and even embolization of both of them. Whether it's done routinely or practically or not is another thing. But possible. You can go through literature on same. Antegrade venography by direct pampiniform plexus puncture. 1/3

Depends if u r talking about Sarteschi grading. Usually grade 5 means that testis has started shrinking. So it may mean start of irreversible damage. And yes surgery or Embolization is usually contemplated at this stage. But always after urologist or Interventional Radiologist opinion.

So those veins which are coiled are useless? and what if it reoccurs and we block others so it may end up blocking every vein? Curiosity arises when we think of metal. Doesn't it shrink and disappear on its own for years into that particular vein?
We don't block all. We block 1 or maximum it's bypass collaterals. The metal never causes harm. The veins scar and fibrose. And keep metal stuck there with no adverse effect
And yes the veins r in 1 sense useless. As they r causing reverse blood flow. And more harm than benefit.
@@DrGauravGangwani alright sir, thank you sir for clearing my doubts. You didn't mention of reoccurrence and blocking other veins. Is reoccurrence due to the failure of the previous coiling of the veins or functional veins at that moment become a victim of varicocele?
Can be because of both.

Micro surgical best or embolization?
It's for patient to choose. Each has its pros and cons. My personal opinion - embolization has a few advantages- Scarless, no groin infection, incision far away from groin, local anesthesia, can see collaterals in venography and few more.

Yes. That's pretty normal for most cases. But always discuss with the treating interventional radiologist.

U should. With adequate scrotal support. Icing, yoga and inversion.

From some bunch of days i started noticing that my balls are getting saggy,i feel higher temperature in my testicular area and moderate pain while i tap my balls slowly from downward side to check ...i really feel some sort of worms like swollen veins ..I'm also a biology student ..and i can definitely say that it is varicocele only nothing else and it's starting, it's been from like 2 months,i thought it might go naturally cuz of my healthy life style I'm 18 years old and follow very balanced and healthy diet.
I eat around 5 whole eggs a day and some egg whites and even some supplements like omega-3 fish oil, daily multivitamin,whey protein,pre-workout ( which has Citruline malate,Beta alanine,creatine monohydrate,L-Arginine,Caffeine and Taurine in major)... Please help me out by giving your valuable advice sir..I would feel blessed and glad to have your advice....
and sir yeah i forgot to say it is kinda very impossible for me to stop lifting and to do some intense bodyweight training ...What would be the precautions should i take in supplemental level btw i don't take any of these supplements for longer time..i give break for each and every supplement even for multivitamins.. @@DrGauravGangwani Doctor, I had this Vericocele problem many years ago but it went just unnoticed and I have been itching the scrotum for many years and I came to know about the problem and I have both the grade 3 on left testicle and some spider veins on left testicle. But on right testicle, everything is normal. Will surgery cause any problem related to blood circulation? Please let me know as I am very much depressed about the blood circulation post surgery
