This is often the difference between competent actors and good actors who insert something extra into a scene. It’s also the difference between competent directors and good director who know when to keep the moment rather than bin it!
I don't know if I really believe these are all genuinely unscripted. Like the _True Lies_ one at 5:36 -- I think that reaction from Arnold Schwarzenegger completely fits with something a writer would write for his character in that movie. He's pretending to be a character within the movie for her -- but he's really her husband -- so it's funny that this character would "break character" for instant. I think somebody watching this didn't get the joke and wrongly assumed it was unscripted. The fact that Jamie Lee Curtis didn't break character and pops up so quickly supports that his was for intentional comedic effect. Also, notice the camera changes focus to him for that instant. If that wasn't supposed to happen and he wasn't supposed to react there would be nothing for us to see with him -- and the focus would have stayed on her (or rather where she was supposed to be).
I agree with what you're saying here, in that it was planned for the final shoot, but it could still be possible that on the first take, or in rehearsal is when it first happened by accident. Then it's up to the director if he wants to go with that idea instead. So while it may have been pre-planned, it technically isn't scripted.
