They can't because they are brain washed to believe that the Kims are god-like humans and that his father is always hearing his people and their thoughts. Even if they thought about that, they'll get major anxieties that he'll go after them.
He has a very nice tv and many other things and yet he limits his people to watch only shows from north korea, they are also not allowed to listen to music from outside and the punishment is severe for that simple things. Insteas of revolting, people are trying to cross the border to south korea, the ones that failed are killed or worst they are tortured. The ones that escaped tell their stories and experiences about that place and you can watch it here on CZcams. It really is a terrible place to live.

People are short there because not enough food. It's most likely Kim's way of controlling them by making sure they stay hungry. Hungry people don't have energy to rebell and worry only about food. If they would have plenty of food they would take him down as they would be able to fight.
