As someone getting a master's degree in translation (English is not my first language), it's true what you've said both about two translators not being common practice and about there not being a hard and fast rule for how it's done. Without getting too much into translation theory, what sometimes happens is someone translates a text from the language it was written in to a different language and then someone else translates it from that language to a third one. Someone could also have done a second pass to make it sound more natural even though I don't think a secondary translator would be hired just for that unless the text was really challenging. I've also personally never heard of two translators both doing their translation of a text and then mashing them together, primarily because it sounds like double the work to reconcile two completely different iterations of a text, but theoretically it could be done.
As a translator, there is no straight answer to your question hahahah. A professional translator should be able to translate the informal register and the cultural aspects as well, plus research and dictionaries are our best friends. However, sometimes what you explain happens, especially with lesser known languages: the person who speaks the language is not a professional linguist, so an editor may be needed. In other cases, people just work well together and decide to collaborate, and only they would be able to tell you who did what section (and probably both worked on the editing process of the whole project, to maintain consistency).

Regarding translations: a friend of mine is a translator. Sometimes when a book is translated into one language, say Portuguese to Spanish, and then into another, Spanish to English in this example, they will credit both. Also translators don’t always work alone, especially if it’s not their native language to ensure there’s not a lot of muddling of the text. And translation is different from transLITERATION which is where it’s word for word. Translation is more conveying the concepts and isn’t always word for word. This is why there are so many versions of the Bible.
