Not sure if you're still dealing with this but that is where you're exhaust and exhaust cooling water comes out of the lower bellows which has slits in the bottom of it. Exhaust does not exit through your prop like an outboard. The flow looks very sufficient so if you have a open cooling system (No heat exchanger-no coolant ) system I would suspect a stuck or failed thermostat
i have a 1989 cobra 5.7 , a bag covered the intake while in the lake and system overheated , from that time on it ran hot until a buddy told me that the thermostat will not open at the right temp after that happens , changed it and now runs at the right temp all the time.. you can temper the thermostat when overheating. you can take the thermostat out and see if it overheats then go from there.
The water pickup tube has slipped off. it is difficult to reattach but turn the outdrive slightly to the right and have it midway up and you will see the hose that is off.
Honestly I don't remember. That was 6 years ago. It wasn't a major fix. I know I replaced the impeller but nothing related to the inside of the stern drive.

My 88 OMC 3.0 normally runs around 175. The water discharge looks fine.

the water comes out mostly at the transom mount and will only come out the prop at high speeds

Seems like a leak. Also turn the hose up more
Ur inpeller went out for sher I got 215 sundowner and am going throw the same thing u got to split the out dive its about 6 bolts I believe
