It was basically planted as protection. It ensured that he would flee, had he run into a fight that he couldn't win.
It's also why Silva was so adamant that Killua would come back home. Back when he made Killua promise to never betray his friends, he anticipated that the needle would come into play eventually, and that Killua would break that promise; thus abandoning his friends in time of need.

The reason they had killuas needle in his head was to keep him alive. If he was going to be out on hunter missions they want to at least make sure he doesn't get himself killed against a stronger opponent
