Why Train Internal Rotation of the Left Hip? (So you can diaphragmatically BREATHE).

  • Internal rotation of the left hip via the left adductor and anterior glute medius is where Postural Restoration training programs start. The reason is that a body that can't internally rotate at the left hip can't properly pressurize the pelvic floor and thorax for diaphragmatic breathing.
    So we don't train internal rotation only to establish better joint range of motion. Too much joint range of motion can be detrimental because it's leads to instability.
    We train hip internal rotation, mostly on the left, to re-establish proper pelvis stability off which we can breathe without compensation. And once we can breathe without compensation we have a better chance of moving without compensation. That's what you need internal rotation for.
    The left hip musculature does have to be trained in an internal rotation position, but that doesn't necessitate weights. In fact, weights probably won't help. PRI techniques generally use only the weight of your own leg to train the movement.

    7:58 - 9:30 I've been thinking about this for a couple years now. I had no idea what to call it, but my theory was that the human body operates on X's. For example, the left hip is connect to the right shoulder. The right knee is connected to the left elbow. Your left big toe is connected to your right thumb. It makes no sense when you first hear that. It sounds imaginary, but I have found it to be true.
    I was in a car crash 10 years ago and it destroyed my entire upper left thoracic chain. One vertebrae was severely misplaced. I could walk and talk and do everything normal, but years of letting that injury settle into my body took it's toll. I couldn't feel my toes. I could never get a deep breathe, always just out of reach. This lead to massive anxiety and decreased quality of life. I went to chiropractors, tried resistance training and yoga, tried losing muscle and running like a dog (which arguably had the most benefit), but nothing "fixed" the problem.
    I recently started sleeping on the floor, and that has allowed my spine to decompress and self-adjust. Just a yoga mat, a pillow and a blanket. Your back needs something solid to support you. Walking with weight (20Lbs in both hands; I'm 190lbs) for a mile every couple days while maintaining proper form and rhythm has also been EXTREMELY beneficial.
    Learning to box breathe, while walking in rhythm (nose only, maintaining molar contact, 3 steps inhale, 3 steps hold, 3 steps exhale, 2 steps hold), WHILE maintaining an isometric hold in my torso, like Neal is explaining in the timestamp provided, is so effective it feels like magic. My hands are typically cold as ice, and when I walk like this my whole body is radiating heat after for hours.

    I'm just learning about the scalene muscle and the stuff you're talking about.. it's relevant to me because in my teens I had two events that affected me physiologically.
    First was playing tackle football at the park. I received a pass and was running a touchdown when I was tackled from my right so hard that my left ear banged into my shoulder. My ear was hot red right after.
    I got right up and continued playing. Only later did i realize how.much my neck bent for my ear to hit my shoulder, especially that hard.
    And a few years ago I had an x-ray for a work related exam and the doctor asked me if i knew that my first vertebrae had been broke. I'm in my late 40s at this point. I wasn't aware of that but i immediately thought this may have happened in the football game.
    But there was another very traumatic event. I was wrestling with an older cousin. He was bigger than me and had overpowered me having me curled up with all my weight on my head. He pressed down on me and I felt the muscles in my neck rip. I swear I even heard a literal ripping sound. Somehow, I think miraculously, I got up immediately and walked away and into the house. I remember a hot feeling on my neck and ... It's hard to recall exactly...I was able to hold my head up but i definitely tore some muscles in back of my neck.
    I went on with life.
    But now I'm listening to this video. I have tended to have a high pitched voice. Not falsetto but strained and tense. I had to make effort to calm my voice at times.
    But I no wonder if the scalenes or other muscles attached to the larynx retain tension and prevent my vocal chords from truly being relaxed.

    My neck is so tight and I have such a strained faint voice because of the compensation you mention. My anxiety feeds on it and it feeds on my anxiety.

    This guy is perfect. I’ve been doing physical therapy for many years, and have noticed one thing that all problems your patient comes in with are conditioned/spiritual. I change their spine alignment by training the intrinsic foot muscles.

      What do you mean "hands on approach"? The vast majority of PRI techniques found on this channel, on the PRI website, and on other social media trains the left leg in internal rotation. It's rarely trained by stretching, though. People may envision it as some sort of stretching regimen, but it's not really done like that.

    I have been binge watching your videos lately; especially since i got plantar fasciitis on my right foot. I am left AIC pattern. Until i saw your videos i thought i was going mad - feeling twisted, feeling leg length discrepancy etc. Neck ache. High arches. I came across this video not long after i had put a gel support arch under my LEFT foot [ just like a patient you mention in this vid ]. Seems to really help.     I know exactly the OTHER system you are talking about. In fact, I'm certified in it and have been using it with my clients for years. I turned to PRI because they addressed something no other system ever does, which is how its all connected to BREATH. Through my learning in PRI I also learned how training hip IR without addressing how that SHOULD affect the system as a whole can create pathology. Now, I do use the other system for situations where, for example, a pitcher needs to have strong shoulder ER and IR but now I make sure that they are more NEUTRAL state from a PRI perspective first.

    Thank you for all these videos Neil! I've been to PT for years. I tore my rotator cuff over 10 years ago. I've had Pudendal nerve pain from this etc. I've been doing floor exercises with some yoga for 5 years every day. I'm still tilted. My thoracic scoliosis is mild. You have described what I have to the T! I'm looking forward to trying these techniques and finally making some headway. I feel truly blessed for finding you.

      Sadly, too many people cannot "feel" what their bodies are doing. Interoception and proprioception are important for health and well-being.
      Some doctors are surprised at how precisely I can describe where I feel what kind of sensation. They're not accustomed to having patients with mind-body awareness.
      If you're not squeamish about T.M.I.:
      ...
      A few months ago, I went to the emergency room for abdominal pain. I described it as "an expansion pain in a small place that doesn't like being stretched, and I still have an appendix here," as I planted my finger on where I felt the pain. The imaging revealed a kidney stone in my right ureter. It had lodged itself and was backing up traffic. I could tell when it moved, releasing the backed-up urine, because the expansion pain stopped. Over the next couple weeks, I discharged crumbly bits of white sediment that must have scratched on their way out. I felt "inflammatory swelling." No itching, no burning, no stinging, just the presence of swelling that made me feel like I had to pee! And then ... 😬 ... The Time Had Come. I felt a crunchy, lumpy, jagged little "rock" making its way along the last inch of its journey. Yes, I felt the surface texture of it.... as well as the involuntary spasms. Pain meds and a few hours later, I caught it. Its appearance confirmed the texture that I felt - crunchy lumpy jagged, and about 3 to 4 mm in size.
      Did I forget to mention: I am prone to oversharing. It's part of the Autism Package. 😉

    How can a hip video answer a prayer that was never spoken and has nothing to do with the hip? 😮 I’m trying to make a complicated decision that I inwardly FEEL the right choice but outwardly everyone is telling me their opinions and I’m swaying back n forth. Whenever I go to quiet or seek inward I know. I decide to take a zone out break to watch mindless show n for some reason I decide to go to CZcams instead and I hit this video. Wow! This is so helpful for my work, my own physical imbalances, but what the heck??? Reminding me in such a DEEP way of trusting my own answer to the prayer. ❤

      @@paul-ie In general everyone is more or less breathing with the neck muscles. The scalene muscles on the side of your neck are designed to lift your chest up to get more air in. I found out when your lats and chest are tight, they pull your shoulders down and make it harder for the neck muscles to lift the chest up and then the neck muscles get overworked.

    I never knew that some bodies will activate neck muscles for breathing.
    I had taken Chorus class in high school, and so learned to use my abdomen to allow my diaphram to move downward and also expand my ribcage to give my lungs more air and support for singing with a stronger voice. Even when singing softly, a supported voice sounds far better than a short-breath whisper-singer.
    I've demonstrated the difference to some friends by placing one of their hands just below my collarbones and the other on my abdomen and comparing the movement of shallow high-chest breaths that run out of air quickly and of deeper breathing with abdominal and ribcage expansion that brings in a lot more air. Then i have them place their hands on themselves the same way and ask where their hands feel movement when they inhale and exhale. Some have said that their abdomen doesn't move much or at all, and that they feel it in their upper chest.
    I imagine the shallow, high-chest breathing might be using neck muscles. I'll have to pay more attention to that.

    I found the same result when I put a flat slipper on my left foot. I don’t feel the pain in my knee the way I do without it. However, my hip seems to thrust forward and needs an adjustment.

    Hi Neal. Thankyou for these fantastically presented videos.
    I am curious about one thing. Apologies if you've answered it elsewhere and for a long question
    In the past, ive been told my unstable gait is due to shortening of the left psoas and scarring of the fascia of my diaphragm tissue.
    (Diagnosed through the lens of somatic trauma based physical therapy)
    I wondered if in your observation, the issues revealed in postural restoration, always have their root cause and solution in the anatomy as you work with, or might equally have their root cause, for example, in the psoas , diaphragmatic tissues or ligaments of the organs, or fascial lines?
    Is the fundamental human assymetry and breathing patterns that follow having a knock on affect on everything and could you imagine that those issues too would be at least somewhat resolved eg shortened left psoas, by using the exercises you outline, given that they are communicating with sensory and motor nerves, and the hard to define and locate mind body connection encapsulated in trauma responses holding us locked in fight, flight,freeze,fawn stress patterns?

      The root cause won't be the psoas, but since the psoas is directly connected to the left diaphragm, it will become overactive when you are in the Left AIC pattern and thus lose left diaphragmatic activity. The entire point of PRI is to restore left diaphragmatic activity, actually. I'm not sure how many videos you've watched, but I can only suggest watching as many as possible. In summation, the entirety of this channel explains how "asymmetry gone wild" leads to everything you are talking about.

    I have flat arches. My right hip I think, is stuck internally rotated. (I did have a Rt hip/femoral injury, recently). But have had some symptoms on this side prior to the injury. What is your observation of this? Thank you for responding. In pain, cannot function normally and do my normal things.

    very helpful...but what if it's the left side (meaning extremely tight SCM, dysfunctional neck muscles, tight occipitals) being affected and not the right? would my problem be my right hip joint/adductor instead of the left like you always mention? very dysfunctional diaphragm that seems to stop working sometimes or get "stuck"...causes neurological issues, breathing issues and tight abdominal/psoas muscles which then affect digestive ability. it's my left neck that is super tight and gets stuck, which i think it pinching my vagus nerve, causing anxiety...breathing issues...and limited/slowed digestive capacity/bloating/reflux etc.....would love to see a video on this NEAL or someone address this

      you have to do hip and shoulder testing - IR ER flexion abduction... to see where your restrictions come from.
      Also look for Bill Hartman, Zac Cupples and Chaplin performance videos on this subject (all PRI based)

      You'll still be in the right TMCC pattern, it just so happens that your symptoms are on the left, probably because of extra compensation (possibly bi-lateral TMCC pattern). Forward head posture, basically.

      @@NealHallinan I assume by bilateral you mean both leg bones are turned outward from hip socket…(walking with feet pointing out, instead of in). I also have high arches. Forward head posture is right…do you have a video explaining best way to fix FHP and how to turn off overused neck muscles and turn on the correct ones? Does fixing this start in the left foot/hip joints as opposed to keeping your chin back and not looking down at things, phone etc

    I got two people to check my leg length and they told me that my right leg is very slightly longer (by about 6-7mm) than my left leg. Also, they said my right ribcage is very slightly lower than my left one. However, they said my pelvis looks symmetrical on both sides. I've been suffering with LEFT ear tinnitus, LEFT lower back pain, TMJ on BOTH sides, Snapping Hips in BOTH hips but worse on RIGHT, eye floaters in BOTH eyes, severe pain below RIGHT ankle (which in reflexology points towards a problem with the lower back/pelvis) and clicking in BOTH knees but worse on RIGHT. Please can you tell me if these symptoms are typical in my situation and would they be easily treatable? I want to know whether PRI is for me as it is hard to find a PRI specialist around here. Thanks for your help.

      I think you'll need an experienced PRI practitioner, truthfully. Your symptoms indicate a lot of tension.

    This is fascinating, i am 48, when i was 14 i had hip pain whenever i walked ran or stood for too long, Drs did some internal rotation tests and found out zero rotation, sent me to xray and found out i have SCFE (Slipped capital femoral epiphysis), which has caused my right leg to be 3/4 inch shorter and left leg 1/2 inch shorter. ever since i have been in pain if i did anything on my legs or if i tried to ride a motorcycle because it pushes the legs and requires internal rotation. fast forward to 3 years ago, i started having breathing issues i tried to breath from my belly but it seams very difficult like there is no room to stretch there. this caused some sort of hiatal hernia, because i had silent reflux issues while sleeping and made breathing even worse.
    listening to you everything makes since like pieces of a jigsaw puzzle.

    So is a sign of this... Have a seat then look at angle of feet. Or stand and look at angle of feet? They should be straight? Mine seem to be slightly angles outward. BUT when I walk I can feel supination/pronation in my feet when I walk... least I'm pretty sure

      Well, you have to also take into account the position of the tibias, which can be opposite the position of the femurs. So it's hard to tell just by the feet.

    Breathing is a real issue for me. I hold my breath when I'm concentrating and get light headed.
    I broke my nose 40 years ago in a car accident and I didn't notice until recently that wearing my glasses block the nasal passage!

    This is spot on with what’s happening to me. The only thing that is different with what you are saying is that I’m breathing through my “left” scalenes. My scalenes/scm are always contracted on the left. My right scm/scalenes are always tight. Pain though is always on the left from neck, shoulders, scapula /ribs feel twisted torqued, now left hip/sij have kicked in. Not sure how gradually this has been going on, but the last three years it has been getting worse and of course everything’s being piecemealed on the actual symptoms and not the root cause. My right side from neck to leg is much thicker feels like everything’s bunched together. I have right sided rib flare. So my question is can this happen on the left side of the SCM and scalenes and do I still do your exercises the same? This is ruining my body it’s screwing with my spine and getting worse. Also do you see clients I’m in another state but needed I may consider traveling?

      The pain can be all left-sided. The right side is still the issue. Check out my video about the right subclavius muscle. It can cause lots of left sided neck pain. Check the PRI website for providers in your state.

      Hi Neil again, if I may ask one more question… please
      I know you mentioned in my previous comment (above this one) that the pain can all be on the left, got it. The thing is though my breathing I feel in the left area scalenes etc., my rib cage on the left feels like it is being pulled down. Now it also has become a left hip instability issue. You are correct, my knee on the right is now hurting. My right ear is more compressed than the left. In fact the whole right side from neck to spine is more dense. I do release my scalenes/scm on that side. I also do the subclavius. It all releases quite well. Not so on the left side. Also, My QL is causing me horrible pain on the left. I cannot feel my left glute (inhibited?) firing when doing a focused exercise on that side. The QL will always kick in.
      So I guess my question is aside from the pain and instability on the left, I just wanted to make sure it would still be a right sided issue before I start doing your exercises . Thank you very much!

    Neal, you suggested finding a PRI practitioner near me a while back, I’m not having any luck and I seem to be stuck in the overextended on both sides pattern. My entire body is tight and fighting itself, my breathing is getting so bad I am barely functioning. Is there ANY way you can help me, I’ve exhausted everything just trying to feel ANY comfort at all, I’m so tired but my entire neck, face and head get tight and burn while I sleep, like it’s trying to lift my ribs through my skull. I seriously can’t live with this much longer, I’m throwing my last Hail Mary to you sir. Is there anyway to get help?

    After listening to this, I wonder since my pain is on the left side neck and shoulders and chest, would I be the opposite of what is described here or maybe both sides?

    PRI seems so easy but it’s so difficult. I do the exercises correctly (to the best of my knowledge) But I still can’t execute the exercises fully because of my neck being turned on when I inhale and exhale, I hate it!

    Neal plz reply, I'm stuck with confusion on this, in your left Zoa videos you describe the right lateral chestwall to be tighter, but although I think I have left aic pattern I've always been feeling tightness on my left lats and discomfort on the left side of my upper back, when I raise my left shoulder above head there is slight grinding sound. But no such tightness or discomfort on the right side of my body, never got tightness on right lateral chest wall, no sound or discomfort on upper back or shoulder either. By hand measurement, it feels like the distance between my ribcage and pelvis is lower on the left side although it could be feeling that way because they both moved out of the initial position, I do feel like my left pelvis is a bit forward and rotated towards the right

      It doesn't matter where you feel the pain, the underlying pattern is the same. You may not feel anything over on the right side. Left sided pain is usually due to prolonged compensation.

      Great question Gerald..was wondering the same. are my problems from stuck diaphragm, tight neck muscles/dysfunctional hip joints/flexors OR is the REAL issue cervical instability.....meaning lax/loose ligaments, muscles and tendons not staying in their place and holding the body together. is it possible to fix postural/diaphragm/digestive/neurological issues without addressing the instability of the cervical discs or does PRI come first? it's a chicken and egg scenario....Dr. Hauser from Caring Medical in FL has a youtube channel and i follow him.......the ultimate question is: do i do PRI or do i need to get prolotherapy for the instability prior to doing PRI. i think it's def a missing piece of the puzzle here in SOME people

    How can i find a therapaist in Gibraltar (not the one in the US the tiny country at the end of Spain). I was knock kneed from when I started to walk. Its got better but i still trip myself up a lot LOL. I had years of achilles tendonitis in my left leg and realised it was due to pronation and my excessively high arches. So i got supports to stop that. Amazingly the achilles tendonitis went which was replaced by plantar fascitis and then later sciatica 🙄
    Have spent a lot of money on physios and chiropractors but I need someone to teach me how to stand and walk first! Ive checked online but i couldnt find a therapist in my country 😮😢

      It can be any sneaker that fits the criteria listed on the list. The heel has to be solid and it needs to have an arch. That's really it. Doesn't have to be on the list.

    This is was of your best videos ever at explaining how the Left AIC pattern causes a chain reaction of compensations and related issues. It really crystalizes and connects the audience why it is so important and necessary to reclaim internal rotation of the left hip. This video really created that ah hah moment for me and how the Left AIC pattern (for me) is likely the contributing factor to what Dr. Google describes as Maignes Syndrome a/k/a TLJ Disfuction on the left side side.
    Have any of your patients exhibiting symptoms of Maignes Syndrome gained relief from PRI Exercises and training themselve out of the Left AIC pattern?

    I like what you're doing here. It is the same thing I'm trying to achieve on my channel (and have achieved in quite a few videos).
    I would encourage you to keep it up. It's nice when concepts get blended together this way like you've done with Campbell and internal rotation.

    What happens if you walk strongly to the inside since birth. Is that a different problem? Or am I stuck in internal rotation and thats the reason why I have the same problems, mostly heavy neck breathing.

      It's possible that there is a structural issue that is causing your problems at your neck. You'd have to see someone who can evaluate the situation.

For us who can t find a professional with your understanding is hard to find a solution, what is what one has to do to fix this exact pattern you described here? Is it the grounding matter?     In your videos, it seems like you tend to sit on your left side which is a good thing if I understood correctly because it opens up your right side. Are you sitting on your left side on purpose?

    Neal, where do you operate your business and are you taking on patients? Been dealing with right hip issues for 18 years and I've seen and tried everyone /everything. Recently seen a physio that was supposedly PRI certified and he never once looked at or talked about anything PRI related.

    I suffer from :
    - left pinched femoral nerve impingement symptoms but no one knows where it is trapped (mri revealed no disc herniation).
    - Pelvis lateral tilt. Left side is 10mm higher.
    - Left hip FAI.
    - Phase 1 of bone spurs on lumbar spine without disc inpingement.
    - Frequent torticollis on left side.
    - Cannot sit for more than 1 hour otherwise extreme left thoracic muscle strain and pain.
    - jaw shifted 5mm to right.
    - left shoulder 8mm higher.
    - very slight lumbar scoliosis due to imbalance.
    I consulted more than 100 specialist to find a solution, as my hip is developing arthrisis because of the imbalance.
    Source problem = years of prolonged sitting in front of a computer. And abrupt stop of bicycle training.
    I am clueless what to do to get that pelvis back in place.
    Should I go back to bicycle sailing?
    The pain is almost unhuman.

      If you are in the US, try finding a PRI therapist to help. When jaws are shifted to the right, things can be very difficult/complicated.

    In my everyday life I try to become neutral by enforcing all the cues I've gathered... But they are so many. Maybe that can inspire a video idea for you? I never know where to start / focus on, everything is connected.
    You want the left diaphragm. To get air in the right chest wall.
    For the Diaphragm you need the lowerspine to go left and not extend. Means you need your pelvis to orient left. Means you need your left hip to come back. Means you need your left hamstring. Means you need your left heel, left sit bone. And your left foot supinating. You also need the left adductor and the femur coming in, and the left abs and the left ribs coming in. Now *everything* reversed on the right. And that's just the diaphragm, not even the chest wall, or grounding.
    It's so overwhelming everyday, simply too much to focus on any given moment, any ideas on how to strip it down more effectively?

      PRI techniques do exactly what you describe (very nicely described, by the way) so you don't have to think about it during the day. It's just walking and breathing without compensation. A PRI program trains all of this so it becomes as automatic as walking and breathing should be.

    Hi Neal, is it normal to struggle with the pri exercises at first? I've started working with a PRC last week and find the exercises difficult to perform without breathing with my neck.       What you describe is the exact point of PRI: to inhibit the overuse of the neck as respiratory muscles. That's at the core of everything PRI is trying to do. So yes, it can be very difficult.
      I posted a salsa video on Instagram two weeks ago. Nothing on here at the moment.
