some items will trigger when you ult since udyr builds either Q or R but not both then in the Q max build Q is his ult effectively so that's what that means and why.
The 14.3 patch made it so double proccing an ability(which is essentially his ultimate) will interact with items that trigger off using an ultimate ability. Malignance specifically requires the Q or R because it applies to damage from the ult and W and E don't do damage.
