They had a falling out in Mingus' apartment so Mingus punched him and fucked up his teeth, and by extension his embouchure. Messed his splaying right up.
@Spencer Paulsen yeah because it messed up his high range on trombone, which you might notice, Jackie did not play

Are the chord is concert pitch or adjusted to the saxophone? Because the original key has a d flat in it which means that there probably would be a d natural chord there. It would be f in concert pitch in that case. Is the fist chord d or f?
