Of course they really kissed behind the scenes, look at how nervous Yoko was about it! To do it for the first time in front of a whole film crew would be even more awkward.
You can see that they first act out the whole scene, but they don't actually kiss; that's when they're "blocking" the scene, meaning they're working out all the movements. They don't need to kiss that time.
Then, the second time, they really act out the scene, which is why they kiss that time.
I'm sure they all discussed it first and asked what each person wanted and was comfortable with before they started.

I think it's great they had a supportive staff on set or someone to help, and they were supportive of each other. I hear about some sets where they meet on first day and have to do intense romantic scenes right away, and they didn't get a chance to talk about it or get to each other's comfort level. Except being on set, saying hello and trying to get the scene done properly. I'm glad they took into consideration that some scenes would need discussion and work.
I really love there is no awkwardness between them when they are kissing and doesn't look vulga* just pure emotions and so in love with kissing each other.
