I feel like if you're promising your patients mum you can fix him with spider silk that you'd want to make sure you actually had enough silk to use BEFORE you started operating.
It's pretty against the rules to promise _anything_ when it comes to the hospital. Comfort a patient as best you can, but you cannot promise things will be fine, cause they could not go the right way

Arachnids are amazing creatures that definitely don’t get the respect and admiration they deserve. Which is why I worked to over come my fear and now own two beautiful tarantulas. One day I will have a third and several jumping spiders.

Last week I got a bee out of my bird’s water bowl when he was outside. I mixed up sugar water and put some near her and kept an eye out while she was drying off…..

That lady doctor just wants to be right. I encountered a couple of pediatricians when I was a kid who wanted always to be correct, because kids can't argue on the same level as the adults. But the guy doctor is exactly the type of pediatrician that my main practitioner was when I was kid, he would listened to everything I said, he made sure all conclusions and methods were considered before coming to my mother and me and giving us the options, and made sure we understood our options. As a doctor, you don't just make sure your patient lives, but you make sure your patient thrives as well. The "common" and "easy" solution is not always the RIGHT one.
well it's a risk vs reward curve... if the risk is him living possibly a very uncomfortable life vs a life without legs but still being able to utilise the ability to run and walk. I think the later is clearly better....

No this is what we call medical malpractice. Something this risky that's never been tried a doctor can't say this is absolutely what the decision we would make. We have to look at is the juice worth the squeeze. And young kids are very quick to bounce back from amputations and prosthetics below the knee don't stop you from running. I spent two years at Walter Reed with the best prosthetic doctors around not to mention I'm missing my entire leg so I have a little bit more information than you I've watched people try to salvage limbs for years before finally giving in and having it cut off the pain the anguish the depression you have no clue.
I'm not an expert, but even if you manage to somehow manage to fix the damage that's been done on his legs, I highly doubt he will ever properly utilize those legs, plus months if not years of pain medication, maybe never being painfree, most sports might be out of the question, the bones might grow wrong and any issues that stay into adulthood will grow worse.
The female doctor was just worried about the boys chance of survival. Shure, he would loose his legs, but he had a far higher chance of keeping his life.
My son had a pediatrician like that female doctor as well. I quickly got rid of her and put her down to never talk to us again. My other doctors for him are awesome.
There is another problem with bones fractured this much.
In the military they often have to amputate not bc they couldnt fix the legs
but bc of abnormal very painful growths.
my aunt was in AF and jumped out a plane and her parachute malfunctioned n broke both legs. she’s had over 50 surgeries bc she didn’t want to amputate them. so sad to see her in pain often

I am not
When he said he wasn’t going to let them amputate the little boys legs, that’s the same idea my docs had when I suffered late stage CRPS in my right foot; except they were wrong. I pleaded 10 years for an amputation, but they insisted the pain would only get worse (not possible when you already want to end your miserable existence just to escape the pain, even though I’d never actually do that … that’s why they call it “The Suicide Disease”, though). Then in August 2019, I took an infection in that same right foot (secondary to sepsis, which I had almost 6 years prior) & it appeared to be moving up my leg … so, they finally amputated in September. Unbelievably, my CRPS was gone the moment they took away that diseased limb & the agony they said I would be in was absolutely laughable (I’ve had menstrual cramps far worse than that). The doctors were stunned by my recovery, as I was back home 3 days later, with regular checks on how the wounds healed. To this day, I’m still amazed at how long clueless doctors kept me in painful limbo, all because they didn’t know enough about CRPS. More & more people with the condition in its later stages have since gotten their amputation surgery & went on to lead normal lives. I’m only getting ready for my prosthetic now, though, due to Covid holding things up for so long. With any luck, I’ll soon be walking for the first time in 13 years… 🙂🏥🦿

I have CRPS in my left arm. I don't think I am in the later stages but they are not really treating me the moment. All I get told is medication is bad and that I need to come off it. Yet I have to use all my tools that are non medication based to just cope with the pain. I have built tolerance to medication but never dependent on the meds and always took the dosage suggested by my doctor. I love how they just lump this pain with other pain and say "well long term pain meds are bad and so you get nothing, even though this condition is excruciating". I am now only on Lyrica and thats all. No pain meds (well paracetamol with ibuprofen and an anti-histamine that can know me out for the night), no anti-inflammatory meds or anything else really. Even CBD oil with additives didn't work. I can cope with it most times but other times I just want something to stop the pain

@@droid88-b56 Yeah, I wish I could say that I’m up & walking, but alas; things are more complicated than that. Because I’ve been immobile for 10 years & my legs have wasted over time, my attempts at using the prosthetic were a huge failure. I left the clinic, only being able to take a few steps with the wheeled zimmer walker … & then I had to stop using the prosthetic, because I was getting too much weakness in my good leg & I knew I was going to have a fall if I continued. I’m sadly now bedridden again, only getting up with my powered wheelchair. My fam have promised to make my life a good one, though & as I’m also awaiting a major court case to be settled (medical negligence for life-threatening errors & life altering consequences), they’ve put a large sum into an account for me to use in the meantime & I only have to ask my mum or sisters when I want something, then they’ll get it for me. Got some new clothes, new supplies for art & craft stuff, as well as a lot of games I’ve been wanting (including a few rare collectible PS1 games that I wasn’t able to get before). I’ve been glued to my Nintendo Switch for a while now, playing “Pokémon Shield”, so I’m still reasonably happy. We’re hoping the court case is settled by my upcoming birthday (May 2024) & perhaps we’ll even get a new place to live that’s more spacious & caters to my disabilities. Anyway, I’ve been told not to worry about it, so I won’t. I’m a little sad that I won’t be able to climb into a rollercoaster, but you just never know what the future holds. Maybe with a few years of recovery & gentle rehab, I’ll be able to wear the prosthetic again. I’m not going to stress about it, though. My mum & sisters have promised to take me on trips (which I’ve insisted that my little chihuahua with PTSD will be coming too) & some plans for those future trips have been discussed. I’ll be alright, with my wonderful fam & our family solicitor sorting all the legal stuff. I’m just glad to be alive. 🥲💖👩🦼

Kid being concerned for his mom in the beginning was so heart warming. I had a situation like that. I was 17 almost 18 and I was at running practice. Our school was next to a decent neighborhood and we were running past it. My best friend and I were with our running coach and my coach was a few steps ahead of us and a drunk driver who was our age came ripping out. I remember moving as fast as I could and shoving my coach out of the way and the side of the car slammed me so hard when he came to a stop. Our coach was our best friend and a parent of three and was like a parent to us all. The only I remember after waking up was me in the back of an ambulance in serious pain and she stayed with me the entire time and she was thanking me for being able to live for her family and the rest of us. Drunk driver would have killed her if he didn’t stop and if I hadn’t shoved her out of the way. Doctor said I was lucky to be alive and still walking. Even said I was lucky to have just bumps, bruises, scrapes, and a hairline fracture in my pelvis and a small one in my skull. All I did was worry about her and my best friend being ok and not me. Not too many people think like that. I’m still to this day up and running and just had my 21st birthday and I still run into my coach from time to time. We’re both blessed and thank God for every day we have with our friends, family, and getting to run to this day.❤

I’m no expert but I’m learning right now, didn’t they put that kid on the spinal board wrong? If he possibly haves a spinal injuries, they need to tip him to one side and slide the board under him, not lift him onto the board, if I’m not mistaken. And I think they did suspect a spinal injury considering they immobilized the neck.
Speed, vs. everyone's safety yoinking the kid out from under a car on a jack that was applied in a hurry that may slip, and a crowd of rescue workers is pretty tricky to film correctly. Immobilizing the neck is a default for extractions, it's easier and safer and helps prevent people from thrashing around. It's been decades since I worked ambulance, but some things seem not to have changed.
It is best practice to log roll a patient who’s on spinal precautions onto a board
Spinal boards can also used for short transport if a patient isn't ambulatory

I don’t want anyone to think this is how we would actually work this. As far as I’m aware, spider silk isn’t tested for something like this. Also that fracture is so incredibly bad that I’m shocked he didn’t die within 6 hours. He’s not walking again and he isn’t keeping his legs. And 0% he could wiggle his toes after that
Thankfully this show isn't meant to be realistic.
So, here's the thing. What is the risk of taking the chance on the spider web surgery as opposed to amputation? The infection? Then, you try the spider web option and if its goes wrong then use amputation as the last resort. Amputation shouldn't be the first go to when there is another option that they can think of.
If you have to amputate, you try to do it as soon as possible so you amputate as little as is necessary. Those bone fragments are going to move and cause other issues the longer you wait.
And they didn't have enough spider silk to be ready to do the other thing quickly.
The reality is even if you put the bones back together and don't cause an infection. Chances are the bones wouldn't heal properly. The kid would be stuck to a wheel chair for the rest of his life till he chose to amputate. Also with being in horrific pain the entire time. With a much longer recovery time from the amputation that he put off.
@@nathaniels9141 to be fair there are some case studies that patients had time to come to terms with their limbs needing to be amputated so having the limb attached for a couple of days could help with phantom limb and helps them adjust to a life without that limb. As suddenly not having a certain body part to a very jarring experience.

the issue would be getting enough spider web but it should be possible. and also one would have to exactly calculate the decay rate of the web itself
It's been tried, but hasn't caught on. I'm not sure why, whether it's not yet strong enough or still way, way too expensive, or there is some compatibility problem, or what.
Resource-allocation, that and human incapability with patience. Spider farms are a thing and very profitable in both silks and meats but is focused purely on long-time profits which is an uncanny concept to mortal men. That kid would literally be much better if the hospital board weren't possessed by their troubled childhoods.
My mother told me a story when I was probably 4-5 about them having to use a spider web in a tough scenario on one of her siblings ( I can't remember, it was 11 of them total) when they were kids and playing one was cut real bad. This was 1989-1990,when she told me this and I was just as fascinated then! I'm shocked to see this it took me back to her telling me about it and I'm like an actual spider web? She was like yep! Now, to see it and it's amazing.

The actual cost of the screws, all the costs related to complication, and all the costs related to this 1 in 1000 chance literally would cost tens of thousands more than prosthetics. These medical dramas are just getting wayyyy too focused on the drama.

I feel so sorry for that boy, when I saw the x-ray and saw Clay's neighborhood my heart broke, I even almost cried, and I've never heard of spider silk screws until now it's amazing doctor's invented these to help keep bone's together but they dissolve after a while and the bones are healed and the nerves stay together

One of my most FAVORITE critters in the world! Ironically, I made up a treatment involving a fantasy spider for a story that allows the body to absorb it as an energy source while the injuries healed! This is awesome when I see it in this! ^^
BTW, the way this pediatrician, NOT a doctor, goes on and on about prosthetics like it's EASY to chop off your own legs and replace them with metal contraptions! She's a chatacter who needs to pull her head out of her ass and grow up! New Medical treatments are popping up every day, and it takes a slecial kind of person to think outside of the box and maske it happen!

Amputation would be best in my opinion. His bones may fuse abnormally and he could have chronic pain for the rest of his life. He’s also much more likely be able to run again in prosthetics than his legs like this.
I think they are both at fault. The woman is somewhat dismissive of at least better exploring the option to keep the legs with the patient and her comment was uninformed at best. She should have given more consideration to the possibility of avoiding amputation. However, from a purely practical standpoint, she did have the better option in terms of risk vs outcome. (Saying this as a student of health sciences) The male doctor was assuming prejudice on her part and, therefore, was being unnecessarily critical of her analysis. He has difficulty being objective toward his college's suggestion of amputation due to his ethnic connection to the patient and her uninformed comment. However, he is expanding the options with his creativity which does drive healthcare into new realms: a good thing. They should have been professional, carefully explaining the risks and benefits of both in a neutral manner without hijacking the conversation. With both there would likely be no guarantee of removing pain-- nerve damage would occur regardless of the choice and could cause chronic issues-- there is a risk of losing functionality in the legs if they are kept, risk of infection for both, but increasingly higher with more internal manipulation (infection could easily lead to gangrene or septic shock which would threaten his life and be cause for amputation). The male needs to be gracious towards his college's valid, though misguided, frustration and not let defensiveness or even counter-prejudice against perceived prejudice influence his decisions. Likewise, the woman needs to be more careful with her words at best, worst, or put aside her prejudice, at worst, and be as objective as possible in her decisions.
Amputation, at that point, would also have been much cheaper and faster for the hospital. Hospital budgets are not bottomless, and surgical theater time is not endless nor are the hours of available surgeons. Triage, using resources to treat the most people effectively, is a thing.

probably would amputate, the doctor got his feelings on the way which is absolutely dangerous and can lead to malpractice, but this show's writers managed to turn it to a race problem

The council who owns that city should make a grass verse or a park in that area where the doctor showed in his neighborhood but not being horrible or anything but the mother should of made a plan to take the kid to a park that's close by to play with his friend. But tbh i think it's the council's fault for this one, they decide to create buildings instead of creating parks or play areas for kids to play around.

Spider silk is made up of chemicals, one if which is the same thing that clots our blood. I'm a nurse, and got into a discussion with a surgeon (1980s) about this. He was surprised I knew about that effect. Told him I watched my dad walk up to web in the woods a grabbed a handful to STOP a bleeder from a fishing cut. I was 8 yo,and never forgot it, used it a couple times, too.

I don't understand why they needed screws. i thought the screws were suppose to hold in a plate to keep the legs straight. drilling into a leg thats already shattered just makes it worst? i was thinking of sewing the bones back together. but they would need to cut open the leg almost completely. since silk is stronger that steel but more flexible why didn't they just sew the bones to connect them. they only have to make a small hole compared to the large screws he showed them.
Those legs are smooshed. They'd have to open up the leg to apply the plates anyway, and plates in growing bone can distort their growth. They can also require removal years later. I still have mine from 35 years ago,, but the screws heads have worked their way out a bit. Not quite enough to need surgical removal, but enough to be cautious about them.

I use to live in a neighborhood that had no parks and had locks all over the school because the schools were having a high risk of robberies and school shootings it got to the point where they had to have security cameras and too check people's bags and the neighbors were the worst there were so many robbed people and kids were constantly getting hit by cars or almost or getting shot due to the high crime rate that part where he said that there were schools were padlock or parks really touched 😭😭😭😭😭

See something I've always wondered, of someones bones were shot like that, but averything else was fine... I mean people get hip replacesments and stuff, so with all the kinds of modernt materials and compersite materials why not make a replacement bone inside the orginal leg.

Idk, it's pretty obvious he's projecting a lot. Her medical recommendation had nothing to do with race. I think the male doctor is the one struggling to be objective. I'd argue it's more unethical to be glossing over the options with mom. The male doctor is basically trying to ensure she's not informed of all her options because he thinks she might take the less controversial medical option of amputation. Manipulating her is worse than a flippant comment by the female doctor.
It’s always worth the risk to try what exactly going to happen he loses his legs anyway and the problem was partly with the comment more to do with her lack of compassions and disregard or their situation.
You could very well argue that her approach is more coming from a place of dismissiveness. The easiest thing is amputation, so that's what she wants to do. And why go through the trouble of saving the boy's legs if his careless, lackluster mother is just gonna put him in harm's way again anyway? The lack of sympathy and the lack of motivation to give the boy more options is telling
No but seriously. What happens after the surgery? His pain would be out of control. His legs would be saved, but would their functionality, would his pain? I get that the female doctor’s statements aren’t right either, but neither are the other doctor’s.
I don't think she was wrong to tell the mother. It's her job to inform her of all the options so she could make an informed decision.
@@monmon-vk8rz It's not about effort because amputation is not easy. They don't just pop them off, it's a long surgical procedure. Also the boy would need to be fitted for custom prosthetics which is an incredibly time consuming endeavor. Outside of a tv show, there's no way an ethics board would approve an experimental procedure on a kid like this when at best he'll only have partial functionality of his legs. (Also insurance wouldn't cover costs for experimental procedures.)

I would chose the amputation. Like leg and foot prosthetics are seriously far advanced this day and age and with proper after treatment and training you can continue aa normal life as far as having amputated legs go. Hand amputation however I would not do lol

We played football, soccer, hockey, skateboarded, played tag in the street because there was no park in our area to do these activities. Moms do the best they can with what they are given. Letting your children do something has nothing to do with it. They will play where they can regardless of permission or not. Accidents happen, its not child endangerment to let your kids play in the street, its whats available.
