The chant is C.H. Lloyd in B flat. One of the chant books in which one can find it is RSCM Chantbook (1st and 2nd editions).

I had a real hard time finding ANY scores of this, but one that was like a copy of a photocopy of a photocopy, it has been cleaned up and i have the clean copy now, if you still want it let me know an email or something to send it, it's only about two stanzas long and repeats. I don't remember what key its in, but it was the ONLY one I could find anywhere.
