@@YoursCunningPanda 1) for topical minoxidil the half life is around 22 hours, so some people say 1x daily is enough. I however applied it 2x daily and initially experienced great benefits, however it slowly started to lose efficacy over time.
2) side effects of minox? I experienced no side effects of topical minox. Maybe one or two instances of very minor heart palpitations, but that could’ve just been coincidental. As for Fin, the only side effect I experienced was an INCREASE in sex drive. I have now been on fin for 3 years and 4 months with zero sides and it has maintained my hair this whole time. Transplant in Turkey inbound in 3 months.
3) shedding phase is different for everyone. When I applied Minox I had a shaved head. So it was hard to tell if I shedded a not or not. By the time I grew it out I had no shedding. And no, I personally never looked bald. But it’s different for everyone.
4) no idea how to answer this. Tolerance for Fin seems to be a sketchy subject - at least orally. Most long term studies show that fin works almost indefinitely as long as it’s taken consistently. However, some people may experience further balding but this is due to their androgen receptors reacting to testosterone levels, not DHT. DHT will remain blocked as long as Finasteride is in the system. As for Minox, I definitely noticed a decrease in its efficacy after 18-24 months. But that’s just me. If I were you I’d focus on applying minoxidil typically and Finasteride orally.
5) you should leave minoxidil on the scalp for around 4-8 hours to ensure max absorption. Applying first thing in the morning and just before bed is the ideal slots to apply. As long as your application has been on for 4-8 hours you can wash your hair and reapply. This was my strategy and it worked well.

@@haircafekevin the slaphead curse is a truly evil one indeed.
@Sheikh Abdul Rehman No, there is no strong evidence it does anything and bloodflow aka bludflow has nothing to do with hair loss.
You must be new to my channel but here is one of the many videos I made which debunk the "bludflow" theory.
czcams.com/video/icSupaq9hBA/video.html
@Sheikh Abdul Rehman You need to use a 5AR inhibitor if you want to have an oral medication that stops hair loss long term. Ask your doctor about a low dose of finasteride, just 0.5 or 0.25 MG daily or every other day if you're worried about side effects.
Hi guys.....if you are new here and still on the fence about starting the pill because you fear the possible side effects, then you are not alone....I was once just like you, it took me 6 months to build up the courage to pop my first half pill. Before then I was on topical finasteride mixed with minoxidil for almost a year. My hair grew back nicely, but wasn't sure if it was just the effect of the minoxidil...it was for this very reason that I decided that the pill is better as i would be getting a guaranteed consistent dose everyday. No other channel or doctor even could convince me to take the pill as I was just too scared. It was ONLY due to Kevin's persistent, aggressive, no nonsense approach to finasteride use that finally gave me the courage to take my first pill. I was of the unlucky few to have some side effects, i experienced some brain fog for a few days and watery sperm....side effects can happen with ANY common drug so I didn't let it get the better of me. I kept with the pill and my body adjusted, all side effects subsided within a week.
I'm 8 months in, feeling better than ever, still lifting heavy in the gym with no issues......once I was feeling down/depressed for no apparent reason, i was worried that it could be the pill that was causing it. It just turned out that i wasn't getting enough sleep. After i adjusted my sleeping patterns, gymed earlier and went earlier to bed, i felt amazing after that. It's so easy to blame everything any anything on the pill.
So my advice is to just take the pill or half a pill without thinking about it, and then minoxidil. People generally fail with minoxidil as they are not consistent with it. If you are consistent with it, you will see amazing results, and you will almost be happy and excited to do your next application of minoxidil. It doesn't have to be a long and tedious process, you can just apply to your scalp in a few seconds and massage it in while you're rinsing your mouth with mouth wash or something, as easy as that......best of luck to anyone starting their journey. Start tonight if you must, not Monday or next weekend. Trust me when I say, watching yourself go bald is more depressing and probably worse than the side effects you probably won't experience. And also, girls DO NOT prefer bald guys.
Thx for this, man. Read it all

Hey Kevin, you gave me the courage to finally see the dermatologist and get on finasteride. I found your channel by googling evidence against post finasteride syndrome. I've had friends claim that they only took a few pills and it "messed them up" (they were users of the propecia help forum). I was a victim to the fear-mongering but your evidence based videos quickly demolished any fear I had. I have zero side effects so far. Thankful that I'm starting fin at 25 with a norwood 2 and I won't end up looking like Humpty Dumpty by 30.
Ever notice how older, bald guys discourage younger, balding men from taking any action to save their hair? I just find that hilarious.
Hey I had to use finasteride topcially since I got side effects from oral finasteride (testicular pain) which did not go away unless I stopped using finasteride. I can use up to 0,5mg topical finasteride daily without getting those side effects but it still stopped my hair loss. However as soon as I start using more I instantly get the side effects, so I can kinda second this study. If you get side effects from oral finasteride just use it topically in a lower dose.
Was it topically beneficial?
@@alialgadir5610 Yeah as I said, it stopped my hairloss and I get no side effects as long as I do'n't use more than 0,5mg per day topically.

So glad I stuck it out - too many people give up at the first sign of sides. Shit works yo! And I still shed a lot of hair. Love your work Kev
Thank you for sharing that, that is an important detail people often forget about sides. They usually go away even with continued use to give it a few months.
Yep! That was me. And the convenience of popping the pill with my morning coffee is a reason Ive never used minox and won’t changing to topical if it does ever become readily available.

Now I am taking 0,5 mg ED and I stopped taking P5P 1 month ago, and I don’t have this side effect anymore 😀 I am not sure if P5P helped somehow, but I can 100% agree with the statement above.

Have you noticed any regrowth or just a stop to the hair fall? I've been on minoxidil and derma stamp and also taking pumpkin seed oil for 6 weeks and my hair is slowly getting worse. Idk if it's the minoxidil working or just further hairloss. I was gonna order topical finasteride but I guess I should try oral first.
@@Vakator-29 Hey yeah I noticed regrowth and/or thickening. But if you're losing hair after 6 weeks on minoxidil could just be shedding phase of minoxidil still. However, either way Finasteride will help dramatically cut off the hair loss. When I took Oral fin I felt sides right away even with super low dose like 0.25 mg and even less, but iv been dumping Topical finasteride and Dutasteride properly compounded by local pharmacies. No side effects at all! But now I'm not sure it's even working 😕. Il even do demarolling and use topicals with no sides. How do I know if it's even being absorbed? Should I get a blood test?
Did you got results ?
I use topical fin at the front and temples to stop the hairline from receding and microdose tablets orally, as I'm prone to side effects. Topical fin is really underrated.
Just a word of advice, if you're going to use it, have it compounded properly at a pharmacy instead of crushing fin tablets and dissolving them in a bottle of minoxidil. It's cheap to do and you know exactly how much fin you're applying to avoid accidentally using way too much.
The blood Finasteride concentrations are also much lower with Topical and that is a deal breaker and could stop alot of epigenetica changes caused by oral finasteride.

The reason topical fin appeals to me is because you can compound it with topical minoxidil. Currently I only take oral fin and no minox, but if and when in the future I need to add in minoxidil I will probably just get fin and minox compounded topically so I can have an all in one treatment. Its more convenient than having to take a pill AND apply a topical. Also maybeee in the future we could have a topical fin/min/pyrilutamide compound? That sounds pretty sweet.

I don’t see the adherence issue with topical finasteride if you’re already taking minoxidil and can just compound it into the minox solution. In fact, it becomes one less thing to do as opposed to having to remember to use your daily topical AND pop an oral

Ten days post transplant, winning the good fight! Wouldn’t be here without you, Kevin!
Yoo, liked b4 even watching. Btw I started fin recently b/c of you. I'm thinking of getting a topical anti androgen product like fluridil. Would I have to place the fluridil in a sealed container to prevent moisture or is that overkill? Thanks man.
Fluridil comes in sealed glass vials so no need. Just keep it out of direct sunlight.
Thanks for these vids mate. Not only the depth of info but reading other peoples comments and your replies give plenty of info. Your shedding vid helped settle my concerns and locked in my conviction to stay the course.
Im 37 and just finished my first month on 1.25mg fin daily. Hair fall increased a lot 2 weeks ago (150 noticeable hairs per day). Im wondering is this a shed and how long do they typically last for? I’ve seen a bunch of your commenters sharing different shed starting dates and lengths of shed. Cheers mate!
it should last about 3-4 months tipically but it may be shorter or it may be longer everyone is different. just be thankfull and take it as a sign that the treatment is starting to kick in.
Kevin, any thoughts on mixing topical Finasteride with minoxidil and stemoxydine?
Josuke is a bluudflew specialist and his Stand crushes AA ooraooraooraooraoora
Got my scrip of fin 1mg. 3 months for 12 bucks is practically free in my opinion. Still can’t muster the balls to try it!
Just think of it as a testosterone booster. Lots of people report higher libido on it because it preserves your testosterone by preventing it from getting wasted into DHT.
Don't be like me, choom. I waited about a year after filling my first finasteride prescription before I finally started using it. Now around three months into using it consistently, and I only wish I started sooner. Only side I've noticed is a lot of new chest hair, of all things.
hey Kevin , I started fin 2 months ago for my hairline. I have noticed in the last 4 weeks I have a ton of white fuzzies tiny hairs that have appeared around my hairline but I dont have gray hair. what is your best guesstimate on this? I know may be a stupid question but its almost like peach fuzz that wasnt there a month ago
good stuff. not sure if i should do topical 0.25 or just go back to 1mg oral. i want to go to the tablet because i know it works well for me
Kevin will you use Pyrilutamide when it comes out? And when do u think Pyrilutamide will be on the market?
It's currently undergoing phase III clinical trials so based on that I'd estimate 2023 or 2024. Yes, I will definitely use it although even if it is as great as it seems I'll still continue to use finasteride.
Sup, choom. How are you living? I hope everything is nova with you, and you're having a preem week.
Legend, keep up the great work Kevin!
Thanks choom!
Thank you Kevin, although I'm not interested in topical finasteride, I still watch the video because I love the way you analyze the studies. By the way, according to your video about grey hair, I started applying Melitane 5% daily to try to reverse the gray hair on the sides of my hair, luckily I don't have gray hair on top of my hair. I'll tell you after six months if I notice any difference. I'm 27 years old.
Thank you and I look forward to hearing your progress. We definitely could use more evidence on this subject, even just case reports.
I don’t have any grey hair yet, but still very curious about your results. Keep us posted, if it works it’s something I’ll add to my hair arsenal in the future
Me too. Keep us posted
I'll keep you posted guys
What are the results after 6 months?
The gold standard of topical minoxidil and oral finasteride 1mg has halted my hair loss for about a year, but now it’s 2 years since starting and it’s started to progress again. I was thinking I should do topical fin at the same time of taking it orally, does that make sense?
@Haircafe i have a question, as someone who just recently started using finasteride: My libido has gone *way* up, and I love it. Is this a temporary side? I’m quite young, but my hair loss started early. Love the videos, and thanks for encouraging me to start the journey.
Fin may work like tongkat ali in some sense
It stops test from being converted into dht thus spiking your free test. Also how young? If you’re still in puberty reconsider since it could inhibit dick growth
@@2EyesI huh seems my comment disappeared. Anyway, idrc if my dick stops growing as I got lucky with my genes in that department. The only thing I’m worried about is facial hair, but it’s a price I’m willing to pay for my hair. I’m 18
@@lubb213 Facial hair in what sense? Are you worried it might stop facial hair growth or that it might expedite it?
@@2EyesI I’m worried it might stop it. I think Haircafe mentioned it in one of his videos directed at younger people, something about DHT being responsible for beard development (though not later on, when the beard follicles are developed)
Kévin you deserve 4 times more subscribers than BaldCafe ! BaldCafe is just like " just shave it bro " to young men still having a lot of hair (enough to be saved with finasteride and minoxidil at the same time). Yet he has more subscribers than you which I find very unfair !
Thanks God I found your channel before I actually found BaldCafe's channel ! Seriously I might just be bald today if I didn't find you channel Kevin so thank you for everything Kévin :).
Strangest part to me is that Baldcafe used min for his beard. But then using medication on your scalp (which is an FDA approved use) is suddenly being in denial or whatever, but using it for your beard (which AFAIK is not an FDA approved use) isn't? Like by his own logic shouldn't he just accept his own beard genetics? "Just shave the beard, bro"?
@@hj-hv6rt you got a point here . I couldn't agree more !!
@@hj-hv6rt He maybe responding to it on his face rather than scalp, that is a thing.
@@Tate525 Yeah minoxidil on its own obviously isn't a good long term treatment plan for male pattern hairloss anyway. Just strange to me that he's so dismissive of using medication to grow hair on your scalp ("you're in denial bro" etc)... when he's literally used medication to grow hair, just not on his scalp.
Another great video, Kevin. Any update on topical dutasteride?
Thanks, no new research from what I know and I haven't heard of any success stories from its use that have been documented with photographic evidence.
Hi kevin
Again awesome video. I have some questions i would be glad if you could answer them.
1. before i started fin i tried regaine for 1month but i already used like 3doeses ( supply for 3months) cuz im diffuse thinning too. Could this possibly ruin my gin gains?
2. Is it possible to use dut first for more regrowth and later just switch back to fin for maintenance?
3. What type of people didnt get any/ weak fin results? Where it like old people where alopecia already endend or young people with aggresive balding?
4. Around my temple i have many small hair will „vellus hair“ im currently 9motnhs on fin could those hair get back to be like normal strong hair?
Again thanks for encouraging people to start fin. Couldnt imagine how depressed i would be.
Thanks, happy to answer...
1. No, 1 month is too soon for anything.
2. Possibly, but not everybody gets more regrowth on dutasteride compared to finasteride. Dutasteride is a good option for people when finasteride isn't strong enough but absolutely nuking your DHT won't necessarily elicit more regrowth depending on how your androgenic alopecia has regressed. I have used both and they both work about the same for me.
3. People over 50 don't respond as well with finasteride due to less 5AR activity on the scalp. It still works well but the younger you use it the better. A lot of people claim that finasteride doesn't work for them but they're usually hypochondriacs who are freaking out about an initial shed and don't understand the hair growth cycle.
4. If they are tiny transparent hairs then I doubt they'll go terminal. If they still have some colour in them then yes it is possible they will go terminal and become stronger hairs. You will definitely at least stop things from getting worse if you use finasteride.
Happy to help!
When you think about it, Dio became a vampire to remain chad and not finish bald, ugly and bitter like his father. That's an understandable way to cope with a century not featuring finasteride...
True, I would've done the exact same thing in Dio's shoes, although I probably wouldn't have put the dog in the oven.
@@haircafekevin Vampire is not a realistic option for us. But still, we can work on making Jotaro hair-hat style fashionable in modern society! Best way to hide norwood pain until we get back to baseline haha

Hey Kevin thanks for another quality informative video I was just wondering will hairier people have a lower chance of side effects and are they more prone to the drug? thanks
No problem and no, there is no known marker for predicting the incidence of side effects on finasteride. It is a matter of trial and error.
Kevin, if someone can't use oral finasteride, is once daily topical the second best thing? Both for efficacy and for reduced side effects?
I wouldn't give up on oral finasteride. Try micro dosing 0.5 or 0.2 MG. Even 0.1 MG daily is a viable dose.
If oral is not something you can tolerate under any circumstances then topical might be a good choice.
I would also consider oral dutasteride as some people who don't respond well to finasteride respond better to dutasteride even though it is the more potent drug.
Hi there Kevin! What do you think about reducing body fat percent to mitigate the chance of getting sides like gyno on Fin? Would that contribute to a better hormonal profile more suitable for avoiding high estrogen/prolactin? I felt like I was getting gyno at 20% bf when I was taking oral fin (no other sides, and was even hornier if anything), now it's more like 13% and I have been taking topical fin 0.25 mg EOD/min 5% for about a month now, so far so good. Still shedding, no sides.
It's a good idea although keep in mind the mechanism behind what causes finasteride's side effects isn't fully understood. It might have to do with estrogen though so losing weight is a good idea regardless of whether or not you're on fin.
@@haircafekevin Kevin i am 22 years old.
Should i start at 22 years or should i wait some more years
Hey do you think theres a benefit of adding topical finasteride + oral, since the finasteride will fight dht on the scalp?
No, oral works locally anyways since dht is a paracrine hormone.
So it won't help since finasteride peaks at 1 mg per day.
i saw somebody say topical isn’t nearly as effective as oral, and then went on to recommend PRP instead. lol wtf
One key point of this study that I have not heard discussed is the carrier solution of the medication, which may affect penetration. The medication uses a type of chitosan, which is similar to a nail lacquer. Chitosan would have different penetration than solely using alcohol or propylene glycol.
They didn't reveal the precise formula for the solvent, maybe because the sponsoring drug companies want to keep it proprietary. I believe other studies on topical finasteride did not include chitosan in the solvent, so it's hard to know how important it is, but you may be right.
@@haircafekevin I believe the other Almirall Polichem studies by Caserini used Hydroxypropyl Chitosan. Are you going to try Winlevi off label, now that it is commercially available in the United States? It is not available outside the States so I cannot get a prescription.
Thank you very much for the video ❤️
I have a question can I use topical finasteride just once a day?
Could you make a video on how to style hair after starting something like finasteride and minoxidil ? I used hair gel before I developed androgen alopecia. When I started finasteride, I was bordering a 6head and was a diffuse thinner. Trying to manage my hair was kinda odd with the first few months of using finasteride because your hair and forehead are completely different then baseline. I know this is a vague topic but I don't see how a person with a Norwood 4 could pull off Thor's long locks in his first movie.

Hey man, great content once again. What about liver health though ? Not related to serum DHT etc but could taking the pill everyday damage the liver in the long term ? I'm no expert but I heard that some medications could do that. Maybe topical could avoid this ?
There have been slight liver enzyme elevations in some people on finasteride, but it has never been shown to report significant liver disease.
Yo kevin, do u know if there are theories whether taking fin in the morning is superior than taking it in the evening?
No, probably not. I wouldn't be surprised if there is speculation but nothing that is backed up by any evidence.
Anyone have the cliff notes/ main takeaways for those of us strapped for time?
Thank you Kevin! Today after taking my first Finasteride pill, I finally mustered up the courage to tell my wife's boyfriend to stop slapping my bald head. Unfortunately it didn't work and now I have to wear my laser cap while I watch them make love.
That's not cool. Your wife's boyfriend should at least have also gotten you a switch with smash bros so you can game while in the other room.
Hey Haircafe, im about a month into Finasteride, and experiencing some heavy shedding, will the hair that's shedding grow back?
Yes, it will. It is a sign the treatment is working and it will grow back stronger.
I love this dude!
hello kevin, what’s the least dose/frequency that we can get away with topical finasteride?
Hey choom. Is there any point using oral dutasteride along with topical minoxidil 5% and topical finasteride to block some extra scalp dht?
Hey Kevin. Disregarding the incidence of side effects and purely regarding the effectiveness of taking fin for hair loss: When trying to lower the amount of fin taken does research suggest it's better to lower the dosage of fin taken (say 1mg to 0.5mg) and still take one dose a day, or to maintain the same dosage and reduce the rate of intake down to every other day?
From studies that I've found it seems to be that taking fin every other day at a higher dose is more efficient as DHT levels don't begin to return to pre-dose values until after 2 days, and the higher single dosage itself has a greater effect on DHT then 2 days in a row of a lower dose, although I have yet to find a study that truely compares how different dosages stack up at different rates of intake and also measures hair regrowth not just DHT levels alone. Wondering if you've found any literature that points to a solid conclusion?
Even though the half life of finasteride is short the DHT suppressing effect is quite prolonged lasting at least a few days. So taking it every other day is certainly viable. The existing medical literature on finasteride is based on daily use but just based on the pharmacokinetics it is logical to assume finasteride works every other day at least at comparable level to daily use.
Great video Kevin! I've been on fin taking it day yes day no for like 3 months and I noticed my hairline started to recede faster and got worse (ofc I don't blame the drug) and I'm afraid of not getting it back because hairline is a bitch and time is crucial.
What do you think about adding homemade topical fin in order to crsuh the androgens in the scalp (that may be caused by the drop of dht), or maybe starting dut weekly?
Cause I don't want to compromise my hair on minoxidil yet
Do you have any thought in drinking a little of castor oil to increase pge2?
No, no need to stack oral with topical. They both absorb systemically and finasteride peaks at 1 MG. 3 months is way too soon anyways, give it at least a year and if you want to add anything I'd add minoxidil.
@@haircafekevin Thanks, I asked because as I kept receding quickly, I didn't know what else could I do in order to not make the recovery of the hair more uphill than it should
@@guille3622 If things really don't improve then the next logical step would be dutasteride. Again though, 3 months is way too soon. It often gets worse before it gets better. Check out my video on shedding.

Is this a good way to make topical finasteride? Just putting finasteride pills on a 60ml minoxidil bottle let them dissolve over the night and shake the bottle in the morning. I read that doing it that way it could not be effective because most of the pills is made of fillers and it could inhibit the effects of absorption. Can someone help me? Thanks!
Yes you need to strip the fillers just pour in pure powder
@@Tate525 So i just crush them and put the powder in the minoxidil bottle right?
Don't worry about the filler. The active ingredient is ethanol soluable so it will dissolve and like Skidrow said you can filter out the filler if it bothers you.
Very useful information, thank you!
You bet.
Do you anticipate that future topical AAs + oral finasteride will be significantly more effective at reversing miniaturization than finasteride alone?
Perhaps anecdotal cases online of RU + fin can inform this likelihood?
Yes, especially pyrilutamide.
If you get side effects from fina, stop. I just made a recovery from pfs recently and it took way too much time. I was on topical for 4 months, and it screwed me up. Just made a recovery after 8 months. I'm just gonna get a toupee if things get bad

Hey kevin!
Have u seen the study about nac for androgenic alopecia? It was done in 100 men and just came out. Maybe u can check it out if u havent. Its called: Efficacy and Tolerability of N-Acetyl-Cysteine for Treatment of The Early-onset Androgenetic Alopecia in Men
Why do we take 1/1.25 per day if .25 ED/EOD is (almost) as effective?
Perhaps that's a video idea (experimenting with maintenance doses)?
There are some people who respond better to the standard dose and if you're using Proscar it is pretty difficult to cut a pill to get less than 1.25 MG dosing. I use proscar because it is way cheaper than propecia for me so I stick with 1.25 MG daily doses.
@@haircafekevin Have you ever tried to see how little you could get away with and still maintain the locks? Say 1.25mg 4x, 3x or even twice per week?
@@sumsar0125 No, I have always stuck with the standard dose although plenty of people have used smaller and less frequent doses with success.

Hi Kevin , do you think the medication (fin+min in general) can affect how the hair looks? I am asking because before the treatment i had straight hair now it's really curly , i don't mind tho , i am just curious.
Not so much the texture but I do think minoxidil can darken hair. I talked about it in my Grey hair video.
Hey Kev, have you come across much research for topical dutasteride?
There are only a couple of studies I can see, and they're not very robust/extensive.
People on forums have been raving about it, with anecdotal reports of really low systemic absorption (even lower than finasteride). Allegedly, the size of the particles are too big to penetrate much further than the follicles, or something to that effect.
Thinking of trying it once a week with fin every other day.
Keep up the good work brother
Yeah, there isn't much data about it and most of the more promising research relates to mesotherapy.
CHeck out my video on it.

Hi ,
Can you comment about Beard Growth and DHT ... Will using Finasteride / DHT blocker orally or Topically on the scalp reduce beard growth ?
Will using a beard oil which boosts DHTs topically be a solution to this issue ?
Hey Kevin
Is mixing between topical (finasteride+minoxidil) and oral finasteride better than
using (oral finasteride with topical minoxidil )??
That is unknown as clearly there have been no experiments done using that formula. It’s best to talk to your doctor or dermatologist before doing that. Chances are though that the results will be the same either way.
You don’t appear to acknowledge or place great significance to the fact that plasma finasteride were orders of magnitudes lower in topical than oral. This has to be hugely important in terms of side effect profile and is ultimately the true measure of whether the drug goes systemic or not. The DHT plasma levels in my opinion are not a good indicator given that we know the scalp is hotspot for DHT production so Topical fin merely reducing 5ar activity in the scalp region can result in plasma DHT being lowered significantly. Also with respect to potential sides concerning neurosteroids again it must be considered that plasma fin levels are a much more relevant indicator, given that for neurosteroids to be affected fin must cross the blood brain barrier and that is surely positively correlated with the amount of fin circulating in the blood. Keen to get your thoughts on this, as many influencers seem to be brushing this under the carpet, almost because they’re on oral themselves and are therefore reluctant to endorse topical as it forces them to admit that they might unnecessarily be taking the riskier option. Always respect your thoughts and opinion.
I agree that there is less serum DHT suppression with topical vs oral finasteride. However it has never been shown that side effects correlate with DHT levels. Dutasteride which lowers serum DHT much more than finasteride does not have a higher incidence of sides, in fact in some studies the incidence is lower. Given how rare side effects are with oral finasteride despite lowering serum DHT on average 70%, it appears they are not due to the DHT lowering itself, instead they are due to changes in the testosterone/estrogen ratio. This ratio doesn't correlate with DHT suppression. In fact in over half of people side effects resolve over time with continued use of the drug, because the body resolves the T/E imbalance itself. Also I don't think that the finasteride drug levels have any meaning independent of the serum DHT effect. Finasteride doesn't have a significant effect on the type 1 5AR isoenzyme in humans, which is the isoenzyme responsible for neurosteroid production in the brain. So I don't think either oral or topical finasteride has any effects on brain neurosteroids.
Having said all that, I do think there are people who have sides on oral finasteride who do better with topical finasteride. I just don't recommend it as a first line treatment because we have more clinical studies on outcomes with oral versus topical. But I often recommend it to people who can't tolerate oral finasteride. However clearly there are some people who get sides even with topical finasteride, so it's not a panacea.
