Sir the date of form fill is not over yet there is time till next 5th march but today i am not able to fill the form because ... 1st step and after creating OTR profile is not able to submit ,again asking to fill up OTR profile. This incident is happening again and again... I don't understand why this problem is happening again and again... So is the form canceled??

obviously date of issue will always be 2012, if you write 2024 your application will get rejected

Insert any date after February 5th, their meaning is that when you apply, your photograph should not be more than 10 days old
