    The state of New York has approved the use of one ventilator for multiple patients during a surge or crisis. Dr. Seheult discusses some of the implications and challenges of this practice.     My biggest revelation after years of sleep problems, sleep doctors, & pills was this: to sleep well, first *remove* the obstacles to sleep. Most suggestions focus on what to *DO.* I found the key was to remove what I should not do. For me that was 2 hours before bed avoiding things such as any tv, engaging intellectual discussion (I'm easily excitable), problem solving (my passion), loud music, bright light. When I let my body settle down into a calm restful state, sleep was unimpeded by all these antagonistic burdens. I use an alarm on my phone 2 hours before bed which reminds me its time to start winding down for the day.
    I hope these ideas can help other people too.

      Thanks for some great information. I have been an insomniac for my entire life. It was a sudden acclimation at age 18. My mother was the worst insomniac I have ever seen. My sister and brothers “inherited” the disease at aboutthe same age. When I married and had children, I swore to myself that they would not get this same malady. Unfortunately, as all four turned about 18-20, each one became a serious insomniac. Throughout the years, I have relied on many sleep medications and programs to try and get at least 3-4 hours of rest a night. I firmly believe after years of research that being sleep deprived is an inherited disease. The doctors doing the research are not the ones that have insomnia. None of my doctors could appreciate what it’s like going for weeks without sleep. Finally, your body gets to the point to where it just crashes for 3-4 hours. This disease has been the bane of my life!

      I find success in avoiding news or data on COVID-19 several hours before bed. Obvious I know but I was waking up at 3 am and looking at worldometers which was not helpful.

      Here are some additional sleep hygiene tips from a former insomniac: Don't eat for at least 2 hours prior to bed( 3 hours is ideal). Exercise will help you sleep better at night and give you more mental clarity and energy. If you exercise in the evening just don't do it right before bed. See BrightSide video's on CZcams for lots of great ideas since we need to be working out in our home environments. Don't underestimate just getting outside and going for a walk. This is also beneficial in many ways. Just wave your neighbors and keep your social distancing in mind.

      @@Medcram It's great that you have brought attention to the need for quality sleep however from my experience in hospitals it's almost impossible to get good quality sleep due to lights being on and continual disturbances. Is sleep that is induced by medication still able to provide the same benefits or as with alcohol induced sleep, it does not provide the quality sleep?

    Dr. Seheult, you are more precious to our mankind than the world leaders. I personally don’t feel comfortable that you are working in the front line. I’m sure many viewers feel the same. The famous biochemist Otto Warburg who joined the fight of WWI and later was persuaded by Einstein that he can serve mankind more by not risking his life in the war and continue to be a scientist. I implore you to dedicate fully to educating the public and leave the front line to those who don’t possess the talent for educating the public. The world desperately need you to save countless lives by spreading the knowledge and the truth.

      While I agree with your logic, I somewhat doubt that Dr. Seheult would be able to bring himself to do that. This crisis is showing just how dedicated the vast majority of doctors, nurses and health workers are to help others. When we finally emerge from this crisis, I hope most people remember just how much they have given of themselves.

    As a Respiratory Therapist for 40 years, I completely agree with the AARC, in their opinion of using one ventilator for 2 or more patients. There are way too many variables in between patients needing ventilator support to be able each patient. One variable, such as lung compliance, for example, would affect both patients, possibly causing more volume, pressure to one patient, and less to the other. Both patients could be injured, compromised in this situation. Yes, I’ve have heard this method discussed, but never used. I think it unwise to consider this as a viable option.

      Hearing you, but as an alternative to death? Hm.
      I would hope that patients with similar needs would be the ones sharing a given ventilator, for a start.....and too, some of the experimental mechanical designs could well be expanded or 'ganged' to deal with the variations between individual patients....
      Not an RT, but hoping that the disadvantages get dealt with and more people survive....

      M &M agree, RRT here.....this is definitely not recommended. I would however, be willing to use for two patients if I needed to. This whole thing really shines a spotlight on some major equipment shortages. I hope when this is done, we manufacture enough supplies to never be in this position again.

    Dr. Seheult, I’ve been following your channel since mid-January. I have referenced your videos many times when people would ask my opinion then two weeks later what you said would be in the mainstream news. People would come back to me and say “Wow! How’d you know that was going to happen or how’d you know about . . . After a minute of pretending I have medical visions, I remind them of MedCram. I’ve referred friends with anxiety who want to know what’s going on without the theatrics of 24-hour news reports. You have become a trustworthy and rational source for so many.     Anecdotally - Dr. Li, theWuhan ophthalmologist who was the “Canary In The Coal Mine” for the COVID-19 outbreak in Wuhan, had reportedly worked non-stop for almost three weeks before being hospitalized and overcome by this virus. Many have characterized this as a classical case of “working one’s self to death”. Apparently (as we have seen especially in Italy and now Spain) he was not alone.

      @@Medcram By 'sleep efficiency,' I take it you mean sleeping soundly rather than fitfully? If I routinely wake around 1am and again around 3am, what can be done about this?       @@Avital4414 I think in one of his previous videos about sleep, he mentioned something about the first few hours of sleep is the most important for immune function. Additionally, multiphasic sleep patterns have a decent amount of credibility as being just healthful as monophasic sleep as long as you are getting enough throughout the day.

      @@Medcram I am a critical care nurse working in a dedicated COVID-19 ICU in Italy... it's hard but we keep it up!
      Can you help me in finding data about infected medical workers and the number of deaths? In Italy I found newspaper articles reporting 45-46 physicians deceased... but what about nurses and other healthcare professionals?

      Well yeah, so don't worry about it. Give it 9 hours of your schedule. 1 hr to wind down, 8 to sleep. No screens at night, sunlight during the day. Bam.

    I've always mentioned when I get sick, I usually can sleep it off. And with lack of sleep I'm more likely to catch a cold or flu. This video just explained it's true. I'm going back to bed now.

    Most people are presently struggling to sleep properly because they are worried about their health and their loved ones' health as well as loss of income etc due to the pandemic.     Great information. Number one thing I do when sick is sleep and rest. Way above everything else. Sleep is the number way to fight off sickness. Your body seems to really work hard to fix the body when sleeping.

    Love your work. I had CMV a few years ago. I slept all day every day until I recovered. 3 or 4 days. My doctor was surprised with my liver function test a weel later. These videos are doing the world a great service. Thank you.

    Thank you again! Your earlier videos on this (updates 16-18) got me to take my sleep seriously. I started taking a hefty Melatonin tablet (not toxic or habit-forming) after getting my disabled husband to bed every night. Now by the time I get the chores done and catch up on maybe only one or two things online (instead of 19 or 20!) I am actually yawning and sleepy instead of wired, helps me TURN OFF the news and your much-appreciated updates and go to bed! WAY better to get up rested in the morning and have a whole day of sunlight, than to oversleep and have only part of a day (as in the past when I didn't have to get up early to take care of my husband) or now that I have to get up early, to get up sleep-deprived, feel like crap all day, AND be more susceptible to Covid-19. Having enough sleep also seems to be better for my state of mind in these nervous-making times. Even if the melatonin stops working, I am determined not to go back to my old patterns, I do not (usually) suffer from insomnia if I can just make myself adhere to a bedtime - it's this childish stubborn refusal to go to bed that keeps me sleep-deprived.

      He went into even more about it in updates 16-18, pretty sure there were other immune response pathways as well that he showed had been proven to be helped by sleep and harmed by sleep deprivation. Totally made me change my sleep habits!

    I would like to thank you for all the time you spend trying to enlighten the general public on what is being developed to fight this pandemic. I’m 70 and started working when I was 14. I can’t thank You enough. I look forward to reading every article. My daughter is a RN in California in Nic U they were reusing face masks because none where available. This was at a Top hospital. I guess no one saw this coming. While I’m at it Thank God for all the Doctors and Nurses that are risking their lives for (strangers) the general public . The real hero’s.

      Hahaha same with the serotonin, tho im taking SSRI's... The thing is, do ssri's affect us in a negative way when fighting the infection? Bc they probably increase setoronin plasma levels...

    This is a fascinating study! It also suggests a cellular mechanism (as briefly noted in the conclusion) for the observed diminished immunity in chronic anxiety/stress states in which individuals have elevated endogenous catecholamines. Bless our health care workers who are both sleep deprived and stressed during this demanding time.

      In America it will probably be determined by health insurance rather than who has a better chance of surviving. I wouldn’t be surprised if deaths outnumbered recovery because of this.

      So. Why, when we know this, are physicians still having to go through days of no sleep for their educational certificates? To me, it's common sense, but apparently it's had to be proven. Maybe it'll change now.

    So far what I have learned - please correct me if I got anything wrong - :
    1- fever helps the body fighting against viruses.
    2- sleeping at least 8 hours a day improves immunity.
    3- in case of people on ventilators, turning patients on their stomach reduces mortality rate (prone positioning).

      @Random Stranger On The Internet the bat virus doesn't have the component that allows it to connect to human cells.
      So the virus obviously mutated parts of the "capsule" and proteins on it to be able to connect and enter human cells.
      In any case, we don't have the same virus as the bats
      There is something else.
      When your body reaches a fever say 38°C, a few degrees higher than normal body temperature. It's not just the temperature alone that kills the virus. Or it would not survive in aces that are hot right now.
      It's because the higher temperature in the body, triggers the body to up it's immune response, a few enzymes and messages and proteins like those that signal that cells need to die are released in higher amount.
      So it's a bit more complicated than temperature alone

    Interesting to see this work with patients having different demands from VT to RR. Patients would need to be within the same ideal weight but sharing infection as well.

    Medcram. Thank you for what you do. I work as a Respiratory Therapist at a UC hospital in California. I really believe it is time to start using APRV (airway pressure release ventilation) to treat ARDS. It has not been widely accepted in the pulmonary critical care community as far as I know. Dr Habachi has had great success with APRV. I have herd any talk about this but it would be a good study. Im going to look into finding a pulmonologist champion to possibly do a study of "early APRV for intubated Covid-19 patient" china did a human APRV trial in 2017. Could you possibly help me out with creating a study like this. Hope to hear from you. Thank you.

      Wishing you a speedy recovery. I also have a few vague symptoms. It's very scary knowing that so many people have very vague symptoms. This site suggested vitamins d3 zinc and vitamin c. Also a commenter added that k2 and b6 can help. I stocked up. K2 was hard to find.. Found it in the gnc section of my local rite aid. None of the other brands seemed to have it. Also to avoid ibuprofen

      I got really sick the end of February. Cough, fever, complete loss of smell, felt like breathing was “work “. I just assumed I had the flu and the breathing was anxiety. I’m not so sure now.....oh and not to be gross but thick sputum that was like glue and it would literally get stuck in my throat 🤷‍♀️I’ve NEVER lost the ability to smell. I’m a 55 year old nurse with an autoimmune disease.

    I think ventilating patients like this is taking a huge risk to all of the patients hooked up to the vent!!!!!! Sometimes it’s hard to just manage one patient on a vent and can’t imagine doing it for 4!!!!!!!!
    Sleep is so important because before I got off working night shifts I would get sick a couple of times a year! Now I have had one mild sore throat in December and that was it!!!!

      We unfortunately don’t have the luxury of being able to ventilate every patient with their own ventilator nor are we able to dedicate a staff member to stand by their bed 24/7 to bag them. These patients are also requiring very high pressures to ventilate their lungs with a need for precise small volume breaths and we just cannot do that with a traditional manual bag valve ventilator. The alternative is death and many hospitals and staff have created protocols to be able to ventilate these patients with one ventilator in the safest manner possible. We will optimize what we have, we will learn and we can save lives because the alternative is not acceptable.

    I’m going to be honest I can often sleep 😴 more than 4-5 hours. This is attributed to decades of working variable shift work all the way back to when I was in the U.S. Military. So my body learned a different sleeping habit where I’m often a Nightowl even at home. I rarely go to bed before Midnight and awake by 5 AM, often beating the alarm clock. Even when I try to sleep late on a weekend or on vacation time off my body automatically wakes me up by habit and when I check I the time it’s only been 4-5 hours of sleep. Unless I’m drop dead tired as can be maybe just maybe I get 7-9 hours of sleep and when I wake up my body is aching and sore all over. Still I appreciate this video in still showing the power of sleep and why we all need sleep for body repair and to fight infections and boost immunity. This was well explained video and for people that aren’t even medical personnel.
