  • Dr. Ebraheim’s educational animated video describes Cervical Spine Nerve Root examination and its indications.
    Neck pain and shoulder pain or referred pain to the scapula may overlap. Shoulder pain tends to be located on the postero-lateral aspect of the deltoid muscle. Neck pain tends to be localized over the trapezius muscles and may radiate to the upper extremity. Radicular pain from irritation of a nerve root in the cervical spine is associated with paresthesia, numbness or weakness in the dermatomal distribution. The combination of shoulder pain and neck pathology is not uncommon.
    •Herniated C4-C5 bulge: motor function: C5 shoulder abduction, C5 elbow flexion. Deltoid C5, biceps C5. Sensory C5. Biceps reflex is primarily C5.
    •Herniated disc C5-C6 bulge: motor function: C6 elbow flexion, C6 wrist extension (extensor carpi radialis longus and brevis), biceps C6, wrist extensors C6. Sensory: thumb and radial hand. C6 brachioradialis reflex.
    •Herniated disc C6-C7 bulge: C7 elbow extension, C7 wrist flexion, finger extension, triceps C7, wrist flexors C7. Sensory C7. C7 triceps reflex.
    •Herniated disc C7-T1 bulge: C8 finger flexion (flexor digitorum superficialis).
    •Herniated disc T1-T2 bulge: T1 abduction of the fingers (palmar interossei muscles (ulnar)). T1 sensory.
    Test for cervical radiculopathy:
    The Spurling's test is used to assess nerve root pain. The patient should be seated with the head turned towards the affected side and the clinician standing behind the patient. downward compressive force to the top of the patient’s head is applied. The test is positive when the compressive force being placed on the cervical spine causes radiating pain down the patient’s arm.
    Shoulder abduction test: the patient’s symptoms are relieved by shoulder abduction and placing the hand over the head. This test helps to differentiate between cervical spine pathology and other causes of shoulder pain. It is an important test for the cervical radicular compressive disease. This is also important in regards to exam questions. The relief of the symptoms occurs due to decreased tension on the nerve roots.

    These are wonderful videos, I broke C! and C2 in a bad mountain bike crash and have a fusion with 5mm ti rods from C3 to scull base. I managed to get up
    walk down steep mountain at Rocky Peak , Simi Valley, CA to ride in a pickup truck to hospital. I got up from intensive care and was able to walk out of the hospital
    4 days later. God saved my life that day and Dr Gill my neuro surgeon says that I am 1% and maybe 1% of 1%. I am living proof miracles can happen so grateful to God.

    I had carpal tunnel surgery which after 3 months my tendons are popping and hand is sore. What I see here is C6/C7 disc compression was causing my guitar hand finger numbness. I'm afraid I may have made the wrong decision to have carpal tunnel surgery without a C7 MRI. But Drs read the Nerve tests and the nerve analysis which said C7 and Carpal Tunnel indicated. So I went with the neurologist summary and the hand surgeon agreed without a C7 MRI. Every DR is specialized and willing to do surgery but not look for other causes. That is why I switched Drs to a DO as primary physician. I hope my guitar hand comes back 100% but it looks like it may be a long road. Rotator cuff DR is next I have MRI and I have partial tear.

    Hi Dr. Ebraheim, thank you for the great video. ( You have some confusion at 3:54 where you are showing the palmar interossei and seem to imply that they are responsible for abduction. As you know, the palmar interossei are responsible for adduction and it is the dorsal interossei that are responsible for abduction.)

    I do not have pain but I have Neuropathy of my hands and fingers. only. I did massage for 33 years. I can't sleep on my right side only my left. So I assume nerve flow is the C5,6,7,8 and T1 are compressing the nerves in that area.

    Boa noite! Sofri um acidente à 10 meses atrás onde tive emulsão de c7,c8 e t1,mas com 4 meses de acidente fui submetida a uma cirurgia de transferência nervosa. Hoje estou com 6 meses operada será que tenho alguma melhora na mão??

    The doctor recently did some of these tests on my left side. He used the reflex hammer thing, but no reaction/jolt was produced. Why was there no jolt?
    I do currently have a neck injury that has been there for close to a year, and currently have an ache that radiates down into my thumb and the neck pain increases when I twinge it sometimes.

    I have tinitus, left side, my x ray shows, cervical SPONDLOYS, e. N. T. Dr said that ears are perfect, now I have tinitus, neck pain shoulder pain, ear burning on sides, some times eye flatters, wrist, fingers pain, low back pain, knees and feet pain,
    And also, tinitus on forward neck bending
    Now, suggest, that my which nerve is pinched in neck, and which tests should I perform, because vit. C, vit d, traction, stimulation, suggested by cervical dr is not working, or working but for some days, and not working in tinitus

    Does a c5- c6 herniation disk can cause chest pain sometimes? My pain is in my lert inner bicep, under arm(axila) back, forearm and wrist...it feels heavy, tingling, numb and I feel like electrical shocks simetimes...it is weak as well.

      @michaelflores9796 I still have that, I am thinking it is a slipped rib syndrome, the pain is awful and all my labs, x-rays, CT scans, MRIs are normal, don't know what could it be.

      @michaelflores9796 Yes, I'm seeing my doctor to see if they can diagnose the slipping rib syndrome because my body now is crooked, my right shoulder is higher than the left, I think I have a lot more than a herniation. I hope you find the medical care you need soon! 🙏

      I suppose it really depends who you ask................Most conventional Dr's I have ever talked to about my spine do not like Chiropractors, but hey will refer you to physio's all day long. I don't think conventional Dr's really understand what they do, IMO, yet they (chiro's) have helped me the most out of anyone I have ever seen, over the longest time. There are some physio's that can do very similar things to what a Chiro can do but they are few and far between. The interesting thing is that my chronic pain specialist says to do what ever helps me............. and believe me I do. I use anything that works, out of any camp. Not a fan of surgery, I have managed to put cervical
      eurosurgery off for over 3 decades. 2 weeks ago I finally had neurosurgery and honestly wish I had, had it done sooner. I'm not even sure which of the 3 surgeries I signed off on, I ended up having as yet.

      They are not all bad. I put off cervical neurosurgery for over 3 decades because I simply didn't feel comfortable with any of the surgeons, I came across. I eventually had a big neurosurgery a fortnight ago. I couldn't have asked for better care or treatment and had a surprising little pain afterwards and far more flexibility than I had before hand. Can't wait to see if I can swim properly in a month or so's time.
