  • Care after a hair transplant with medication is the topic for this video. As you may know, medication after a hair transplant is provided for one of the following reasons-
    1. Infection after hair transplant

    Hair transplantation - Minoxidil
    Question: I have Norwood 2. I have been taking Finasteride for a year, but unfortunately, I have not seen much improvement. Initially, I only took 1mg three times a week, but now I take 1.25mg daily. It hasn't gotten much worse, but it hasn't gotten any better either. I have a hair transplant appointment in October. Im thinking of going on Minoxidil and my doctor said I could try it out. However, I read on the internet that Minoxidil should be stopped for about 7 days before and after the hair transplant. My question is, should I start using Minoxidil now and stop using it for two weeks in 5 months because of the hair transplant? Or should I wait until after my hair transplant to start using Minoxidil? On the one hand, I want to stop my hair loss and promote hair growth as soon as possible, but on the other hand, I am afraid that the back-and-forth use of Minoxidil could do more harm than good. Or is stopping Minoxidil for 2 weeks not a big deal? Thank you in advance.
    To clarify, my concern is that if I start using Minoxidil now to achieve fuller hair, the doctor may use weakened hair from my donor area that has been treated with Minoxidil for the hair transplant, resulting in potential hair loss at the front in 3-6 years. Therefore, I wonder if would be more prudent to wait until after the hair transplant to ensure that the implanted hair is strong and stable before beginning Minoxidil use. How long should I wait after the hair transplant before using Minoxidil?

      Just 10-20 mg a day to start with and then weaned off over 5 days. Only for 5 days is the medicine given.

    Hello dr Bhatti, thank you for making these amazing videos, along with all the medicines, do you think prp sessions are a compulsory after a hair transplant ? Without prp would the grafts grow healthy and can we obtain a great result along with medications ? Would you recommend prp to your clients post op ?

      And thanks for subscribing to my channel. I do not subscribe to the belief that PRP does any good after a hair transplant. It is not a DHT blocker. It is neither an alternative to Finasteride nor is it of any use with Finasteride. Finasteride + Minoxidil combination is all you need after a hair transplant.
      I only use PRP in 2 instances- (a) to sore grafts alongwith A-Cell during my In vivo preservation technique which is part and parcel of my 'Anagen +' technique and (2) when a patient wants me to.

    Ive watched so many videos of yours and you keep repeating the importance of finasteride for the first 6 months post op referring to it as „umbrella of protection“ I think. On the other hand you say if the harvested hair is from the absolute safe zone, the transplanted hair will last a life time - why the finasteride then?! Could you please elaborate on this? Thx in advance!

      Thanks for watching the videos. Finasteride is required for a long period of time till the baldness gene switches off in those with aggressive baldness esp. if they happen to be young. If your baldness has stabilised - and this happens after 35 years of age, Finasteride is not needed. There is no 'permanent' scalp donor. Like all other body organs, hair also continues to deteriorate with age. And this is more pronounced if you carry the MPB gene.

    How you Doc! A question. 14-18 days after procedure, the crusts fall off.. Now, we start on 2% minoxidil and if it stays good to skin we switch to 5%. How do you apply this? - Procedure of application of this foam? Like, we cover the scalp(donor/receptor) with foam, leave it for few minutes and then rinse it away? or just apply and leave it and not rinse it at all? is it twice a day?

      Minoxidil has to be left on the skin and washed after 3-6 hours.

    I've a quick question doctor. Its my 11 day of HT. My doc has prescribed me finesteride & biotin tablets and neurotrat as well. Can I go for growth factor concentrate or prp as my pre existing hair is also thin. Please educate me on this. And yes one more thing! What's your take on genetic baldness. Can It be stopped?

      Your doctor is right in suggesting what he did. PRP does not help in baldness since it is not a DHT blocker however it decreasesthe resting phase of the natural hair cycle making more hair appear on the scalp at any given time. For your last question: czcams.com/video/jLQTzLM5-8c/video.html

      Traction alopecia is not due to androgenetic alopecia or FPB. hence Finasteride will not help. Minoxidil will.

    Hi Sir,It was a wonderful video.Very Informative.I am a scarring alopecia (Lichen Plano pilaris) candidate where my condition is stabilized now and I have done my hair transplant just before 2 days.
    Can I use APREMILAST,Follihair,Minokem n 5%(Minoxidil,Azelaic Acid,Tretinoid) or would it better to change medication especially from Minokem to plain Minoxidil?

    Would dutastride be more beneficial after hair transplant compared to finastride. Or can you combine the both, so that you can you finastride in week days and dutastride on weekends ?

      Dustasteride has 17 % incidence of side effects as compared to Finasteride.

      Some people have sensitive skin others do not wish their hair to be oily all the time. These people opt for oral Minoxidil.

    If you're at a Norwood 2 stage with some thinning in your donor area, and you've been taking Finasteride for 2 years, and recently had a hair transplant, should you consider using topical minoxidil for at least 6 months, or even longer? If you stop using it after 6 months, could your existing native hair fall out or become super thin, potentially making your situation worse than if you hadn't used minoxidil at all?

      Stopping Minoxidil will only take your hair back to where they were when you started. Not much worse.

      Only Finasteride is a 5 alpha reductase blocker. Rest are only adjuncts and not alternatives to Finasteride.

    Sir . Thanks for this video . I have had transplanted my hair five and half months back . I had 7 grade baldness and 8200 hair had been grafted . While I use shampoo or minixodil I find 7-8 thin hairs in my palm , Is it normal after five month of operation and after 5 months of use of minioxidil ?

      Hair grow and fall in cycles. This is normal even with transplanted hairs. These are living tissues after all.

      People are responding to it. But it not being a DHT blocker, baldness progression will contiue. Not an alternative to Finasteride.

    1.My transplanted hair started falling once i stopped topical minoxidil.its been 7months.Will it grow back if i start minoxidil again?
    2.Can i use it on alternate days ?..will it double the time of hair growth ??....or is it that we should use it daily if we hv to see the results???

      If hairs are still thin and you are continuing Finasteride in the long term, you can stop Minoxidil.

      Minoxidil alone will not benefit much. But the growth phase is still on and your starting Minoxidil will do benefit.

      You can use aloe vera once a week or twice a week. if you are past 3 months after hair transplant and Minoxidil is causing a lot of dryness and flakiness of the skin, you can stop it and continue only Finasteride, since Minoxidil id not a DHT blocker.

      Follow your doctor's advice since he has seen you in person. Your hair might not be permanent in the donor zone. But rather than Minoxidil, Finasteride is more useful in such situations.

    Can I ask to be put on a pain med like oxycodone during the surgery? I have surgery coming up and I already take anti anxiety meds and am very nervous to be sitting for hours for 2-3 days straight I really want to be as out of it as possible during the surgery.... am I aloud to ask for that?

      Grade 7 with slick baldness will not benefit since hair roots have gone. If you have sufficient thinning hair it will have a good effect. Other vitamins/ supplements have no effect on hair loss when genetics are at play.

    Post transplant have used dutasteride for a month and switched to finastride basen on dr. Advise because I was having low sex drive, erection problem, but even after using finastride for 2 months same problem as before lox sex libido, and no erection. So after using for 3 months since my transplant so would stopping the medicine. Moreover finastride affect lipid profile like it raises triglycerides... Before transplant triglycerides were 152 and after 3 months it is 192 which is high at borderline. It is the same food patterns have using before and now. Please advise will my hair still grow by stopping the finastride only .

      If your hair loss has stabilised, stopping Finasteride will not affect. If it is still progressive, you will reach your destined baldness grade as encrypted in your genetics.

      Minoxidil is started after crusts have fallen off at 2 weeks after hair transplant.
      We recommend 2% Minoxidil for 2 weeks since I wish to be sure there will be no skin reaction to it since skin is sensitive immediately after surgery. If there is no reaction , I ask the patient to switch to 5% Minoxidil which is continued once a day till 6 months are over after the hair transplant.
      It is however the patient's choice if he wants to continue to use it thereafter.

      There is no alternative. However many have got a good result without it. But it's like taking a chance.
      Finasteride is not at all as bad as you think it is.

      I do not use medications that do not have long term established utility on my patients since do not want to risk their hair in experimentation. 10% Minoxidil is of no use. Beyond 5% M, the benefits do not increase, only the side effects do.

      This questions cannot be answered since the answer will differ from person to person. The scalp donor deteriorates with age in some people. In these people the transplanted hairs with age will begin to thin too, necessitating DHT cover.

      Some shedding does take place for transplanted hairs too, but is a fraction of what happens to the native hair if they were thinning before Minoxidil was started.

    Hello Doctor . Recently i saw your excellent and well educated video on youtube regarding minoxidil and its safe usage. I have taken lotinen 10mg half a pill each day for 5 months and approximately a month ago I had a hair transplant FUE procedure . Due to the procedure my doctor told me to stop the usage of loniten as it is no good for the hair transplant . So my whole scalp turn into sparse and thin . I mean i can see through my eyes the scalp clearly at some spots . What do you think i should do?

    That's really helpful video for me sir.. I did transplantation 2days before my donor area was good and the extracted grafts are taken from beard and back area of head.. Sir I have doubt weather my result will come good because doctor told me that they have transplanted 6700grafts..

    It's been 18 days i think I am having a pain on my donor area due to bacterial infections which painful to touch feels like bumps like type of pimple not round in shape very ful to touch all over my scalp mostly to the donor area, can't rest my backneck and head due to inflation and pain what medicine I should use I think , I have used cipro and Amoxicillin not better I think resistance to antibiotic what can I take the best linezoild ?

      Dr Reddy's and CIPLA are good brands. 1mg a day for 7 days for 3 months and then take a 15 day break. 5% Minoxidil application at night - wash off in the morning. twice a day will be messy and defeats the purpose of good looking mane.

      Imran ji, Minoxidil is not a DHT blocker. It is a stimulant. It is likely that with time the body decreases its response to Minoxidil like most other drugs.

      Thanks for the comment. Your doctor must have ruled out Finastreide after consulting with you and thinking you to be a high risk person for side effects of Finasteride. You should discuss this with him if this is not the case. But follow your doctor's advice- it is in your best interest.

    2. Biotin
    3. Genesis tablets
    But i am not using any because i have some other health problems..
    Will it be affecting tge growth of my hairs that has been planted ? Or these meducines are just to speed up the process ?

      This question cannot be answered without seeing pictures. The answer will be different for different donor hair, for different age groups and levels of balding. However, one thing is common- not taking Finasteride does not allow control over normal growth of planted grafts.

    Doctor, I'm having excessive irritation, inflammation and itching of the scalp in the recepient area and the FUT donor area. My scalp has become overly sensitive after the surgery and I had extreme hair loss in the crown area(not the recepient area) after applying minoxadil. Should I try oral or topical corticosteroids? I have always been able to tolerate minoxadil (foam, 5%) with occasional irritation but since the surgery everytime I apply minoxadil my scalp starts burning and itching. What should I do?

      After the crusts fall off, to be safe (since skin is tender), start with 2% Minoxidil. After 2 weeks switch to 5% Minoxidil. If at any stage you have excessive reddisness, oozing of fluid or swelling, contact your doctor. Typically Minoxidil is needed for 6 months post hair transplant.

    Hello Sir g,i am a Gp doctor in mid thirties and have been suffering from hair loss for many years,my only issue has been Finasteride,i started the drug once after my dermatologist batch mate suggested it's an extremely safe drug but i had sexual problems,i couldn't believe i was amongst the unfortunate 1%,but sadly that was the case,i had erectile trouble and so stopped the medication(most probably wasn't a placebo,but i am just human,so i can't be 100 % sure),then i saw a hair loss channel on youtube by two Australian doctors and they suggested taking Finasteride 3 mg /week instead of 7 mg per week or even 1.5 mg per week in extreme cases of higer sensitivity to the drug,as latest studies have proven,it is a drug with diminishing returns and it builds up in the body and 30 mg per week might be an over kill for a very less benefit.
    Though my side effects discontinued a week or so after i left taking the drug,i am afraid about giving it another try and if i do opt for a hair transplant and if this medicine is compulsory after a transplant,can microdosing finasteride be an option or even topical finasteride?

      Dear Dr Raj Verma many patients have had good results without Finasteride. But it is a chance you will be taking. Finasteride is absolutely essential to have a good result to my mind.
