Best to minimize elevator use but you have to use it to SIE extent if it’s submerged. So, you should keep it to a minimum but you still have to remove the tooth. So, you have to take some chances.
Dr,. Nasseh do you recommend restorations with cuspal protection always after endo? Watching Dr Restrepo follow ups that question comes to my mind since he doesn't seem to be worried about it. In his cases, the teeth he transplanted have adjacent teeth and good contact points but for what I've read, it doesn't matter if your access cavity is contracted, it would be better to protect those teeth with indirect restorations since the survival rate is higher for those teeth with cuspal protection at the 5 years mark.
There are a lot of opinions circulating around that topic; frankly, while a full crown will likely last longer than a filling it’s much more aggressive in tooth structure removal and more expensive. Recommendations such as a crown for every single tooth after endo, are just recommendations, and recommendations work best when people can’t make their own decisions.
Sir, i sent a cake to your clinic a few days ago as a gesture of thanks for sharing your knowledge. i sent an email that if u had received it, i didn't get an answer back Please do confirm with me on that if you can. Dr. shaheer ahmed Postgraduate trainee Pakistan

The extraction of the third molars not easy task.
I guess very skillful surgeon.
Topic of extraction itself will be appreciated.
Thank you!
Yes I agree… I’ll have to ask him.
Great case sir! Thanks for sharing such a great knowledge 🎉
Hi doc, aren’t the PDLs getting damaged during extraction with elevator, or he is using something else, thanks

Brilliant 2nd case - would PRF increase success?
Interesting point. I’m not sure it has been studied in this specific application of autotransplantation. But it would be a good project to find out.
Is there a doctor in Mexico or US you can refer me to?
Hi doctor, is there a risk of creating a ledge if you start with a large iso 30 rotary file without using hand files and creating a glidepath??
Yes there is. But not if you don’t engage it beyond light touching.

Thank you but we did not receive any cakes at the office. But I greatly appreciate the gesture. It’s the thought that counts. 🙏
