It looks like the index changes when you switch from grinding primary to secondary relief. This is the reason for the secondary relief facets to look like triangles rather than rectangles they should be.
You may need to have additional stop to adjust the index for the secondary relief grid.

#1 is the hole clean?
#2 if clean, I wonder if you can indicate the hole wall to see if it is such a grind can improve hole size accuracy and position.
Where did you find the 5C collet attachment? I want to get one for my grinder than uses long skinny who knows collets.
you have a angles issues. you said you set the griinder for a 118° but by eye I can say your tip angle after sharpening look like 135°.
second problemn the cutting edge isn"t parallele to the secondary facet edge.
this such drill bit for stainless style have a thick web and need pliting point for thining the web.
Optima drill grinder does this same thing supper easy
