Don't forget that you can also get a spreadsheet/excel sheet with your results with a time stamp as well. Just click on the green plus sign on the upper right corner when you are looking at your responses. This will create an automatic spreadsheet

Can you do this for multiple goals with various categories?

This is great! I am trying to find out how to put each student into a folder (I push in to 7 teachers as an ESE teacher to take data for the ESE kids in Gen Ed.) I want to be able to click on teacher class and all the student forms to fill in data and also allow only that teacher to fill in also as I'm not in their room everyday.. How would I do that? I would greatly appreciate any help with that.

So simple! How do you track each objective on one sheet? How can I copy this for each child? For example, if I have 5 students using the same goal do I have to create it all over again 5xs?

My question too! Each of my kids have a goal and then 3-6 objectives to measure

Not sure if you found out, but you can import the question that is repeated.

Do you create a new form every quarter?
