When Should I Replace The Other Knee Or Hip?

  • Patients who are having a knee, hip, shoulder or other joint replaced often ask if and when they should have the other side replaced. In this video the surgeons discuss the timing of second sided surgery. They recommend 3 months between surgeries and they discuss their reasoning. Reasons include time to recover, time for blood levels to return and time to see if the other side gets better without surgery.

    I am on the 4 week countdown for my right knee replacement . Been a nurse for 20 years and prior to that I worked in a lumbar yard. Can't even walk around the block. Excited and nervous about the new knee. Just wanted to say your videos have been really helpful. Thank you for taking the time to produce these.

    Good evening Docs. I hope you both are well. Dr. Weening, you did my left knee replacement between 9-10 months ago. I took me about 5 months to recover.....a bit more time than most. I did the therapy at the hospital and continued the exercising at home. I'm still doing the exercises every day and it's helped me a great deal. My wife and I live in a 3 storey condo and we live on the top floor with no elevators. I was still using the cane to get up and down the stairs, until the beginning of November. I'm able to do the stairs without the cane, now. My right knee is still good....I get the odd bit of pain in it, but nothing too serious.
    Thanks to you, I'm feeling so much better. I don't know what took me so long to get it looked at, but I'm certainly glad I did and that you were the one who did the surgery. I'm in no pain now and I just want to thank-you for being my surgeon. You're a great surgeon and your bedside manner is impeccable. Steve

    Thank you both! Just had my TKR done about 10 days ago and doing ok. Had it done in day surgery and was home that night and able to do 17 stairs up to my house. My physio is surprised at my progression and I gave a big shout out to you both for the best guidance ever regarding preparation. I am an RN for 26 yrs and I guess will wait until the 6 week visit to find out about knee # 2. Thank you for the great, logical advice.

    I'm 68. LHS hip replacement at 65. Was told I was a model patient. I so hope u wrong. Don't want my rhs replaced. Active and so careful not to stress my rhs. Thanks for sharing.

    Hi Docs, I had both hips replaced during the beginning of Covid, it was a 8 week wait for the second hip. I knew both had to be replaced and I scheduled for both. This was a great experience. Posterior surgery. Recovery was quick and with little soreness.

    Hello Docs. I had my right knee replaced in early November. I will not say never but I can't imagine having my left knee replaced no matter what. The pain that I've endured during my rehab has been so excruciating it has left me wishing I had never had it done in the first place.

    I'm 2 weeks post op left hip replacement and doing fine. I have thought many times how much I needed to rely on a pain free right leg during recovery. X-rays indicate I will need the right replaced, but I will wait until it is giving me a bad time. I enjoy your videos, thanks.

      Ive been on waitting list for total knee replacement here in qc now for 8 months !! I can hardly walk anymore stuck in my living room and with covid they say maybe another 8 month wait !! Whats a person do to in order to relieve pain at this point !!

      @@hudsoncrabappleyohandri8307 I didn't have to wait but I had to use a surgery center rather than a hospital because of the COVID issue. I know what you mean about the pain. I was stuck in the house when I was at home and had a heck of a time at work. I couldn't walk more than a few minutes and I would have to sit at my desk for a while to let the pain subside. I will say I am pain free at this point. In fact about 2 weeks after the surgery and I actually walked a couple of blocks with my wife and our dogs and all without pain. It is so cool. Good luck with getting it scheduled.

    I had double total knee replacement back in 2020.. it's been a long recovery..and just like you said in a video few back one my heal different..I am happy to say I am all most there..one of my biggest problem was lack of movement before surgery.. Thanks again.. Linda Payne..U.S.A

    Although recovery was not as bad as I imagined it to be, I am hesitant because 5 months post-op I have difficulty kneeling and getting off the floor (even though I am short, I still have to get down on the floor to get something that has rolled under the chair/table/bed or whatever) so guess I am in the “wait until it is no longer tolerable” group before I get my other knee replaced - Lynn

      I have had both knees done and have learned a different approach to getting up and down. Although it does not hurt to kneel I find it very uncomfortable. I consider that a small price to pay for walking pain free. When I get up from the floor I make my body a bridge and walk myself up but I am very limber. Maybe this we’ll help?

    Great talk guys thanks. 11 months between total hips for me. Two additional considerations for me. I live in a northern climate with winter snow and ice and I have a hobby farm with garden, animals, orchard etc.. so September was the ideal month for me as summertime obligations are complete and enough healing can go on before the snow and ice hit the ground. September and then again in mid winter? No thanks. Ruin the whole summer? No thanks. The other factor living in a small town; lots of people travel 3 or 4 hours to have hips and such done to Spokane, Seattle, Portland etc.. The stories I would hear of staying a few days in a hotel after surgery and these horrific drives home laying in the back seat.. not attractive... and my small town Orthopedic was at the forefront of anterior approach and has done many thousands of hips in 30 years as a doc and he had my every confidence and I knew that it was only a matter of time until the second one needed replaced so I wanted to get it done with him before he retired. That was important to me.

    I had my second knee replacement six months after my first. That was a good recovery time for me. It is no joke that a knee replacement is stressful for the body. A big issue for me was being able to sleep. It took about three months before I could sleep a whole night without pain meds. I wanted to give my body some time off meds before I had the second knee replaced. Good luck to all, and thanks for the videos. I watched many of them before and after surgery.

    My left was replaced last September an the right 9 weeks later. My criteria was to be rehabbed enough to get off toilet, but before year end when co-pay restarted. All went very well, P.T. was key. These videos were huge help in getting educated on the process.

    I knew going in I would need both knees replaced, had second one 14 weeks after the first. Almost three years after number two. I was 72 years old at the time. Back up to 30 plus miles on an outing on the bike.

    I had a total knee replacement Dec 15,2021. I went to see my doctor a week ago and he couldn’t believe I was able to straighten my leg 100% and bend my knee 110%. I had no therapy as it was in the middle of the holidays and my grandson flew here from Florida for a two week visit. I did all the exercises they gave me from the hospital.
    My knee is doing fantastic! But…… I’ll NEVER do my other knee because the pain was so bad that I’d rather give birth lol I’ve had many surgeries after my car accident to save my leg but nothing was worse than the pain I had after the knee replacement! 😆

      I hear you. My right knee replacement was in 2012. I have lasting PTSD from the way the original surgery had to be fixed 10 days afterwards. It didn’t help that I woke up while still in the surgical suite because I’d been given too little anesthesia and painkillers. Being told by a nurse in Recovery that I couldn’t be hurting THAT much and that others in the room also were post-surgery and didn’t need any painkillers as I moaned, screamed, shivered, and couldn’t speak from the pain was traumatic. I was less than human from the pain and got treated like a drug seeker.
      Nope. The pain from that 2012 replacement and being treated worse than a dog has me very reluctant to deal with the the other knee.

      @@nancymueller6206 so sorry to hear that. I can’t imagine! After my accident and them saving my leg I had close to 50 surgeries. I had 3 a week while I was in the hospital for 2 months just to power wash the galvanized paint chips from my wound so it wouldn’t get infected and each surgery I woke in tears. One time I did wake up after surgery only to hear a nurse say I was too heavily medicated and took my pain pump away from me. I couldn’t believe she treated me like that as I just got out of surgery and it was the anesthesia that was making me sleepy and the pain pumps only give you so much at a time. I felt like I was being treated as a drug addict. So I know exactly how that feels. I personally got myself off all drugs after 7 years of surgeries because I didn’t like the way I felt. Hope you are recovering well😃

      I feel the same, I’m younger and fit. Had an excellent surgeon. Did all the pre-hab and post. Had a great recovery, perfect flexion and extension, but I never want to do it again. Nothing helped with the pain, narcotic did nothin so I stopped them, took Motrin and Tylenol only after a week. I didn’t sleep for 10 weeks. Like literally no sleep, tried everything. 15 weeks out now, have a bit of PTSD. Sleeping better, my 12 hour work day is a bit rough. It is great to have no pain, but I just can’t think about ever doing it again.

      @@nancymueller6206I’ve had one nurse like that in post-op, and it was beyond awful. I’d had my chest cracked to remove a tumor around my thymus. It was traumatizing for sure. The worst part is that there’s no one to advocate for you in the recovery room. My mom is a nurse practitioner, and she’s intervened more than once for me after surgery when I was in such terrible pain. Attitudes towards opioids have only gotten worse since then. Multi-modal pain management should be individualized for each patient. One size does NOT fit all.

    I had such a great recovery with my RTKA that I knew at my 6 wk check up that I wanted that same feeling on the left. I had severe bilateral arthritis with valgus deformities and my right knee felt so god that I was ready to do it again. I don’t even think the pain was as bad the second time around. I am 3 wks post op and so happy I decided to have it done right away. I was 3 mo and one wk when I had the second surgery. Although my blood loss was only 50 ml for both surgeries my blood count took a bigger hit the second time around. My Hgb dropped from 13 to 9 causing me to be a little more fatigued, but it will recover and I’m not concerned. I also have iron deficiency d/t being vegetarian although I do eat fish and I have to inject B12 q 2 wks. But this isn’t new so I’m not worried about it. Just looking forward to being able to throw my cane away and get back on the golf course, maybe even the tennis court. Heck I’ll just be happy to be able to walk normally again. I’m too young to be this old. Ha , ha. Thanks guys for the great information along with the entertainment!❤️

    Had BTKR in 2016 at the same time. I waited untill I could barely walk-L knee was bone on bone, R knee had fused & couldn't be straightened. I can't imagine going through that twice! The nine days at the nursing home was the worst & I will never walk , willingly, thru a nursing home door again. The food was uneatable & I had to beg for ice & pain meds. I worked hard in PT & returned to work at full duty in 12 weeks. (I am a truck driver)

    I am 6 weeks out from rt TNR. I am ready for my left one. But I teach and have not missed any work. Surgery done over December holidays. Now looking for next opportunity in my schedule to get the other done.
    No pain, great recovery, had outpatient at a surgery center and walked out! I worked hard at physical therapy.

    My neighbor had her knee replacements four months apart and she did great! I am 3 weeks post op with mine and I can’t even think about the getting other one done at this point. But that knee without surgery yet has really been difficult being the “good knee” during these weeks. Regrettably I put off my first knee surgery when the other knee rapidly got worse. If both knees are heading toward replacement surgery, I’d advise do it sooner than later before the second one isn’t healthy enough to carry you through surgery recovery.

    3 months into a new hip..all good..except the other knee had ACL ACL repaired 20 yrs ago..is now giving me grief...not sure if it's just from compensating...guess I'll ask orthopod next visit?..dont want to go threw a knee replacement...hip was easy...love your videos!

    Can you talk to some orthos in the US who make patients verbally agree to schedule the second surgery within 6 months of the first one? I have been told by several patients the MD rationale is “There is evidence that having one good side and one bad side will compromise the joint prosthesis”

    I have my first knee schedule for next month, and am pretty anxious about the recovery since my other knee is also trashed. I’m definitely not a candidate for both at the same time. My surgeon suggests to do the left side 3 months after the right. Joy!

    I had my left hip replaced in Sept 2021. I have had almost no pain since then. I was walking the next day. I got rid of the cane a coup,e weeks later. Not only my hip painless now, I don’t have as much knee pain in my old, reconstructed left knee so I can put off it’s replacement for a while longer!

    I am terrified to have my other knee replaced. Had a TLKR in Aug 21, and in pain every day. On to second opinion and still no answers. I am 125/0, so no problem there. But my quality of life is zero. Why, I’m curious, don’t Doctors want to MRI the knee? I have pain with walking, and at the tendon behind the knee. All I have had done by both Orthopedic Dr’s are knee X-rays, and then I am told every time “the knee looks perfect” and to give it time. Also, a big deterrent is that I have NEVER felt such agonizing pain. I literally can’t describe how horrific it was post op. I wish (no offense) that orthopedic surgeons could go into a knee replacement simulator, much like pilots can do for flying, and feel the intense pain. Because the cavalier and dismissive way I have been treated thus far is a sad statement. I am at a loss as to,where to turn. 3rd opinion? Please, do a video on outcomes that are less than good, and advise us what questions to ask, and what tests to request.

      Hi Trish. So sorry to hear that. An MRI is not useful due to the scatter caused by all the metal. Dismissive is no good. Sometimes there is no identifiable cause.     I had both my knees done at the same time. Yes it was an undertaking but for me not as bad as you would imagine. My doc wouldn’t have done it if he didn’t think I could handle it. He said I was the perfect candidate being younger (54) and in great physical shape. I had no comorbidities and wasn’t on any medications. Prior to my surgery I worked on my strength, including my upper body as to help me sit down and get up from a chair or the toilet. Sleeping was the hardest because I’m not a back sleeper and I had to sleep on my back. I worked on my physical therapy every day for at least 2 hours as I was rehabbing 2 knees. It was a good decision for me but obviously not for everyone. It’s been 10 years and my knees are going strong,
    If you and your doctor think it is the right thing to do I say make the commitment and do it. Also don’t slack on the PT and don’t stop the pain meds too soon. Good luck!

    Im having my second hip done on March 1st which is 18months after my first but i had complications during surgery which has delayed the 2nd. Keep up the great videos guys. Love them

    Hi Docs - Three months sounds good. I still have a long way to go for my first knee (weight reduction) so by the time I get to one, and recover, I want to do the next one, also, bone on bone, in a timely fashion. And I liked the fact t hat you talked about the impact on the care giver. I can do a lot of prep before each operation to make it easier on my hubby, but trust me, by t he time both knees are done and recovered my hubby will be exhausted (old men are better patients than caregivers) Tee Hee.     I had my Left hip done September 7, 2021 and was scheduled to have the right hip done on December 7 but because of shortages of staff they have canceled elective surgeries so my second hip has not been done. However I’m very thankful that it wasn’t, I’m having so much pain in my left hip, It’s painful to put weight on it. Then just last week I fell down our back steps, most of my pain from the fall was on the right side but also having pain on the left hip area and my tailbone area. I did make an appointment to see the Orthopedic just to check things out but I’m having to wait till next week. At this point I don’t want to have the right one done.

    Well, I had a total hip replacement in April 2022. I am going to have my left hip replaced in less than two weeks. I knew I would probably have to get the other hip replaced. Gradually, I couldn't put full weight on the leg and after discussing it with my doctor, I scheduled the surgery. Hoping I didn't wait too long.

    I watched my husband during the recovery from total knee replacement. I recommend that spouses take a vacation and go away during their SO's recovery time if they believe they themselves will ever need a knee replacement. I haven't recovered from HIS trauma yet. I'm close to needing it but with covid around, who knows if my date will be after my 6 feet under date.

    I am 11 weeks after total knee replacement and I am absolutely not ready to get the other one done. Although I have pain from bone on bone in the other knee my replacement is not ready to carry the load while the second one heals.

    I thought I would never look forward to having the other side done but I had my hip done three months ago along with my right knee replacement eight years ago. I now have bone on bone on the left knee and even though I’m not looking forward to it it’s at the point of no return.But I need to if I want to walk without a cane and pain. Thank you for your input as always you two are great

    I'm five weeks post TKR left knee. My house is on four levels, so lots of stairs. The added stress to my right knee is causing more pain than either knee ever had. I'm trying to avoid getting the right knee done any time soon for various reasons. I'm hoping the right will settle down as the new left takes on more of its share. Any comments on this? Love your videos. They are now my go to 🙂

    I had my knees replaced 3 months apart and one thing I found was that I could still rehab both knees at the same time. I was still rehabbing the first knee when the second knee came along. I did find that the second knee was much harder and longer to rehab. Each knee is different!

    Hi Docs I had my first knee done on Aug 12th 2021 and I needed to have the other one done because of bone on bone and I had so much pain with the first one I said oh heck no way I'm going thru this again but when I saw my surgeon in November I was so much better I told him im biting the bullet so put me on the list lol so I had it done on December 9th and I'm still having some pain and like I told my surgeon it's like child birth you forget the pain you go thru but then when your in labor again it all comes back with a vengeance 🤣 . I'm happy I only have 2 knees and now they are done and I'm doing great and happy I did it. Thank you for all your great videos and humor and your cuteness helps to. Hugs from Maria in Massachusetts

      Isn’t really as bad as child birth or are you exaggerating? I’m having my first knee replacement next week and you have frightened me now. I hope it’s not that bad. What did you find more difficult, the pain or recovery. How long did it take you to feel better?

      @@nf2885 sorry if I scared you 😟 I didnt mean to frighten anyone it's just that its painful and everyone has their own pain threshold and it's just how mine was . The first couple of weeks were very painful for me and I've had people tell me they didnt have the pain I did so I only used my experience when I sent this email. I'm hoping you dont have much pain when you have it done as I did and it's worth it all to have the surgery . Again I'm so sorry and I'm sending lots of hugs to you and my best wishes for your upcoming surgery .

    I have had both knees joints replaced. At the time of the first one, the right one, hurt the most. After what I thought was a short recovery, and the pain was gone in the right, I realized how much the left hat been hurting too. At that time, I wished I had done them both at the same time. However, after thought, that would have lengthened my recovery I feel. At the time of the right replacement the left felt ok until I experienced the pleasure the right operation had provided, and at that point realized the pain in the left. Having had the experience of recovery of the right, I feel made the recovery of the left even less of an ordeal. I waited about a year, but could have done the left sooner if not for other plans. Personally I feel thee longevity of the procedure keys heavily on the PT or strength one maintains in the quads and hamstrings. This the surgeon explained before any work was done. I PT my legs twice a week and that works well for me for a fairly long period of time.After the right was done, even though it was painful some, I pTed the left prior to the procedure to the left. THAT seemed to really accelerate the recovery of the left.

    I am 72 and getting my first knee replacement in a couple of weeks with the other sometime soon after. Thanks so much for the information in these vids. It has really helped, bless you both :) I have some concerns about using outside apartment stairs right after surgery for getting home and for up and down going to therapy. My ortho guy seems hell bent on no in home therapy at all and frankly, I am scared going up and down stairs now. Both knees are bone on bone so I have to hang onto the stair rails. I have been watching exercise vids and have a recumbant bicycle so I have decided if I am too scared to man the stairs the first week, I ain't hehe and I'll exercise at home.

    I had a total knee replacement on my left leg on 12/8/21.The pain from the first knee replacement is still on going though I have ramped up my outpatient PT. It was not a walk in the park . My surgery was performed by a robotic arm assisted machine.

    December 2023 a lot of news about titanium hip replacement hardware failing due to manufacturer's design. Can you please comment about this, including what to ask for when considering hip replacement? Are certain maker's products better than others? Are the substandard products still on the market in the US?

    I'm pretty hesitant to get my other knee done.
    I had a TKR on my right knee in October 2020, on my way from surgery to recovery my heart rate dropped then stopped, and I was unresponsive. I suffered a total heart block. I spent three days in the ICU, so I'm not in a big rush to book the left one.

    Ugh I'm 3 weeks in from my first knee and have my good and bad days. I'm still waiting on TDI so in 3 months I'm not so sure but that may be good to keep the disability income flowing. Income is key too. I'll see how my recovery goes because my right knee is just as bad.

    I'm heading into a left hip replacement in a month. My right hip was replaced Dec 2020.
    They would have replaced the left hip sooner, but I ended up with unexpected quadruple bypass in June 2021. Soooo, I'm up to 2.5 mile hiking a day again. Wieght lifting, swimming 2x a week. I lost 35 pounds before first replacement. I lost 50 pounds after open heart surgery. So I'm in great shape for my 2nd hip replacement. Looking forward to not having pain when hiking, walking, sleeping. Lol

My physical therapist asked me what had happened...that she would see this kind of injury in a rugby player or football player....dealing with auto-immune is interesting... MRI L shoulder 1. Severe glenohumeral joint osteoarthritis. small glenohumeral joint effusion. 2. Small effusion and moderate osteoarthrosis at the AC joint, 3. Fraying of the labrum. Possible tears at the superior, inferior, and posterior aspects of the labrum. 4. Low to moderate grade partial thickness intrasubstance insertional tear at the distal end of the supraspinatus tendon,. Infraspinatus tendinosis. Grade 2 atrophy of the teres minor muscle. Osteophytes at the glenoid. The largest cyst at the glenoid measures approximately 2 x1.8 x1.3cm. Surprisingly no significant pain, stings a little ... definitely gets grumpy when worked with and I have about 135 degrees arm up to ear... haven't had MRI of R shoulder but considering the L wouldn't expect to much different... definitely a little more clunky though....Sigh.

    Post-Op infection! I am scheduled for a full Hip replacement soon. Pre and Post Op steps, to a lay person, seem extreme. After the outer wound heals, how big of a deal is this?

    Hi Docs..had the left knee done and never thought I would have to get the other one done. I’m booked in September(2 years from the first surgery) because I was given a cortisone shot beginning of January and my Ortho prefers to wait 6 months instead of 3 months before surgery. How long do you guys prefer to wait after cortisone shots? Interesting fact about the blood taking 8 weeks. Thanks again for your videos.!😊

    I'm due to have one left side hip replacement and the post op doesn't worry me its the op that is crippling my mental health.
    I have an excellent surgeon but I have never had an op before so really scared so just the thought of having the other side done would definitely not be something I would be rushing to have done eeeek

    I’m 2 weeks out from my 1st TKR. My surgeon told me my X-rays on my other knee don’t show near the arthritis progression as there was on the one I just had replaced, and am maybe looking at several years before it will need replacing. Is this normal?

    This weekend is 46 years since
    I went to hospital for surgery to put 3 pins in my hip aged 13. They are still there and I have no plans to have them taken out. At that time I was not happy about them shaving my balls before surgery. These days I would not mind so much.

    2 years on and no hurry to get the other one done. It still isn’t great 85-90 bend even after MUA. I’ll wait until there’s no other choice.

      Yup stiff knee - tka was the 4th knee surgery. Lots of scar tissue. Looking at debridement

    My THR is coming up soon. Posterior incission. Some people are telling me that I will be back to normal within a month. Doesn't it take more like 3 months for my body to heal and walk and do normal activities?

    Hey docs, great channel, believe I’ve watched almost every video! I’m near end stage knee OA, planning for TKR in 4 months. Unfortunately, I just sprouted a hernia on one side, which will require laparoscopic repair. Question: how long after the hernia repair should I give before undergoing the TKR?

    Getting my right knee replaced was so extremely painful and hard to get over that I’ve always said why would anyone go through that again? Painful to live with, but I can’t get the courage up to do it again.

It’s been five years since my knee was replaced, and I have better mobility and much less pain since it’s healed. But true, it was brutal for me. Many thanks to you two doctors, I love your videos.

    Stressful 👍..yes sir reeeee , 3 days ago I had TKR and you couldn't pay me to do both at the same time...I had my right hip done 4 yrs ago and just now my right knee, the trauma from when the nerve block wore off was almost too much for me to baer ....I wanted to put a bullet in my brain, and I messaged my Dr with concerns but he was just an ahole...not to mention he didn't look at my allergies, Im allergic to nickel and he was going to put a Dupuy in my body but the anesthesiologist caught it so my surgery got put off for a few hours...I won't be going back to him when the other side needs to be done...good luck to all going to surgery and all I can say is be as prepared as you can be for the pain that will set in when the spinal and the nerve block wears off...
