About 2 years ago I did an mri of my lumbar, the results were: A grade 1 retrolisthesis of L5 on S1 Modic type II, large inferiorly extruded central to right paracentral disc protrusion with near-complete obliteration of the spinal canal at the L5-S1 in part aggravated by ligamentum flavum hypertrophy at the inferior endplate L5 level, L5-S1 severe central spinal stenosis at the superior endplate to the midportion of S1, there was additional severe spinal stenosis. There was deformity of the thecal site displaced posteriorly towards the left with moderate right and severe left foraminal narrowing with impingement of the exiting left L5 nerve root. The overall size of the large disc herniation was 2.9 cm x 1.4cm AP x 1.7cm transverse. Two rounds of PT and a lumbar discectomy later (1yr 3mo post-op) and I get another mri: L5-S1, there is disc bulge with central disc extrusion with minimal caudal migration. Mild central canal stenosis with moderate left neural foraminal
stenosis. So while the surgery did improve things, it didn't end up being what my surgeon promised me. I want to make sense of what can be done now, I'm getting a lumbar facet injection, I need to get straight talk on this, my options are limited at present(insurance) so a second opinion is out. I need to know so I can advocate for my health properly. Please Bob and Brad, help me understand can I get rid of the nerve pain, can this heal? Will I need surgery again? HELP!

At 11:28 you mentioned "stenosis", what is that? My MRI reading mentioned "spinal canal stenosis" and "recess stenosis" as well as "annular fissure". What do those mean? I know I could Google that, but I trust your articulate responses more than the interwebs. Thanks! (I had the microdiscectomy and the same disc has re-herniated (within 6 months). This cleared up so much about my MRI, and was so helpful! I was terrified because I now have a sequester, which I thought was as bad as it could. I'm so glad you showed me otherwise.)

Hi there, long time watcher of your channel which has helped a lot. I do have 1 question. commuting to work, I have an undiagnosed back problem which I think is a protruding disk. if I stand from seating, then I feel quite a bit of back stiffness, which isn't usually bad pain so long as I get up in a slow controlled way. second day of work and I am now having a day off to recover. The work is light and the company is working with me to find me something so it doesn't happen again. Now to the question. Would it be beneficial to get an electric bike and travel to work that way?
I don't drive because of the strain on my back from driving, also travelling long journey on the bus triggers the pain too.

    Hi Bob & Brad
I've had chronic pain in the middle of my lumbar, out to my right butt, down the back of the right hamstring, and into my right foot. Everything since May 2022. I start to worry if I'll ever get back to feeling pain-free. I have 2x disc bulges, degenerative changes, and an annular fissure from an MRI I got in September as pain didn't get better... Annular fissure to the right in my L5-S1 disc. The pain came as I did a heavy set on deadlifts and suddenly felt a sharp pain. Been in pain ever since... I've tried chiropractic care, and have also done physiotherapy. Both without luck. Also have done core strengthening and McKenzie press ups. No luck there either. I'm about to give up on life, but I REALLY DON'T WANT to get surgery. I don't have bowel problems so my doctor says she won't even considerate surgery, as it's only done on people who have bowel problems, like holding urine in, etc.
    Cheers from Denmark

I suffered a bulge 13 years ago that took almost one and half years, now two months ago I experienced disc extrusion and that's painful like hell you can't move your head or hand because of pain. After MRI I met doc and he told about surgery I was in shock, then I met a therapist and told him about the case he assured me it will be done without surgery. Now after 1 & half month alhamdulillah I am walking sitting and I feel 70-75 % better than before.
Here I would like to tell everyone that even you are going with surgery your 70% problem will not be solved without therapy.

6:24 Hi from Italy, what's about Schmorl's hernia? My one happened some time after a vertebral body fracture (L1 upper side squashed), when the affected disc (T12-L1) herniated into the bone. Even it cannot provoke me pain directly by itself (no nerves are located into vertebral bodies) it reduced T12-L1 vertebrae space more than how much it just changed by vertebral body form modification (lower at its upper-front side and higher at its upper-back, further such a center hole). I have not realize yet if anulus is present at lower/upper disc parts too, so if nucleus went into L1 vertebral body "drilling" anulus or if it was inevitable just because no anulus part is present under/over the disc (I am suppoaing that vertebral body bone may does this function by itself). So, I do not know if my disc will become inevitably dessicated or if it just may work as before the fracture. I was an atlhete until 6 month ago and I am hoping to do it again. Sorry for my grammar mistakes and I hope that you may do a video about Schmorl's hernia (fracture/no fracture related both). Thank you.

I'm 31, I get diagnosed with a lumbar herniated disc 10 months ago, I just took the medicine my doctor gave me for the pain for two months. After that I had just a little numb sensation on the leg (sometimes). Now I am having the same pain from 10 months ago, and I wonder if I should just take more medicine or get a surgery?? What's your best opinion doc's?

What kind of nerve flossing do you do? I have bulges between c4, c5, c6, c7. The worst is between c5-c6. My neck, shoulders, arms, and hands hurt

Me too. Just had an MRI done disc bulges in c3-c4, c-4-c5, and c5-c6 also feeling nerve pain that goes down into my hand. Did they say 13% chance of it healing on its own? What are you guys doing? I have no idea what to do. My doctor just pretty much left me with a diagnosis and no suggestions.

I had 3 cervical discs pushing on my spinal cord, my arm was weak and numb. Are you saying eventually I might have healed on my own? I had a 3 level fusion. My c3-c4 was herniated but wasn't pressing on the cord. The herniation is abutting the cord and hasn't moved in many years. I wish it would heal.

Recovering from a (incomplete) Cadua Equina Syndrome now 6 weeks after surgery of a sequestred big L4/L5 disc hernia.. The bowel and blatter area was affected but not terminal damaged so it's ok (but still a little numb), and the right calf and foot is still kind of numb and so, but improves.. Walking quite ok with a crutch for support (mostly at home without), so it's getting better.. I guess I was lucky to get help in time. ;) But please do a video of that important injury.. !

So your sequestred disc caused Cadua Equina? I thought Bob and Brad said it would heal better sequestred disc.
How long from your herniated disc did it take for Cadua Equina to happen?
I just found out about this syndrome and been in pain due to a slipped disc for 20 yrs or more and now SO scared of getting this syndrome that I can't even sleep.
