Apparently they didn't want to nuke the codes again so they just made it so that everytime he teleports it counts as a redeploy (which makes him kinda useless in SSS)

Yes, but this is getting patched later today. These status effects and damage on deploy will only apply when they're first deployed and not on skill use after the patch.

サーミローグのデバフ現象に「配置時N%のHPを失う」というものがある
→ウルピのs3は発動時と解除時に「配置」扱いになる模様
→ウルピの残りHPがN%以下だとs3で「配置」された事で即死する
って感じかな？

From the description, Ulpianus' repositioning is considered a redeploy.
This IS probably has injury trigger that made every operator lose HP on deploy. That means Ulpianus is practically losing HP everytime he changes position. 😭
