HDPE Plant Explosion and Fire

On September 13, 2010, a hexane release occurred in the HDPE plant at a petrochemical company, which resulted in one fatality and four injuries.
ISEC investigated about the causes of accident and produced the safety video and animation.

Might have been a corroded pipe seeing as it was a common occurrence, or maybe an emergency pressure valve not properly directed to a flare tower or safe place to vent…who knows?
