  • Midline posterior fossa fluid collections in adults usually represent benign congenital enlargement of the the cisterna magna. Arachnoid cysts of the posterior fossa are also possible and may be indistinguishable by imaging. Think of arachnoid cyst if the fluid is asymmetric and produces significant mass effect.

Thanks for the excellent lectures , please share more videos of neuroradiology and advanced mri imaging ( eg . CSF flow studies , defecography , MRS , prostate etc ) . Thanks much

Hi I have MCM and I'm looking for a brain surgeon to fenestrate the posterior fossa cyst. I live in Colorado but am willing to go anywhere for treatment as I'm losing my vision. I'm 59 years old and have been suffering for many years with debilitating headaches. Thank you so much!

This is sad that nobody seems open to doing this. I managed to speak to a neurosurgeon within Kaiser who said he has done it but thinks mine is megacisterna magna. Would love to communicate with another patient in A similar situation. Maybe you can give me your contact info. Mine is 3.5*5.5*7 red, swollen eyes and terrible headaches and loosing balance/double vision.

Hello, interesting video! I have a (possibly) related personal question:
An incidental finding of an arachnoid cyst on my posterior fossa showed up on a brain CT scan I had back in 2013.
Oct 22, 2020 I had a "normal" brain MRI scan showing a mildly prominent cisterna magna and partially empty sella but no notation about arachnoid cyst of posterior fossa.
My question: is it possible they initially mistook my "mildy prominent cisterna magna" for "an arachnoid cyst of my posterior fossa" or vise versa? I think you mentioned these 2 anomalies being nearly indistinguishable from each other in this video...
I have a history of migraine with visual aura and other troubling symptoms, which is why the MRI (& MRA) was done, but they said my results are normal...or some "variant of normal" which is good news I guess.

Yes. Sometimes it is impossible to tell a mega cisterna magna from a midline arachnoid cyst.

What about a csf flow study? They just found a 3.5*5.5*7.5 one in my brain and the drs are saying they don't know if megacisterna or arachnoid cyst. How would they attempt surgery in some cases if there is no way to tell the difference? I am having trouble balancing and terrible headaches and went to ER three times before this mass was even discovered.
